Employee was stepping from one barge to another an struck left knee on edge of barge. Employee continued to have pain in the knee an was scheduled for MRI on 12-24-2019. MRI revealed meniscus tear and was scheduled for surgery. Thus making the case reportable in 2019. Employer became aware of tear in January 2020.

Affected employee was standing in the middle of an empty barge preparing to tie it up when the barge bumped the dock barge causing the employee to lose his footing and fall onto his right shoulder.

Employee was replacing a guard on the conveyor after repairs were made. The guard slipped and smashed the thumb on the left hand. Sutures were required to close a cut, however, there are no fractures.

Employee was exiting his vehicle when he slipped on the ice and fell. He injured his right elbow and right lower back. Contusion of the right elbow.

This accident was not reportable until now. Employee strained his shoulder in November and he continued on regular duty as his shoulder got better. File was closed without medical, however, now he is complaining of it hurting again and they suspect nerve impingement. An MRI is scheduled and therapy. This is now a reportable accident.

Employee was working on jig, he slipped on the ladder going into the jig and sprained his back. this was not MSHA reportable at first, however, they have determined he needs therapy. He is still doing his regular job.

Employee suffered heat exposure on his last day of work. He was hired as summer help before he went to college. Employee was dehydrated and hospital had to rehydrate with IV's. He is now fine but will not be coming back to work as it was his last day.

Employee was tying a barge, he slipped and hit his right leg causing a bruise. this just now became reportable, because on 4/28/06, the bruise became infected. He is taking antibiotics and has a wrap on his leg. He is doing his regular job and has had no lost time.

Fell backwards while descending loader. Employee caught himself and strained his left shoulder and lower back. This just became MSHA reportable because the clinic decided to send employee to therapy for two weeks. He is on regular duty and was never on restricted duty.
