Employee was descending off a conveyor while wearing fall protection and self-retracting lanyard. While descending the retractable whipped around striking the employee on the top of the head, causing a laceration to head that required four staples.

WHILE CUTTING A TREE LIMB WITH A 20" BAR CHAIN SAW, EE ACCIDENTALLY CUT HIS LEFT BIG TOE, WHICH REQUIRED 7 STITCHES.
