Employee was manually moving a 55-gallon drum of DEF off of a containment pallet with the assistance of another employee. The drum shifted during moving, and the employee injured their right shoulder when they tried to steady the drum. Physician diagnosis unspecified injury of muscle(s) and tendon(s) of the rotator cuff of right shoulder.

THE CONDITION WAS COLD WEATHER, BELOW 32 DEGREES. YESTERDAY WAS ABOVE FREEZING SO SNOW BEGAN TO MELT AND LEFT PUDDLES OF WATER BY TRASH CAN (DUMPSTER). WATER REFROZE OVERNIGHT AND EMPLOYEE SLI PPED AND FELL. HE INJURED HIS LEFT ANKLE.
