Employee was injured when EE fell through an elevated platform in a restricted area of the underground mine after EE climbed over a berm to access the restricted, inactive area in violation of the mine's rules and EE's training. The employee was not authorized to enter the restricted, inactive area and was not performing any work activity at the time the accident occurred.

On 3/5/22 employee experienced shoulder pain but only reported it two days later. The doctor found it was not from work. At employee's request, a second doctor opined that work was the prevailing factor. Employer requested an IME and on 11/4/22 we received notice that the third doctor would likely opine work was the prevailing factor. Thus we are now submitting the 7000-1.

Employee was assisting with the replacement of an outrigger on the mechanical scaler, when the hydraulic cylinder that was being supported by a block of wood fell from the block striking the employees finger. The employee was seen at a medical center and released that night.

Employee was modifying the flashing for a new belt. During the process, the employee needed to remove a piece of flashing. The employee began cutting the piece with a box cutter, the cutter slipped and the employee cut their left arm. The employee immediately returned to work after care the same day.

Injured worker was using 1' impact to remove cutting edge hardware (bolts). Worker reached around to push bolt back into place and accidentally engaged the impact causing laceration to palm of right hand.

Employee was adjusting deflector gate on 1 South screen on Monday 4/10/2017. On 4/12/2017, EE noticed that EE's abrasion was more painful and started to swell. At 2:00 pm I went our clinic with employee to be seen by our doctors. Doctor advised employee to continuation of EE's anti-biotic and Dr wants EE to be seen on 4/14/2017 to check EE's infection.

Employee was adding a drill steel to the drill so they could get the desired depth for a bore scope hole. A piece of shrapnel hit employee in the arm so employee was taken to the ER. Employee was treated and release back to full duty.

Detached a plugged 3' long 3" to 2" reducer from our concrete pump. Stood vertical to unplug, when tapping reducer on the ground to remove concrete, the concrete shot up the pipe striking the employee in the face, forcing the employee's goggles up and covering the employee's right eye with concrete.

Employee had removed grate to cement hopper and placed on top of B 50 pump. While attempting to replace grate employee reached out and began pulling the grate back into place. Employee's footing began to slip and while attempting to catch himself with his left hand the grate came down and struck his left fingers resulting in fractures to four of his fingers.

Employee was working on the belt scraper in slope four, the belt scraper is located on the head roller. The shaft had walked out of the belt scraper, so the employee hit the shaft with a hammer to move it back into place. When he did, employee felt something get into his left eye. Employee was seen at clinic and was released to full duty on 08/19/2015.

An employee on the ground removed the spreader pipe and tie rods from the left leg causing the set to shift downhill approx. 2 1/2' The moving set struck the employee (in the man basket) causing contusions to head and face area.

Original incident was reported to company on 4/16/2010 but not MSHA reportable/lost-time until 9/14/2010, thus the submission of the report. Incident occurred when climbing down from ladder when his foot/hand slipped off of handrail. Employee caught himself on safety chain on separate piece of equipment parked next to loader.

On the 29th of Sept employee was in a manlift with co-worker loading faces. Co-worker was running the controls as they were going up the face. As they neared the top the operator didn't take into consideration that injured is a little taller & on the high side. He raised the lift too high hitting injured's head and injuring his neck as a result.
