Employee experiencing pain and loss of strength in both hands. After initial review by Occ Med department at local hospital, employee was referred to Orthopedic Surgeon for review of symptoms. Surgeon found that employee had carpal tunnel in both wrists recommended surgery. Surgery was performed on the right hand on 3/20/26 with surgery to follow on the left hand on 4/3/26.

Employee was walking along the combing on a barge that was brought in and rolled EE's ankle. At the time there was just some swelling and no further treatment. Employee stated in March EE was still feeling pain periodically. An MRI was ordered and referral to an Orthopedic. The Orthopedic surgeon provided a prescription for Physical Therapy on 4/6/2026 to strengthen ankle.

Employee was cleaning a chute and their hand was jammed into the side of the chute opening, causing a cut on their right index finger. The employee received 4 stitches at the ER. No restrictions were issued and returned to work full duty.

Scraping / cleaning hydrator product chute / duct. After initially seeing Occupational Medicine department, with no pain not resolving, employee was referred to an MRI and saw an Orthopedic surgeon on 4/14/25 and was put on work restrictions and scheduled for surgery.

Employee was connecting a hose to the top of a pneumatic railcar prior to loading. While moving the vent hose employee lost balance and fell forward. Employee reached out to catch themselves with a hand and felt a pain in shoulder. Pain did not subside over time and was diagnosed via MRI that was confirmed by an orthopedic doctor and was given a cortisone injection on 3/16/23.

Employee was retrieving a 5 gallon bucket of stone from an automatic sampler and while attempting to pick it up, felt a pain in EE's back. Employee initially reported it but did not want immediate medical attention. Showing no improvement employee returned to Occ. Med on 9/20/22 and has been subscribed further treatment for physical therapy.

The employee had nerve conduction testing and it was confirmed on 3/12/2020 by Orthopedic Specialist that it was carpal tunnel repetitive motion illness. Employee is NOT restricted or missed time, but there will be surgery scheduled at a later date which will include restricted work.

Employee was climbing a fixed metal ladder approximately 8' to reach an access platform. On about the 3rd or 4th rung the employee felt a "pop" in left knee. There was no pain, but the next day it felt sore and swollen. After about a week without improvement the employee sought medical attention. An MRI was able to confirm on 4/5/19 that there was a torn meniscus.

Employee has pain in both hands. Believes it is repetitive motion related. The employee was sent for evaluation and further testing. On 2/11/19 an orthopedic specialist concluded that surgery would be necessary and put the employee on restricted work at that time pending scheduling of surgery.

While attempting to remove a belt scraper, the opposite end slid out of the housing, causing the scraper shaft to tilt on the end the employee was working on. The employees thumb was smashed between the scraper shaft and the housing for the conveyor head pulley.

Ee was changing a return idler on a conveyor belt and felt a pain in back. Employee was sent to Medical for evaluation and an X-ray. The X-ray was negative. After further issues with pain, employee returned to medical and MRI. The employee was sent to orthopedic specialist on 10/31/18 and was diagnosed with a damaged disc, further medical treatment was ordered.

Cleaning chute for Hydrator #1, the employee was wearing a PAPR (respirator hood). The employee believed employee was done cleaning and had opened the hood shield to clean fogged glasses, when material fell in the chute getting hydrated lime in eyes.

Employee stated was clearing the pots on the Anfo Rig when EE heard an air leak. While using hand to find the leak, a hose blew off causing Anfo to strike EE's hand.

Employee was gouging anchor in the pre-heater. The air coming out of the gouger hit a small pocket of material dust, which caused the dust to rise up under EE's welding hood and safety glasses. Employee was wearing all of the proper PPE for the task.

While operating the Brokk in the kiln a hose came loose. While ee was repairing the hose the tear out crew was removing the debris from the kiln causing vibration. A couple of brick from overhead fell striking ee on the hardhat bounced and hit EE's finger that was resting on the Brokk.

Pulling Stone Pins from Preheater stone chutes and left elbow pain flaired up from previous repetitive motion illness. Employee saw Occupational Medicine on 5/5/14 received initial medical review. 5/20/14 due to recurring pain employee returned to Occupational Medicine and received a prescription for an anti-inflammatory, making this an MSHA reportable incident.

Employee was demoing an old bullnose setion inside preheater. As the piece was cut it fell, and one of the anchors caught his left knee and gouged/cut him. Employee thought that bullnose section still had one gusset attached when he torched the bullnose piece.

Employee was standing on barge lids on Bunge 323 barge while attempting to move lids with tow boat. Two lids that are supposed to be interlocked and not able to open had a mechanical failure and came apart, causing employee to lose balance and fall onto loaded material.

While working on dust collector, DC450 a gust of wind stirred up dust laying on a beam and blew it toward EE. The result was quicklime particles in the eye. Eye was flushed by Optometrist and antibiotic eye drops prescribed.

EE was inspecting the drive belts and conveyor belt for tension. Conveyor is located in a storage building (250'longX130'wideX68'high). Conveyors runs length of building and has catwalks on both sides. Conveyor is located 50' above ground level. Air temp outside est close to 100 the temp inside building @ conveyor level est 115 +. Was walking on catwalk and passed out.

Incident report was filled out on 3/8/11 with complaints of pain in elbow from repetitive motion. 6/8/11 employee sought further medical attention with orthopedic specialist and injection of cortizone into elbow to relieve discomfort.

This incident was called into the MSHA 24 hour hotline at 12:20am on 2/13/2011. There was an unplanned ignition of lubricant in the Kiln #1 Bull Gear housing. The fire was first noted at 10:15pm. It was extinguished at 10:35pm but continued to reflash until 10:50pm. MSHA inspector of the Rolla, MO field office was onsite conducting the investigation on 2/16/2011

There was a loss of power at the plant. When power was restored operators in plant noticed that a power pole from the local Electric Utilities Substation to Tower Rock Stone was on fire. This power line goes across our property. Local Fire Dept and power company were called.

At approx. 11:50 two miners working near rail loading facility noticed a private car enter fac. and proceeded to travel between our rail and Burlington Northern main line. Car became stuck on the tracks. Driver got out of the car and off the tracks just before train struck his car.

Pumping water out of permanent barge dock. Pump hose was inside barge with lid/hatch open. Stepped in hole with left leg. Over the next couple of days, experienced pain in right leg from shock/strain. Followed up w/occupational medicine on 2/24/09. MRI ordered, surgery 3/11/09.

A fire started inside the gear drive for kiln #2. It was put out with CO2 fire extinguishers but continued to flare up. Fire was out at 1:40 pm but flared up at times after that. MSHA was notified via phone. An MSHA inspector arrived onsite between 3:30-4:00 pm.

After changing a thermocouple on kiln #2 transfer chute and was leaving preheater area, missed 1st step of 8 form transfer chute platform. Hit heel on 2nd or 3rd step, then fell down stairs. Struck arm, missed 1st scheduled shift of work on 6/17/08.

He slipped and fell forward on a walkway that was very slick from sleet and freezing rain that was falling. He landed on right knee, right elbow and right wrist. Restricted work: must wear splint until medical follow-up.

Lead person/Team Leader for Utility/Clean-up Crew was shoveling and other labor. Both elbows (primarily right) hurt. Employee is being seen by an Orthopedic Specialsit.
