Loading maganese onto back of truck employee was holding 4x4 so the forklift operater could push the pallet w/facks. When forks came in contact w/4x4 pinched employees finger between 4x4 and truck.

GRINDING W/9" HAND GRINDER. WELDING HELMET, SAFETY GLASSES AND PARTICULATE MASK WAS BEING USED. SLAG GOT INTO RIGHT EYE. EE WENT TO MEDICAL CENTER, HAD EYE FLUSHED. OINTMENT AND TYL3NOL 3 WERE PRESCRIBED. NO IMPAIRMENT WAS DONE TO VISION.
