Employee was reaching into a tool box and came into contact with an exposed blade. Employee cut their finger and required stitches.

Conco Quarries · Fall to the walkway or working surface

EE started pump at remote quarry location, turned, homeless man (been there before) was right there. Man grabbed/slammed EE to ground, ran away. EE's wrist injured as EE tried to break fall. Dr diagnosed as left wrist pain with restrictions still allowing EE to do normal duties. Follow up x-ray was taken, revealed small fracture/bone chip in wrist. The police were called.

Conco Quarries · Over-exertion in pulling or pushing objects

Pulled hydraulic oil hose until reel locked-made swinging motion to toss hose on top of excavator. Felt twinge in upper left arm. EE got on excavator to loosen fill cap with wrench. Cap was screwed tight-so EE pulled on the wrench with both hands-felt another twinge. EE used different wrench, was able to loosen fill cap. On 10/4, EE requested to be checked by doctor.

Employee said they have had numbness & pain in both hands/wrists for some time and had been going to a doctor who diagnosed it now as Carpal Tunnel.

After 34 years of working as a shop mechanic, EE was having issues with hands. When ee went into family doctor, and subsequent testing it was determined that ee has developed carpel tunnel syndrome. It has now been determined since it was filed with work comp on 5/8/17 that at least partly through time working as a shop mechanic is part of the cause.

Inside 1516 Andreas Rock crusher working on loose liner on curtain inside crusher. Used 3 lb hammer to beat on top of liner to loosen it up from material buildup. As liner loosened, hit it from bottom to knock it out and a piece of metal slivered off liner and went into right bicep.

EE has repetitive motion pain in his hands. We have been working with him all summer. He has completed two operations on both hands. He was off 7 days so far but will add more. Surgery on Sept 28th - this illness has been long and painful.

Company notified 3/7/06, but was not considered an occupational illness until 5/10/06. a repetitive operation of a yard loader. Swelling and cramping in right and left hands. Has been a miner problem for several months/years
