Northstar Materials Inc dba Knife River Materials · Caught in, under or between a moving and a stationary object

Tail Pulley on conveyor was plugged and stopped the conveyor. Conveyor was locked out and gear box brake was set to prevent the belting from moving backwards. As material was removed relieving pressure, belting moved forward pulling miner into tail pulley.

EE GOT ON TOP OF THE HOT MIX PLANT & ATTEMPTED TO WELD SOMETHING & TOOK LID OFF & HE THEN REACHED DOWN TO CLOSE LID AGAIN SHIRT CAUGHT FIRE. HE JUMPED DOWN INJURING FOOT & DISLOCATING TOES. WI TNESS GRABBED HIS SHIRT & RIPPED IT OFF.
