Employee slipped while climbing a ladder and hit his shin. Employee diagnosed with a hematoma on his right leg. Later, employee was prescribed Keflex for infection, making this incident recordable.

EE WAS USING A CUTTING TORCH TO CUT A PLATE INSIDE A BIN. THE OXYGEN HOSE APPARENTLY SUSTAINED A LEAK AND WHEN EE TURNED THE TORCH ON IT IGNITED THE EMPLOYEE'S SHIRT. EQUIPMENT HAD FLASHBACK A RRESTORS ON BOTH ENDS. EE WAS SENT FOR TREATMENTOF BURNS AND RELEASED. THE EQUIPMENT WAS TAKEN OUT OF SERVICE AND IS TO BE EVALUATED/REPAIRED/REPLACED.
