Employee was shoveling rock off of powerscreen when employee felt pop in left shoulder. Went to doctor on 07/13/2021 and received a steroid injection making this a recordable injury.

EE WAS STRIKING A METAL PUNCH BAR WITH A 3 LB. HAMMER, ATTEMPTING TO REMOVE ROCK THAT WAS HUNG IN CONVEYOR BELT PULLEY. A PIECE OF METAL BAR BROKE OFF AND STRUCK EE IN THE LT. FOREARM CAUSING A PUNCTURE WOUND. SMALL PIECE OF METAL HAD TO BE REMOVED FROM HIS ARM. JUST HAVE TO KEEP ARM DRY & CLEAN.

ONE OF OUR EUCLIDS HAD AN EXHAUST LEAK & EE BECAME ILL AFTER AWHILE. HE CLAIMED THAT THE LEAK WAS PRESENT A COUPLE OF MONTHS AGO & WE MADE SOME ADJUSTMENTS. EVIDENTLY THAT DIDN'T ELIMINATE THE PROBLEM. LIGHT EXPOSURE EVIDENT TO EXHAUST FUMES.
