While attempting to remove a hydraulic hose, the employee pulled to hard and gave self a suspected hernia.

While cutting a piece of rubber flashing, the utility knife slipped. The knife entered EE's right hand beside the bottom knuckle closest to the palm of the hand.

While changing screens on the screen deck, the employee lifted the screen to move it into position. While doing so, they felt a pop in their left arm.

Employee was closing out for the day. After parking equipment, employee was walking around it. Employee was stepping over the bucket and their leg twisted. They fell and their knee hit the corner of the bucket.

The employee was riding in the jump seat of a Komatsu HM400-3 during training. The trainer hit a rut in the road causing the employee to strike EE's head on the handle beside EE. The employee suffered a concussion.

While cleaning off cat walk, individual walked beneath area and was struck on the hard hat, knocking the individual to the ground. Entire crew was warned by supervisor that cleaning of cat walk was in process. Individual ignored warnings.

On Monday, January 7, 2019 a miner was injured while operating an articulating truck on surface mine property. The miner was driving the articulating truck down a hill when it struck a berm. The associate was treated at a local hospital for injuries incurred in the accident. Minor damage was sustained to the articulating truck.

Employee was climbing a ladder and somehow missed gripping the ladder with EE's right hand about 5 feet up. EE fell from ladder but landed on feet at ground level. EE's right foot actually landed on another employee's foot causing EE's to roll and break at the ankle area.

Employee was hanging gaurds etc. on the plant. He noticed his elbow getting sore but did not say anything because he has arthritis. He worked all day. (the 28th) Came into work the next day (29th) with his elbow red and swollen. The doctor said he had an infection in the elbow and should be fine with antibiotics.

Injured was changing a tire on a Bobcat skidsteer. He proceded to remove tire and wheel assembly. The tire and wheel assembly then fell, smashing his middle finger between the inner wheel and axle hub. His finger began to slightly bleed. Employee then notified his supervisor. He was then driven to the local hospital where he recieved ten stitches.

EE WAS CLIMBING DOWN THE LADDER ON THE POWDER TRUCK. WHEN HE SLIPPED, HIS ARM WAS CUT. THE CUT WAS SO CLEAN THAT HE HADN'T REALIZED HE WAS CUT UNTIL SOMEONE POINTED IT OUT. IT STOPPED BLEEDING SHORTLY AFTERWARD.

EE WAS LOOSENING THE BOLT & WHILE TRYING TO BREAK IT LOOSE THE WRENCH GAVE WHEN THE BOLT LOOSENED ABRUPTLY. HIS HAND SLIPPED & STRUCK THE NEARBYHAND RAIL & SMASHED HIS RIGHT THUMB A LITTLE BUT PUT A CUT ON IT AS WELL
