Employee was driving a Superior Bowen's (Contractor) water truck down the slope into mine and lost control, impacting a dome out (located at 2N- 0 E/W) at the bottom of slope resulting in ramping up of truck over and through to other side.

Taking hose off of packer while pressure was still up. Hose disconnected and hit employee in face blowing employee's safety glasses off causing chemical grout to blow into both eyes.

Employee was performing maintenance on a crusher. A chain used during the work shifted, startling the employee causing EE to lose balance and fall. Impact of the fall resulted in three stitches to EE's elbow.

A record rainfall of 6.14", the largest amount of rain in one day since the construction of the Stamper Mine portal, resulted in the mine's intake lake releasing water. Water traveled down the slope and into a localized area of the mine. We do not believe that this event is "an unplanned inundation of a mine by a liquid" and are submitting the 7000-1 at MSHA's direction.

Employee was assisting with the clean-up of a conveyor when EE reported EE felt hot and dizzy. Employee cooled off in air conditioning and drank some water. After resting for around 30 minutes employee was still complaining so emergency services were contacted. EMS transported employee to an emergency room where EE received IV fluids and was released to full duty the same day.

EE was helping two other employees tension cable bolts using a hydraulic tensioner. The cable was bent and had bound inside the tensioner. A 4 lb hammer was being used to the knock down on the top of the tensioner as EE pulled downward on the tensioner handles. On the 3rd or 4th hit, the hammer deflected off and struck the employees finger.

Employee was loading anfo into one of the top holes of a heading being loaded. He was trying to insert the hose further into the hole by wiggiling it. While doing this the hose became caught. As he tried to free the hose he knocked a rock loose which fell and strugk his left forearm. The cut required stitches.

Employee was exiting his Toro piece of mobile and caught his left hand pointer finger in the door. The pinching action resulted in a laceration that required 7 stitches.

SHOTCRETE FELL ONTO THE INTERSECTION OF CONVEYORS S-2 & S-3, A PIECE FELL APPROX. 15' X 30'X 6' THICK. DAMAGE WAS EXTENSIVE TO HEADSECTION OF S-2. THIS FALL TOOK PLACE OUTSIDE OF NORMAL WORKHOURS, NO ONE WAS IN THE MINE AT THE TIME. THE FALL WAS DISCOVERED ON MONDAY MORNING. NO ONE WAS ALLOWED TO ENTER UNTIL THE AREA WAS SECURE.

EE WAS LOADING A ROOM THAT HAD BEEN MECHANICALLY SEALED AS WELL AS HAND SEALED. A ROCK THE SIZE OF A BASKETBALL CAME LOOSE FROM THE FACE AS HE WAS LOADING AND STRUCK HIS HAND. IT LEFT A RED MARK BUT HE DID NOT REPORT IT. THE FOLLOWING DAY THE HAND WAS SORE AND SWOLLEN. HE CALLED IN ON THE MORNING OF 06/07/04 AND WAS TOLD TO GO TO CLINIC.

EE WAS OPERATING SKID STEER, CLEANING SPILLAGE FROM IN FRONT OF THE DUMP HOPPER. A EUCLID R-40 ARRIVED AT THE DUMP HOPPER TO DUMP. THE EUCLID R-40 DRIVER PULLED FORWARD AND STARTED TO BACK UP TO THE HOPPER. HE DID NOT SEE THE SKID STEER AND BACKED INTO IT, THROWING EE FORWARD AGAINST THE SAFETY BAR. DAMAGE TO SKID STEER WAS MINIMAL.

EMPLOYEE WAS MUCKING OUT A ROOM. WHILE TRAVELING FORWARD THE EQUIPMENT STRUCK A LOW SPOT IN THE ROOF WITH THE CAB OF THE LOADER. THIS ACTION STOPPED THE LOADER CAUSING HIS HEAD AND NECK TO MOV E FORWARD. EMPLOYEE EXPERIENCED SORENESS IN THE NECK.
