Tech was flushing a cooling system on a loader. When draining the coolant EE removed the lower radiator hose and splashed EE's right lower arm and right neck with hot coolant. This caused second degree burns to EE's right arm.
***6/14/17 - FIRST DATE OF MEDICAL TREATMENT*** Had a 50-pound bag on shoulder & a loading pole in the other hand, went to turn around, foot slipped on uneven ground, left foot went forward & fell backwards to the ground - hurt low back
miner was done operating the drill and when EE stepped down from the machine EE stepped on a thin piece of metal that pierced EE's left foot
Employee struck thumb on right hand with hammer while removing a speed reducer on a lime belt.
Employee was removing snow from tail pulley of non-moving conveyor. Upon completion he dragged his hand across telescoping take up receiving cut near his thumb. He then reported the injury. After discussion we decided that another employee should accompany him to the hospital to have it looked at. He received 7 stitches and was released to return to work the next day.
TRESPASSER DROVE INTO THE SECURED POSTED QUARRY. ACCORDING TO MO HIGHWAY PATROL, SHE WAS TRAVELING TOO FAST, LOST CONTROL ON A GRAVEL LANE, STRUCK SEVERAL LARGE ROCKS AND OVER TURNED SEVERAL TIMES. A NO TRESPASSING CHARGE WAS FILED ON HER. COMPANY WAS NOTIFIED AT 7:00 PM.
EE WAS UNLOADING AN EXPLOSIVE. TRAILER BENT DOWN TO PICK UP A BAG OF ANFO. FELT LOWER BACK STRAIN & HURT.
