Employee was removing v-belts on vibratory screen when the pivot point that the motor was mounted on rotated down and pinched EE's left middle finger. Fracture to left middle finger.

Employee was replacing a guard on a crusher motor, the guard slid into position faster than expected and as the guard came to a stop, employee fell forward, landing on the guard which caused strain on their wrist and thumb.

During tear down operations of the portable crushing plant the employee was raising the jack stands on the jaw crusher. The middle jack stand was malfunctioning and would not rise up for transportation. While attempting to raise the leg by removing the directional valve on the hydraulic cylinder, hydraulic fluid sprayed out from the valve.
