MMG was using 5' pry bar to align holes of tracks to insert pins to reattach tracks to shovel. Pry bar slipped, caused right hand to impact hydraulic pump skid nearby, resulting in small cut to tip of right ring finger. Medical glue used to close cut.

Loader operator reported lower back discomfort while digging in the bank in Group 1.

EE was installing the mill motor cooler on the Line 2 West Mill Motor. While utilizing 2 1/2-foot prybar to line up bolt holes, the employee felt pain in their left elbow area.

EE cleaning Phase 2 HL floor, utilized skid steer to scoop material into skip placed on 4-wheeled cart. EE moved full skip/cart into elevator. Skip tipped dumping material in elevator, caused cart to roll back towards EE. Hit them in the chest/upper right arm, landed on ground. EE fell backwards, head contacted sheave ~10-15 ft. behind elevator, resulted in cut to back head.

Employee was working on P5 outside the Pellet Plant, while closing junction box cover, EE felt something fall behind safety glasses in left eye. Wind was blowing ~10-20 mph. Evaluated 4/3/24 & issued antibiotic drops.

EE on L2 Roll Conveyor was installing new outlets. EE reported they had looked up & dust fell on face, hardhat, in eye. Later that night, EE experienced left eye irritation. Removed contacts out & flushed eye. Reported incident to supervisor on 3/21/24. Evaluated 3/21/24 & issued antibiotic drops.

EE reported pain in wrists not associated with a specific incident. EE reports pain slowly developed over time and now occurs with air-arcing activities.

EE was removing rock from the #1 apron feeder area. EE used hoist with a 4 ft. wire sling and rock hook to remove plug. When the EE began lifting the hook to remove the plugged rock, the crane moved causing the sling to rotate. EE's thumb was pinched in the loop of the sling as it rotated.

EE was operating loader in Group 5N surface. While digging, reported hit a large rock with bucket in surface bank. Continued to operate during shift, then reported sore back.

MMG placing spool back on #1 rougher pump in north truck aisle. While squeezing bottom of 16" clamps to place bolt/nut to secure it, MMG felt pain in left middle finger.

B.O. elec. pallet jack moved w/strap & pickup truck. Unit wheels contacted gravel causing to tip w/forks perpendicular to ground. Unit put in correct position, pushing down w/hands on back of pallet jack forks. EE's left hand was on fork load wheel. When unit drive wheel contacted ground, load wheel receded under pallet fork, pinching EE's left fingers.

EE was changing out empty Oxygen bottle on hand cart. While moving old bottle off by tilting it forward and lifting slightly over lip of cart, EE felt pain in left elbow.

EE was cutting a bolt head off a plate that was secured in a vice. EE was using a portable circular saw. Saw blade contact the EE's left middle finger resulting in a laceration.

MMG was placing wheel chock stand from bed of truck around wheels of parked vehicle. Wheel chock handle laying down in bed of truck, rotated upward unexpectedly striking MMG on right side of safety glasses. Resulted in a cut (stitches) and bruise to right upper cheek of face.

EE stepped down to the ground from cable boat located in shovel cut, G5 North. EE's left foot contacted the level ground and rolled their ankle.

EE reported left elbow soreness, EE attributed pain from repetitive motion when changing grate bar on pallet cars. Began PT for left tennis elbow.

EE was sandblasting while cleaning dust collectors on Concentrator Line 7 and reportedly got dust in their eye. EE reported incident a day and a half later due to irritated eye & sought medical evaluation resulting in antibiotics ointment for eye.

EE operating D-9 CAT was pushing loads off the dump. While operating in reverse (2nd gear), man basket came loose. This caused the equipment to stop abruptly resulting in pain in the EE's neck area.

A forklift was driving East to West in the Pellet Plant South Truck Aisle when an MMG exiting the doors of the Tool Crib, came into contact with the forklift. Forklift's load backrest hit MMG's nose.

Tractor operator was checking the oil at start of shift on dozer for pre-use inspection. EE was positioned to hold the door open with their hip. While doing this, EE lost their footing and fell on the tracks and between the tracks and body of the machine landing on the tailbone area and lower back.

EE stepped down from conveyor belt onto block of wood. Left hand was holding conveyor belt frame while stepping down on the block w/right foot, then stepped down to floor w/left foot when the wood block tipped to the side. Left arm gripping conveyor belt frame resulted in pain to left tricep.

EE exited break trailer to head to vehicle when EE slipped/fell on ice/snow from recent precipitation landing on the right shoulder area. Resulted in right shoulder surgery (Aug. 2022).

EE was operating #327 D-11 on the 1001 dump. EE exited cab and used power lift to get off equipment. The locking pin for the basket had difficulty releasing. As the employee stepped out of the basket, the basket jumped over the catch/stop mechanism and struck the employee in the left lower leg area.

EE was reassembling an 8" line that was plugged from a scissor lift ~10.5' off the ground. EE could not lower lift w/controls from the lift platform. Partner was trying to lower lift from ground when EE climbed down the lift, hopped to floor from bottom brace ~4 ft. up. EE contacted lift base/chassis with hip. Landed on feet (no slip) but had immediate pain in hip and back.

EE was lowering power cable standards at the mine view. The winch was hung up not allowing the cables to come down to the ground. EE tried to manually move/rotate the winch by hand when the winch released causing the EE's gloves to get caught in outside gear. Resulted in right middle and ring finger getting pinched in the spool/gear.

Employee wearing leather gloves was sliding a piece of cut 1 1/4" plate on the cutting table when the plate dropped down pinching the tip of the right middle finger between the cutting table and steel plate resulting in a closed fracture.

EE walking from an outdoor shack to another building and slipped/no fall causing pain to the left knee.

MMG was changing a 8" pallet car arm pin. The pin did not fall into place upon install. EE grabbed hex head of stuck pin with left hand, partner was asked to jog controls while EE pulled upward on pin which caused the MMG's left middle finger to be pinched resulting in a laceration.

EE was removing large sheet metal pieces of an old HVAC unit. While pulling a piece EE lost grip, stepped backwards catching the top of their right forearm on sharp corner of a duct behind which resulted in a cut & stitches.

MMG helping to change 18" pipe in the Concentrator (line 8 Finisher Floor). One section of the 2 piece clamp was removed and placed on the rolling stair. MMNG moved rolling stair & clamp fell from the platform of the rolling stair striking MMG in the face area.

EE working on ladder was cutting corner of guard on finisher in Concentrator to fit around newly installed pipe. While utilizing grinder w/cut-off wheel, grinder kicked back contacting left knee causing laceration.

EE dismounting/exiting cab of excavator missed the hand grabs and step falling 2-3 ft. to the ground. EE was holding items in both hands while exiting cab.

EE was climbing boarding ladder for grader on ready line, placed foot on first step, grabbed handholds, began to pull themselves up on clean, dry step from the ground & felt pain in right shoulder. Diagnosed right shoulder tendinitis.

EE dismounting backhoe lost footing, right foot slipped off lower boarding step (on track side frame), caused left hand to slip resulting in body to contact backhoe tracks on right side of the torso.

EE attempting to clear a feeder plug on line 6 with a pry bar. EE dislodged small rock causing the larger rock to slough. The crane hook used to assist with the plug released hitting EE left hand.

Employee was cutting flex metal conduit for shovel tugger motors at shovel repair site. While attempting to cut through rubber tap over the damaged section of conduit that was being removed, the utility knife being used slipped resulting in a cut to their left wrist area. Cut was treated with 2 stitches.

EE driving production truck to shovel cut on road and felt glute pain on right side. EE sought medical evaluation at a later date, prescribed restrictions & PT.

Loader was getting bucket teeth changed out in the pit (Webb) area. A production truck driver was helping with the task. Employee used a 2 lb. hammer to pound on the tooth to loosen for removal. A piece of material from the tooth struck the employee on the left forearm causing a cut resulting in stitches.

Light fleet vehicles traveling eastbound through pit at shift change. One vehicle on south leg of road, one on north leg of road. Roads met at "V" intersection coming down the hill. One vehicle slowed at intersection, neither vehicle reported seeing each other. Vehicles contacted each other on left door & right front quarter. EE injured was a passenger.

As production truck driver was being loaded, shovel swung over box of the truck & experienced a "swing control loss fault" causing it to lose swing control striking the front of the box. This caused jarring to production truck driver.

EE was arcing holes in Line 3 fans with safety glasses and welding hood on. EE noticed later during the day irritation in right eye but cannot recall specifically when something entered their eye. EE flushed eye several times that day. The following day, EE sought medical evaluation resulting in material being removed from eye and issued antibiotic eye drops.

EE was repairing lighting on top of the lube van, stepped on the first step to cab to turn on light switches to test lights. EE leaned across the cab seat putting pressure on right shin from the second cab step. Resulted in slight pain but EE continued working. EE sought medical treatment days later resulting in a contusion/hematoma. Incident was reported 4 days after occurrence.

EE was using excavator to remove ice frozen in pipe by lifting & dropping pipe with bucket. EE reported pain/soreness in middle of back. Sought medical evaluation at later date, received restrictions.

EE was changing carrier bearing on the drive shaft of Service Truck. While hitting the carrier bearing install tool with a brass hammer, the hammer contacted the ee's left thumb causing a distal tuft fracture to left thumb. Employee was issued protective splint and restrictions.

Employee was going up stairs in the concentrator to perform janitorial duties. Employee was halfway up the last flight of stairs when EE tripped and dislocated knee when it struck a stair.

EE was wearing anti-slip cleats and walking on mineview drill pattern when they slipped on ice falling on their back and leg.

EE was walking out of office trailer near Crusher, slipped on ice landing on left side of their body.

EE set down an engine lifting spreader bar after using it. When unhooking the spreader bar rigging from the crane, it tipped over landing on the EE's left foot.

EE was removing "bolts" from the Meva forms and EE was using a hammer to tighten down a pancake nut. When EE wrenched the nut, EE felt a sharp pain in EE's shoulder. EE treated it with OTC medicine and ice at the time of the incident. As of December 11, EE has now seeked medical attention.

While removing shovel cab windshield, EE pinched tip of right thumb between window frame and 9/16" wrench during removal of bolt resulting in pinch/shear injury to right thumb.

EE was being loaded by shovel when shovel bucket contacted the back canopy of production truck causing jostling to the EE.

EE was standing on the edge of a tank at approx. 56" off the ground and rinsing down a tank with a fire hose. EE attempted to get down from the ledge and grabbed a piece of piping on the side. The piping broke loose and caused EE to lose balance and land awkwardly on both feet. EE sought medical treatment after the shift was complete and was diagnosed with a fractured left leg.

EE was removing lower pin from production truck dump box, left hoist jack using a slide hammer. EE's thumb wrapped around the handle while hammering, putting the thumb in the pinch point causing an avulsion at the end of the right thumb.

Excessive heat and humidity working outdoors in sun our employee started experiencing heat exhaustion symptoms after leaving the jobsite so EE pulled EE's truck over and called an ambulance. EE was given an IV of fluid and potassium pills to treat

EE was de-boarding drill staircase, stepped to the ground and rolled left ankle resulting in a sprain.

Low light circumstances. Employee did not use the issued flash light and tripped and fell causing injury

EE was holding a positioning device while partner was using pneumatic jackhammer to drive out mill bolts on mill. The bolt came loose causing the jackhammer to move forward. Resulted in pinching the EE's finger between the jackhammer & knockout sleeve (positioning tool) causing a laceration to left index finger and stitches.

EE was moving furnace pallet car rail on the forks of the forklift, attempted to manually rotate it while on the forks. The rail slipped pinching the left index finger between the rail & forklift fork a laceration resulting in stitches.

EE was grinding/air arcing on side frame 302 dozer when felt discomfort in right eye. Employee was wearing wrap around safety glasses and welding helmet.

MMG was putting an anchor pin in mast on 7 drill using hammer and felt pain in left arm, wrist & arm. Diagnosed with left carpal tunnel syndrome. Surgical repair 8/17/20.

EE experienced left shoulder pain while operating chain jack to install pulley on conveyor. Diagnosed with tendinosis, degenerative fraying/tearing, AC joint arthrosis, joint effusion and tendon tear. Surgery 3/3/20.

EE swung sledgehammer to cleanout material B mixer drum. Hit left pinky finger on frame of B mixer drum. EE sustained closed fracture. Surgical repair to nailbed.

Employee was walking up steps to drum feed 3 conveyor when right foot slipped off step and foot landed on previous step (no fall). Felt pain behind right knee area and swelling. Area was well lit and stairs were clear & dry. Employee was diagnosed with a blood clot in the back of the right knee.

EE repeatedly swung sledge hammer to release dozer tooth pin, causing discomfort in rt shoulder. Rt shoulder impingement syndrome & tendinopathy.

Employee was welding/grinding inside a duct and felt irritation in their right eye. They flushed with eye wash and got some relief. They stated they weren't sure if it was the result of arc flash or dust in the eye. The next day it was still bothering them and supervisor could see a particulate in the eye.

EE using 3ft pry bar on fan shroud to align bolt holes. Bar slipped striking EE in left ear.

EE mounting bolts for containment hose on 7 drill; tied off to frame, exiting area & strap caught unused mounting plate. EE fell back & twisted lower back. EE has been working. Treating for PT later.

EE was manually adjusting forks on telehandler to fit lift pockets of a skip. Pinched right hand between forks resulting in cut right index finger.

EE was assisting another EE to install a clamp collar on shovel pin and felt elbow (left) pain.

Installing light fixture in admin. EE lowered JLG liftpod, unhooked harness and turned toward left and felt knee pain.

employee was working from their knees and when they stood up they felt a sharp pain.

EE was welding in mill 6. Was kneeling for extended periods of time that caused pain left knee. Diagnosed with suprapatellar bursitis and osteophytosis of knee (arthritis).

EE was working on hose for vac truck and felt pain in right elbow. Pain progressed as weeks went on due to prolonged day in/day out use. EE was diagnosed with Tennis Elbow.

EE assigned to engine C/O on pro-truck 210, stall 127. Believed ee working on installing left exhaust pipe for the pro-truck. Unknown if ee was stepping onto or off of rolling stair next to frame or working from stair when fell 10 ft. to ground. No witnesses to the incident. EE's partner heard noise & their name. EE found inside driver left front tire on floor (hands & knees).

EE reported ongoing right wrist pain. No specific incident reported. Carpel tunnel syndrome and severe radiocarpal osteoarthritis. EE worked up until surgery date of 6/19/2019.

Employee was removing the lip shrouds off the BE spare bucket in Etterville using a 12 pound soft blow hammer. While striking the rear of the lip shroud with the hammer, a piece of shrapnel came off either the hammer of the lip shroud and struck the employee in the right shin area causing a laceration.

EE noticed elbow aching pain & numbness in fingers, no specific incident.

Employee was working north side of P filter on Phase 2 filter floor. Employee attempted to clean the proxy switch on the filter. The employee's right pointer finger was contacted by the bolt on the gear causing a laceration.

EE could not describe a specific incident or event to contribute to wrist numbness. Handling cable and manually labor in the pit - over time.

Employee felt a pull in stomach while lifting an empty CO2 gas cylinder over a 1-inch lip on welding cart. The employee reported that in June 2018, had a non-work related herniated belly button repaired and thought may have damaged the repair.

EE walking on mixer floor heading north to check mixer reducer seals. Cleaning going on overhead so EE headed west to avoid affected area and walked b/t 2 electrical cabinets. Ducking EE bumped head on low conduit-chest height. EE brought to knees, didn't fall by striking conduit with head & felt neck crack. Treated w/PT 3/12/19 filed 7000-1 upon notification that PT started

EE was changing shovel tooth using pin puller bar. Felt discomfort in right shoulder.

Employee climbed up a 5ft fiberglass ladder and adjusted a few come-along that were used to lower a shovel house fan. As employee climbed down the ladder, EE missed the last step and fell to the ground landing on left ankle. Employee sprained left ankle.

Employee was installing one of two bushings on shaft and dropped the shaft from waist height onto EE's right foot. The shaft bent the steel portion of the safety toe on EE's right boot. The diagnosis was contusion of right foot and closed non-displaced fracture of distal phalanx of right great toe.

Employee preparing to cut beams with plasma cutter. As employee was setting and preparing to test the magnetic guide, the trigger activated with employee's left middle finger in front of the tip.

While on break, employee placed EE's left hand into EE's pant pocket. The employee's left middle finger caught and the employee heard a snap. The diagnosis was acquired mallet deformity of the left middle finger.

Employee was assisting the labor crew with manually moving electrical cable. Employee was attempting to manually carry a section of electrical cable when the cable tightened and pulled the employee to the ground causing right knee pain.

Employee was swinging sledge hammer to put plug in mill shell. The employee reported general soreness in right shoulder the next day. On January 22, 2019, the employer learned that employee commenced physical therapy.

Employee was changing clothes prior to the end of their shift in locker room. While removing their pants, the employee's foot got caught in the pant leg. The employee fell over and struck their head on the edge of a locker. Employee received stitches and staples.

Employee was using a one inch impact gun to remove nuts holding the bucket segments on a loader. The bolt spun and cut the employee's palm on their left hand.

On July 11, 2018, employee reported to supervisor that employee's back was sore from prior shift. EE stated that there was no particular occurrence or event that caused discomfort. EE could not identify a specific incident but stated has prior ongoing back issues and articulated a gradual progression of symptoms occurred as EE performed repetitive hosing work.

Employee engaged in repetitive motion of swinging a small chipping hammer to remove slag from dipper bucket. The employee reported a sore right shoulder from the repetitive motion. The employee received a diagnosis of infraspinatus tendinosis and capsulitis. Employee started physical therapy on October 10, 2018.

Employee was walking a PC 490 excavator north of the hydro discharge pump house. The excavator's right track slid off the edge of the embankment causing the excavator to roll over onto its side.

Employee was using a high torque wrench to tighten right hand segment bolts on Unit 326. The wrench slipped off the nut, driving the employee's left hand into the side frame causing a laceration to the left ring finger.

Employee used a lifting bar with co-worker to lift a truck box pin. The employee felt a strain in lower back.

On April 17, 2018, employee utilized a sledge hammer to drive out pins on drill side frame. On May 2, 2018, the employee reported pain in left shoulder. On May 11, 2018, the incident review attributed the shoulder pain to the repetitive motion of repeatedly swinging (~75 times) the 8lb sledge hammer.

Employee was water blasting in a scrubber when debris fell from the ceiling hitting employee in the hand/gun, knocking gun out of employee's hands. This created a potential water injection injury, still yet to be confirmed.

Employee was exiting 205 production truck at the end of shift in the 52 shovel cut. When employee stepped on the ground, EE alleges that EE landed on a rock and twisted EE's left knee.

Employee was changing lip shrouds on the 50 shovel bucket. Employee hit shroud with an 8 lb sledge hammer. A small piece of metal broke off and entered employee's right shin. The employee received stables to close the laceration.

Employee was repositioning an apron feeder guard. After some time, the employee developed pain in right shoulder. The employee received a diagnosis of impingement syndrome, AC arthrosis and a rotator cuff tear.

Employee was cutting a hydraulic hose protective wrap with a knife. The knife slipped and the employee cut left hand, resulting in a laceration and sutures.

Employee was exiting the pickup truck to speak with an employee. While closing the truck door, the employee slammed right middle finger in the door.

Employee was repairing worn spots on a D-11 dozer blade. While building up the blade, a piece of molten metal fell onto the top of employee's left boot and burned through the leather. The diagnoses was a full thickness burn to the left foot.

Employee was pulling a weld torch cart around a shovel stick in the shop. As the employee was walking and pulling the cart, EE struck EE's left ear on a 1" piece of round stock located on the shop material rack.

Employee was exiting pickup truck at the end of shift. As employee exited the vehicle, right foot got entangled in a cord that was hanging down by the brake pedal. The employee fell to the ground as employee exited the vehicle.

Employee was changing idlers on lines 8 and 9 of the CF2 with three other mechanics. A pry bar slid down the ramped walkway and struck the employee in the left gluteus causing a bruise.

Employee water lancing and dumping #2 rougher sump. As employee turned to get out of the way of the material dumping, twisted right knee.

Employee assigned to pipe replacement on the 1st level of the crusher. While walking toward 1st level, a truck dumped its load creating a cloud of dust. Employee noticed irritation in left eye. Employee reported to the clinic and the diagnosis was a corneal abrasion left eye. The employee received and utilized prescription eye drops.

Employee did not report an incident to the Company during or after scheduled shift on December 28, 2017. Rather, EE left a message for the Company on December 30, 2017 stating that EE reported to urgent care that day for a sore back. EE informed the Company representative that EE developed a sore back after operating a rental D10 dozer.

As the employee backed the production truck into the shovel cut, the truck struck the shovel bucket causing the pro truck to suddenly stop. The employee received restrictions on December 19, 2017, that included no operating of machinery.

Employee stepped out of truck and slipped on ice and twisted knee.

While working with the mining pit labor team, the employee slipped and twisted their right knee.

Employee was spooling cable onto cable reel, cable moved upward causing pain in right shoulder. MRI revealed: chronic avulsion, prominent tendonosis, severe degenerative arthrosis, and mild osteoarthritis of right shoulder.

Employee was replacing apron feeder drive chain and sprocket. While placing a nylon sling over the overhead beam, dust fell into the employee's eye resulting in a left eye abrasion.

Employee was stepping off of the left fender of the 16M motor grader after pulling oil sample from the engine. EE stepped onto the ground and stepped on a rock and twisted knee. EE did first aid to knee with a triage doc for a couple of weeks, applying ice and taking it easy but it wasn't getting better. Today. 10-10-2017 ee is going to the clinic to be treated.

Employee helped pull a suction hose that was stuck in the mud out of the Phase 1 Hydro tank. Employee reported that EE felt pain in EE's right shoulder. On February 14, 2018, employee notified the employer that EE had started physical therapy for EE's shoulder.

PC490 backhoe & D-11 CAT were draining trapped water on north side of surface dump. D-11 was pushing the ditch material away from trench over dump edge when berm began to fall away & trees in front toppled over. Both operators left area immediately. The dump edge sloughed along with the trapped water.

Employee was working with partner to replace the latch plate on a shovel dipper. As the employee and partner were setting down the new plate for final placement, the partner dropped the plate onto the employee's left hand striking the tip of the employee's left ring finger. Employee received and utilized a finger splint.

Employee attempted to return a crowd belt tensioning wrench on its hanging hook. As employee attempted to secure the wrench, it missed the hook and the wrench fell on the employee's left index finger, resulting in a fracture.

Employee was opening a welding rod can. The lid cut through glove causing a laceration to the second finger on EE's right hand. The laceration was closed with medical glue.

While exiting the 62 shovel, employee realized that ee forgot to turn on the MG set. The employee turned and caught the door latch as it was closing. EE left index finger got pinched between the door jam resulting in a laceration and fracture to the tip of ee left index finger.

Employee utilized dual eye protection while grinding overhead on the 51 shovel project. At shift end, the employee felt something in right eye but did not see anything. The employee reported to the eye doctor the next morning. A small piece of metal was removed and the employee received prescription eye drops.

Employee reported pain in right wrist and thumb. The employee could not recall any incident at work that caused the pain. Employee believes the pain is related to the repetitive motion required to operate the controls in the forklift. Employee received a wrist brace.

Employee did not report a specific incident. Attributes the condition to repetitive motion. In the fall of 2017, the employee had surgery for a right carpal tunnel release. Employee received a similar diagnosis for the left hand. The employee elected to have left carpal tunnel release surgery on April 10, 2018.

Employee reported that while checking cable standards in the mine employee noticed pain in right index finger from repetitive motion. Employee did not report a specific incident. The diagnosis was right carpal tunnel syndrome and right index trigger finger. Employee recently had surgery for a right carpal tunnel release and right A1 pulley release.

Employee was disconnecting a pipe at the pumping site. As the employee removed the last bottom bolt, the pipe spun and struck the employee on employee's hard hat.

Employee was assisting a forklift operator who was transporting a pipe. The pipe shifted and rolled over the employee's left hand while the load was still on the ground. The employee received left hand second - forth digit contusions. The employee underwent proximal interphalangeal joint capsular release with pinning of the joint surgery on October 31, 2018.

Employee oiling a 4.5 inch air grinder with a cut-off wheel. While setting the grinder on a work bench, the wheel spun and made contact with the employee's knuckle on right index finger. The grinder cut through EE's weld glove and caused a laceration. The employee received three stitches.

Employee is a pro truck driver. On March 17, 2017, EE was lifting/pulling cable on the labor crew. After performing this repetitive work, EE noticed pain in EE's rt shoulder. EE did not have pain at rest. On April 4, 2017, employee went in for evaluation. EE was released without restrictions and referred to PT. EE recently informed the employer that EE began the PT sessions.

Employee turned a nut to remove a filter bag in the Pellet Plant. The employee felt pain in right shoulder. An MRI revealed tendinosis of the supraspinatus tendon and degenerative enthesopathy of the humerus along with osteoarthritis. The employee's diagnosis was rotator cuff tendonopathy, a disease of the tendon, resulting in a referral to physical therapy.

Employee was leaving the drill using the main steps. As the employee reached the bottom step, the employee lost balance and fell forward landing on the left shoulder.

Employee stepped on a piece of skirting while walking on the hearth layer floor.

Employee was replacing a troughing idler. While pulling the idler out of the frame, EE felt a pain in left shoulder. Employee underwent a left shoulder scope on April 13, 2017 for a glenoid labrum tear.

Employee placed employee's left foot on the first step to onboard employee's drill at shift start. As employee stepped up and pulled with employee's arms to onboard, employee felt a pull in employee's back.

Employee stepped down from a storage dock in the lay down yard. As ee stepped down, the employee slipped and fell against a steel channel resulting in pain in right shoulder. Employee underwent right shoulder surgery on May 16, 2017.

Employee engaged in cutting apart WT beams, as EE made the second to last cut, the beam sprung apart and struck employee in the shins causing EE to fall face forward on to the shop floor.

Employee was accessing the discharge end of the L3 furnace when EE's foot got stuck on a rail causing EE to lose EE's balance, resulting in a right ankle sprain.

Employee working on 61 shovel reported eye irritation the following day. A corneal foreign body of unknown origin was removed from left eye and prescription eye drops were issued.

The right rear tire on the 201 haul truck failed as it exited the 52 shovel loading pattern. The tire failure caused a cobbing size rock to strike the 207 haul truck windshield. Shattered glass came in contact with the 207 haul truck operator's face and resulted in a right eye corneal abrasion. The employee received and used prescription eye drops.

Employee was carrying a liner, when they set it down they felt discomfort in their right arm and hand.

Employee grabbed a pry bar leaning against a fan. The fan tipped over and blew dust into employee's eye. Employee received and used prescription eye drops.

Employee was operating a production truck in the pit when EE travelled over a pot hole located in the road. Employee reported low back pain following the incident.

Employee was cutting piece of two inch pipe with a cutting wheel. Employee reported that debris entered left eye. No foreign body was located upon examination. Employee received Ofloxacin solution to prevent infection.

Employee closed door to cooler room on #7 drill, felt pain in rt shoulder. MRI revealed degenerative SLAP tear, moderate tendinosis & undersurface spurring. IME noted prime motivator in impingement syndrome is aging & employee's day-to-day activities over 25 years are a substantial contributing factor to shoulder condition. On 2/13/18, employee had rt shoulder surgery.

Employee was operating a loaded production truck when EE travelled over a bump in the haul road and felt pain in lower back.

Employee was cutting an air line hose with a utility knife. Employee sliced right thigh and the laceration required sutures.

Employee was assisting with the installation of a new drainage culvert. While walking in a trench the employee stepped on a rock. On June 17, 2016, employee received work restrictions.

Employee was installing a guard on a turning gear in the Pellet Plant when felt discomfort in midsection. Employee diagnosed with a periumbilical hernia on August 25, 2016.

EE was sandblasting on elevated platform wearing personal fall arrest gear and PPE. EE sandblasted one of the nylon straps causing the platform to tilt. EE slid down platform, but fall gear stopped fall. EE lost control of the blast hose, and it remained operational. The freefloating hose then turned toward EE. EE sustained sandlblast injuries to face, hand & leg.

Employee was pulling packing from the #4 pump located in the water reclaim building. The employee's hand slipped and struck mouth.

Employee was performing welding repairs on chute for load zone when lost footing and caught self by grabbing a beam. Employee initially diagnosed with right lateral epicondylitis. On August 24, 2016, employee had surgery to repair a partial tear of the common extensor tendon.

Employee was walking down the stairs in the Pellet Plant when left foot slipped and right foot went between two stairs. Employee felt a strain in right ankle and knee.

Employee was trimming the corners of a conveyor belt. While attempting to remove the utility knife, the knife blade sliced into the employee's boot and caused a laceration on EE's left foot.

Employee was changing filter bag, the bag pinched EE's left thumb on the bag tray, which resulted in a small cut and swelling to EE's left thumb.

Employee attempted to place a nylon sling under pipe while it was being lifted with a pry bar. The pry bar slipped and employee's left index finger was pinched between the floor grating and pipe.

Employee possibly exacerbated pre-existing condition when ee twisted right knee. Employee underwent right knee arthroscopy on June 28, 2016 to address pre-existing degenerative joint disease, arthritic wearing of the cartilage and internal derangement of the right knee.

Employee was completing EE's walk around on Production Truck 209 when EE stepped in a hole and rolled EE's ankle

Employee fell from 2 feet above the ground and landed on a piece of angle iron and twisted EE's ankle.

Employee overexerted self as EE was trying to pull a broken piece of cable socket out of a hoist drum. On June 2, 2016, employee was diagnosed with left shoulder cuff tendonitis, left bicipital tendonitis and lateral epicondylitis. Employee is participating in physical therapy and a home exercise program.

On January 7, 2016, employee assisted with the changing of a bit and deck bushing on a rotary drill. EE felt a pinch in EE's abdomen after pushing with EE's left leg. On January 18, 2016 EE received a diagnosis of a left inguinal hernia.

Employee was installing a belly pan with an impact wrench. The socket came loose and struck the employee in the mouth resulting in a cracked tooth.

Employee stepped onto top of launder and felt pain in his calf muscle.

Employee was checking a fire extinguisher and pinched his left hand and fingers resulting in a small laceration that was closed with deruabond.

Employee never reported a workplace incident. On or about November 1, 2015, employee advised that treated one year prior for trigger finger. He followed up, and his doctor opined that his condition resulted from repeated motion and prolonged day-in and day-out exposure to the work environment. Employee underwent surgery on November 23, 2015.

Employee was checking oil level on his grader. His foot slipped on the step and he grabbed onto the equipment's grab handle. Employee felt discomfort in his right arm.

Employee was working on a Cushman haulster Cart with a flatbed that had flat tires. He was putting tools away and when he lifted the bottle jack up, he twisted at the same time he stood up and he heard a pop in his hip joint and had pain.

Employee was diagnosed with umbilical hernia from pulling cable.

Production Truck Driver was de-boarding truck stair case. While using the hand railing employee received cut on right index finger from an apparent burr near the bottom of the hand railing on the stairs.

Employee removing used shovel teeth from bed of a boom truck by hand noticed a bulge near his belly button area. Risk

Employee was stepping out of rougher tub and felt a twist and strain in his left knee. The employee underwent left knee arthroscopy on January 19, 2016.

Employee was measuring holes on a blast pattern. While stepping back from a hole, he tripped over a rock causing him to fall to the ground.

Two employees were moving a cart loaded with quarter inch steel plates. The cart tipped forward, the plates slid off the front of the cart and the cart went up in the air and travelled backwards. The cart knocked the employee over and landed on top of his legs.

Employee was hosing rock/mud in 6th level of Crusher for clean-up. Some mud/dirt splashed into eye. Employee was wearing safety glasses w/side shields. Antibiotic eye drops issued & utilized.

Employee was walking towards a table located in the service center, he stumbled and contacted a battery booster cart causing a puncture wound to his right leg.

Employee stepped down onto ground off of weld truck and rolled his left ankle. The employee was diagnosed with a left ankle sprain.

Employee looked up while sitting in break room while wearing his safety glasses. An unknown particle entered employee's eye. Employee received and utilized prescription eye drops.

Employee was sawing a u-clamp with a chopsaw. The clamp slipped and struck employee's left thumb.

On 4/8/15 ~5:40 p.m. contract ee was traveling in elevator when it stopped between level 2/level 1. ~5:50 p.m. contract supervisor discovered ee in elevator & reported to HTC supervisor. Elevator was locked out and test started & ee extricated from elevator ~6:25 p.m. No personal injuries occurred.

Employee was bending a piece of 1 1/4" round bar. While bending the steel, the round bar slipped and the hot steel burned his right wrist.

Employee noticed a pain developing in their lower back. The employee believes the pain developed over his last two shifts while performing frost ripping with a D11 dozer in the pond. The employee was sent into the clinic on February 25, 2015 and received work restrictions.

Employee was installing allen bolts. Employee attempted to free allen socket from bolt with a breaker bar. The breaker bar slipped and struck the employee in the mouth resulting in fractured tooth.

Employee was performing grinding work. He was wearing safety glasses and a grinding shield. Employee reported the next day that he had a particle enter his eye on the prior shift. The employee requested medical treatment.

Employees were repairing/changing out a shovel motor when the motor slipped in their hands resulting in a pinched finger and laceration.

Employee was removing a part, the part began to drop to the floor and employee caught the part with his right hand. Employee felt a strain in his right shoulder.

Employee informed employer on December 17, 2014 that he had bilateral carpal tunnel syndrome. His treating physician recommended continuing using the wrist splints, possible injections, physical therapy and surgery.

Employee was attempting to reposition a guard on the line 6 feed tunnel when he felt pain in the lower back.

Employee slipped while working inside Mill #6 trommel, twisting his left knee and straining his right shoulder. Employee referred to physical therapy on November 6, 2014 and stated he would let employer know when he had his first session.

Employee struck by McClellan machine while exiting Mill 6. On November 11, 2014, employee received a diagnosis of right hand contusion and tendonosis.

McLellan machine boom came into contact with employee's right hand resulting in right wrist contusion and splint.

Employee reached for drill hole plug and pulled a muscle in her back under the left shoulder.

In October 2014, employee reported wrist soreness as a result of repetitive motion. Employer informed on January 26, 2015 that employee had right hand carpal tunnel surgery on December 12, 2014 and left hand carpal tunnel surgery on January 13, 2015. Employee refused to provide medical documentation for this alleged work injury.

The employee did not have an accident or incident. He was diagnosed with bilateral carpal tunnel syndrome on October 10, 2014 utilizing a Neuromuscular and Electro diagnostic Medicine Consultation. Employer was informed on Monday, December 15, 2014 that employee had underwent right carpal tunnel surgery on November 11, 2014 and left carpal tunnel surgery on December 15, 2014.

Employee placing heater hose into the back of a pickup truck. In January 2015, employee had right shoulder examined. Assessment revealed an impingement of right shoulder. A March 2015 MRI revealed tendinosis of right shoulder, osteoarthritic changes to shoulder and bone spurs. Employee underwent right shoulder surgery on April 2015.

Employee reported to the clinic for concerns of carpal tunnel syndrome. Employee was diagnosed with bilateral carpal tunnel syndrome and bilateral CMS arthritis/degenerative joint disease. Employee began occupational therapy on November 12, 2014.

While removing a concrete blanket a bat flew up and bit the side of the employee's neck.
