EE was attempting to use pry bar to lift up shaft on roll press roll. As EE attempted the pry bar slipped and EE fell on right side, EE placed left arm to catch self and felt a sharp pain in left bicep.

EE was switching cement lines as gate was stuck, as EE was switching, gate got stuck halfway open which caused cement to spray out and into EE facial area. Causing cement to enter EE's eyes.

EE was replacing drive chain on bucket elevator. As EE was attempting to pull chain together to pin, EE's hand slipped and caused EE to hit bottom of guard causing a small laceration on left hand ring finger second knuckle. Requiring 5 stitches.

Contractor received an injury while power washing the inside of a railcar. While repositioning self the contractor slipped and accidentally squeezed the trigger on the power washer wand. The wand was aimed just behind the steel toe of EE's boot. The water jet cut through the boot and pierced the skin of the individual.

While moving a punch top drum, Employee received a cut on their right thumb from a sharp edge. After the thumb was cut, Employee's thumb was glued for closure of the wound, hand and arm were placed in a splint to stabilize and keep the thumb from moving. Employee was prescribed Hydrocodone, Acetaminophen, and Cephalexin, they also received a Tetanus shot and T-Dap Vaccine.

Employee was talking with another employee in welding shop and other workers were trying to free a steel liner from the Finish mill door. As they struck liner with sledge hammer, a small piece of scrap metal shot off and hit employee in right leg below knee cap. Injured employee was 8 feet from other workers.

Affected employee was assisting in the removal of scaffolding from the Pre-heater tower via pallet jack. Whilst loading the scaffolding on to the elevator, another employee slipped on snow/ice. This sudden movement resulted in the shift of material thus trapping the EE's left hand between the material and elevator wall, smashing the fifth digit (pinky finger) on the left hand.

At 12:44 there was an explosion in the quad shredder. One employee was injured and significant damage the facility occurred. The employee received 3 staples for a laceration on EE's head and was given pain medication and antibiotics for two holes in EE's eardrum

The employees began to open the inspection port when pressurized cement unexpectedly began flowing outward. One employee attempted to quickly reinstall the port, and as EE pushed the port inward, cement began to spray into the employees' eyes. The employee was struck by cement in both eyes and was placed on restrictions and prescription medication.

Continental Cement employees had left a jackhammer bit on a raised platform and left the area. A contractor was working below when the jackhammer bit fell through the raised platform and hit the employee in the side.

EE was rotating the motor manually of the fan pulling the belt and the pulley when cotton glove was caught between the belt. The momentum pulled the glove and cliff pulled hand back. When EE pulled hand away, it removed the hand from the glove; left thumb had been lacerated a portion of the thumb was lost.

Employees were inside the coal mill replacing the rubber sleeve. The employee's left leg was standing on a lip and right leg was kneeling on the roll in order to repair the rubber sleeve. When the employee lifted the sleeve, EE's left knee "popped". Employee diagnosed with a lateral collateral ligament sprain and had surgery on June 13, 2019.

On 6/11/2018 ee was performing mobile equipment inspection on the cat 262C skidsteer. EE knelt down to check the cab filter when Left knee popped and ee felt a slight pain. EE was seen by a physician who recommended ice and OTC meds for pain. On 9/4/2018 ee noticed pain in the same knee and was seen by a physician who ordered an MRI. Surgery occurred on 10/4/2018.

Employee was changing a sight glass on a drive. When on the east side of the platform on the VSK bearing pedestal, EE hooked their left foot on an RTD Cable going to the outboard bearing, catching the wire with a foot and falling forward, landing on the left knee. Employee was prescribed 3 weeks of physical therapy by Ortho Surgeon on 10/2/2018.

EE was feeding the system when EE was signaled by co-worker to come upstairs. EE stopped the skid-steer, set the parking brake and exited the equipment. The bucket was full of material. When EE stepped down from the skid-steer, right foot came down on a piece of material. EE came down on it in such a way that EE's weight shifted and EE rolled ankle.

The employee placed small metal cylinder onto a vice grip and was using a handheld grinder to cut a notch into the cylinder. The grinder became wedged in the cut of the cylinder, jumped out, and cut the employee on the left hand. The employee bandaged the wound themselves and called the Safety Department. The employee has received six stitches.

EE was operating the hydropulper and obtained a laceration on left middle finger. A 55 gallon drum fell over while being rolled on a hydraulic lift. EE used a shovel to catch the empty drum and it fell again. After a couple attempts at grabbing the drum with the shovel, ee instinctively reached for the drum and cut finger on the sharp edge of the drum.

An employee was walking a 5-ft ledge of loose gravel to check the delays on a shot line when the employee lost footing. The employee fell against right forearm, which jarred shoulder and back. The employee requested medical care and cannot complete regular duties. The employee will be reviewing MRI results with a doctor shortly.

On July 19, an employee was walking down a belt ramp when EE stubbed boot toe on the metal grating and twisted right foot/knee outwards. The employee did not require medical care. On July 31, the employee stated knee had not reduced swelling and requested medical attention. An MRI has been scheduled. The employee cannot climb and squat.

On June 27, the employee stated right eye became irritated, the employee flushed it out, & continued on the task. Did not report the incident to supervisors. On June 28, the employee's eye became inflamed & irritated. The employee reported for work on June 29 & requested medical attention. The employee was diagnosed with a chemically burned eye & prescribed eye drops.

Two employees moved the wagon through the narrow pathway with a 500 lb tail pulley. The bottom of the wagon struck a bracket protruding from the floor. While pushing the wagon to free it, the tail-pulley rolled off wagon, Employee 1 fell backwards into the roll press door, and pinned Employee 1's left ankle between the pulley and the removed roll press door.

The employee stepped off the ladder onto the berm area and rolled the left ankle. The employee sought medical attention and was given a rigid boot and restricted duty for one week.

Employee had cut a 2 15/16 shaft 36 1/2 long and was placing it in the 4th rack approx. 49" from the floor on top of two other pieces of pipe when the employee set the shaft down the two pieces of pipe separated allowing the 36 1/2 long shaft to fall through and strike the employee's left foot. The employee's left foot had an impact tear on the outside bottom of the foot.
