Employee was cutting bars out of the grizzly with a torch. Employee sent another employee for a tape measure. When employee couldn't find tape measure in the truck, he got in truck and drove to get tape measure. Employee forgot that employee was using the torch that was attached to the truck. Torch jerked and twisted and struck the left wrist of employee that was using it.

Employee was walking down steps at the sand tanks when he slipped on the ice covered steps. He went to catch himself with the hand rail and that was also ice covered which caused his hand to slip on rail. He bent his right index finger backward and fractured his finger.

OPERATOR STEPPED OUT OF EXCAVATOR CAB ONTO TRACK STARTED TO STEP OFF OF TRACK ONTO THE NEX STEP EXITING THE MACHINE. HIS FOOT SLIPPED OFF OF STEP AND HE FELL TO THE GROUND LANDING ON BUTTOCKS ABOUT 3 FEET. WENT TO DOCTOR 4/15/02. FIRST DAY OFF 4/16/02.
