The employee was in position to attach chains from a loader bucket to a conveyor. The loader bucket was raised, and the chains were hanging below the bucket. The loader operator backed up to reposition/center on the conveyor, and in doing so, the chains caught the conveyor, moved it, and it tipped over. Employee fell to the ground.

While tearing down the portable plant for a relocation, the miner was moving the short section of metal steps leading into the tool trailer by self when they fell onto the top of EE's foot just behind the steel toe's coverage, breaking a bone in the foot.
