EE was at SD-1 Substation skid placing a 480VAC breaker into cubicle. EE backed truck into position approximately 6-8 ft. from cubicle w/breaker in the box of truck. EE carried breaker to the cubicle, crouched down to insert the breaker into cubicle & felt burning sensation/pain in left arm.

EE was working on balling floor by "A" bentonite feeder in the Pellet Plant to address a bentonite delay. EE turned to leave the bentonite feeder when right pant leg hooked on a piece of angle iron structure near floor. EE fell on their left side & knee contacted the floor grating causing pain to left knee.

EE was removing adapter fitting off 2" washdown hose used for cooling for belt cooking equipment in the Pellet Plant. EE used 2 - 18" pipe wrenches to the reducing bushing & coupler to remove & replace w/washdown hose nozzle. When applied pressure, EE felt pain in right shoulder.

MMGs were prepping belt for splicing PP-P3. MMG picked up 1/4" die grinder w/buffing wheel, pressed trigger to test prior starting work. The quick connect holding 3" buffing wheel sheared at the base of the shaft nearly instantly upon rotation, sent buffing wheel/quick connect spinning to the right side of the tool, striking a nearby MMG ~3 ft to top of left-hand knuckle area.

MMG was using 5' pry bar to align holes of tracks to insert pins to reattach tracks to shovel. Pry bar slipped, caused right hand to impact hydraulic pump skid nearby, resulting in small cut to tip of right ring finger. Medical glue used to close cut.

EE was installing the mill motor cooler on the Line 2 West Mill Motor. While utilizing 2 1/2-foot prybar to line up bolt holes, the employee felt pain in their left elbow area.

EE cleaning Phase 2 HL floor, utilized skid steer to scoop material into skip placed on 4-wheeled cart. EE moved full skip/cart into elevator. Skip tipped dumping material in elevator, caused cart to roll back towards EE. Hit them in the chest/upper right arm, landed on ground. EE fell backwards, head contacted sheave ~10-15 ft. behind elevator, resulted in cut to back head.

EE on L2 Roll Conveyor was installing new outlets. EE reported they had looked up & dust fell on face, hardhat, in eye. Later that night, EE experienced left eye irritation. Removed contacts out & flushed eye. Reported incident to supervisor on 3/21/24. Evaluated 3/21/24 & issued antibiotic drops.

EE was removing rock from the #1 apron feeder area. EE used hoist with a 4 ft. wire sling and rock hook to remove plug. When the EE began lifting the hook to remove the plugged rock, the crane moved causing the sling to rotate. EE's thumb was pinched in the loop of the sling as it rotated.

B.O. elec. pallet jack moved w/strap & pickup truck. Unit wheels contacted gravel causing to tip w/forks perpendicular to ground. Unit put in correct position, pushing down w/hands on back of pallet jack forks. EE's left hand was on fork load wheel. When unit drive wheel contacted ground, load wheel receded under pallet fork, pinching EE's left fingers.

MMG was placing wheel chock stand from bed of truck around wheels of parked vehicle. Wheel chock handle laying down in bed of truck, rotated upward unexpectedly striking MMG on right side of safety glasses. Resulted in a cut (stitches) and bruise to right upper cheek of face.

EE was sandblasting while cleaning dust collectors on Concentrator Line 7 and reportedly got dust in their eye. EE reported incident a day and a half later due to irritated eye & sought medical evaluation resulting in antibiotics ointment for eye.

EE operating D-9 CAT was pushing loads off the dump. While operating in reverse (2nd gear), man basket came loose. This caused the equipment to stop abruptly resulting in pain in the EE's neck area.

A forklift was driving East to West in the Pellet Plant South Truck Aisle when an MMG exiting the doors of the Tool Crib, came into contact with the forklift. Forklift's load backrest hit MMG's nose.

Tractor operator was checking the oil at start of shift on dozer for pre-use inspection. EE was positioned to hold the door open with their hip. While doing this, EE lost their footing and fell on the tracks and between the tracks and body of the machine landing on the tailbone area and lower back.

EE was operating #327 D-11 on the 1001 dump. EE exited cab and used power lift to get off equipment. The locking pin for the basket had difficulty releasing. As the employee stepped out of the basket, the basket jumped over the catch/stop mechanism and struck the employee in the left lower leg area.

EE was reassembling an 8" line that was plugged from a scissor lift ~10.5' off the ground. EE could not lower lift w/controls from the lift platform. Partner was trying to lower lift from ground when EE climbed down the lift, hopped to floor from bottom brace ~4 ft. up. EE contacted lift base/chassis with hip. Landed on feet (no slip) but had immediate pain in hip and back.

EE was lowering power cable standards at the mine view. The winch was hung up not allowing the cables to come down to the ground. EE tried to manually move/rotate the winch by hand when the winch released causing the EE's gloves to get caught in outside gear. Resulted in right middle and ring finger getting pinched in the spool/gear.

MMG was changing a 8" pallet car arm pin. The pin did not fall into place upon install. EE grabbed hex head of stuck pin with left hand, partner was asked to jog controls while EE pulled upward on pin which caused the MMG's left middle finger to be pinched resulting in a laceration.

Employee was cutting flex metal conduit for shovel tugger motors at shovel repair site. While attempting to cut through rubber tap over the damaged section of conduit that was being removed, the utility knife being used slipped resulting in a cut to their left wrist area. Cut was treated with 2 stitches.

Loader was getting bucket teeth changed out in the pit (Webb) area. A production truck driver was helping with the task. Employee used a 2 lb. hammer to pound on the tooth to loosen for removal. A piece of material from the tooth struck the employee on the left forearm causing a cut resulting in stitches.

Light fleet vehicles traveling eastbound through pit at shift change. One vehicle on south leg of road, one on north leg of road. Roads met at "V" intersection coming down the hill. One vehicle slowed at intersection, neither vehicle reported seeing each other. Vehicles contacted each other on left door & right front quarter. EE injured was a passenger.

As production truck driver was being loaded, shovel swung over box of the truck & experienced a "swing control loss fault" causing it to lose swing control striking the front of the box. This caused jarring to production truck driver.

EE was arcing holes in Line 3 fans with safety glasses and welding hood on. EE noticed later during the day irritation in right eye but cannot recall specifically when something entered their eye. EE flushed eye several times that day. The following day, EE sought medical evaluation resulting in material being removed from eye and issued antibiotic eye drops.

EE was repairing lighting on top of the lube van, stepped on the first step to cab to turn on light switches to test lights. EE leaned across the cab seat putting pressure on right shin from the second cab step. Resulted in slight pain but EE continued working. EE sought medical treatment days later resulting in a contusion/hematoma. Incident was reported 4 days after occurrence.

EE was changing carrier bearing on the drive shaft of Service Truck. While hitting the carrier bearing install tool with a brass hammer, the hammer contacted the ee's left thumb causing a distal tuft fracture to left thumb. Employee was issued protective splint and restrictions.

EE was removing "bolts" from the Meva forms and EE was using a hammer to tighten down a pancake nut. When EE wrenched the nut, EE felt a sharp pain in EE's shoulder. EE treated it with OTC medicine and ice at the time of the incident. As of December 11, EE has now seeked medical attention.

EE was standing on the edge of a tank at approx. 56" off the ground and rinsing down a tank with a fire hose. EE attempted to get down from the ledge and grabbed a piece of piping on the side. The piping broke loose and caused EE to lose balance and land awkwardly on both feet. EE sought medical treatment after the shift was complete and was diagnosed with a fractured left leg.

A March 2015 MRI revealed tendinosis of right shoulder, osteoarthritic changes to shoulder and bone spurs. Employee underwent right shoulder surgery on April 2015.
