Miner was sitting on apron of screen before the Roto-Lift chipping clay out of fins while screen deck was not operating, but still energized. Roto-Lift started turning while miners feet were between fins, causing left leg to become crushed between Roto-Lift fin and frame of screen. EMS was called and miner transported to the ER.

Miner was standing at the tailgate of EE's personal pickup truck while taking a lunch break and using EE's own knife to cut an apple when the knife cut EE's left pinky finger, requiring stitches to close the wound.
