Employee was cutting peaned material from some boltholes, when slag blew back hitting him in the chest and going around his face shield and safety glasses. He reported feeling something in his right eye, went to a doctor and had an embedded object removed from his right eye.

Injured employee was helping change urathane screens when a coworker pounding wedges lost his grip on a hammer. The hammer struck the injured employee in the nose causing a nose bleed and immediate swelling. A follow up appointment with an Ear, Nose and Throat Specialist on 5/27/2008 determined as the swelling went down that there was a hairline fracture to the tip of his nose.

EMPLOYEE WAS WASHING EQUIPMENT IN THE WASH BAY. HE WAS STANDING ON A PLATFORM LADDER HOLDING A WAND FOR WASHING. HE STEPPED BACK THINKING THAT THE METAL WAS THERE AND THERE WAS NONE. IT WAS FO GGY AND HARD TO SEE WITH GREASE AND DIRT COMING OFF THE MACHINE. HE FELL TO THE FLOOR AND LANDEDON SIDE. HARD TO BREATHE, CUT HIS EAR.

EE WAS STANDING ON A PLANK WELDING EYE BOLTS ON THE CONVEYOR OF THE CRUSHER THE PLANK BROKE AND EE FELL INTO THE CRUSHER STRAIGHT DOWN SOMETHING HIT HIM IN THE NEED WHEN HIS HARD HAT FELL OFF. REQUIRING S STITCHES. HE IS SORE-HIS REMOVED HIS STITCHES AND HE DID NOT MISS ANY WORK.
