Employee was a degenerative disc condition as well as bad knees with three knee replacements. These conditions make him prone to back and knee injuries. Employee was moving a box of rain jackets and felt pain in his lower back. He stated the pain was similar to the incident on 02/27/2012. This is being submitted under protest.

Employee has a degenerative disc condition as well as a bad knee with three knee replacements. These conditions make him prone to back and knee injuries. On 02/27/2012, he slipped but did not fall when getting out of his personal vehicle and hurt his back and knee.

Employee was opertating a hand held grinder when the grinder kicked back, hitting him in the chin with its handle. This caused his mouth to close quickly, chipping an upper front tooth. Employee received a crown on 7/21/10.

A natural gas odor was present in warehouse at the start of the shift. The doors were opened and the odor cleared. Late in the shift, an employee felt ill and went home due to the odorant. Her work location was changed due to the exasperation of a pre-existing condition.

While installing torque tool onto wheel nut, the torque tool slipped off of the nut causing the left 3rd finger to be pinched between the reaction arm of the tool and the rim of the tire. Employee changed jobs & has restrictions in his new position. However, the restrictions were short lived & did not affect his job.
