The employee was connecting a water hose to the drill to fill with water. Once connected, EE turned back and tripped over the hose that was laying on the ground causing EE to fall and dislocate left middle and index fingers.

An employee was installing pipe supports for a grizzly. The supports consisted of 8" schedule 160 steel 52" long. As employee was installing a support, they pinched their hand between the wall of a the crusher and pipe support causing a laceration require 12 sutures to close the wound.

Trying to lift hood on Mack truck and it caught and employee pulled second time and something popped in shoulder. R. shoulder/cervical strain plan - alternate ice & heat to area, take Naproxen (he already has a prescription) Robavin 750 mg & Ultram 50 mg. Follow up in one week - use sling provided - no use of R. arm.

EE was opening another EE's shoot for dust release, the other EE backed up too far and hit the clam gate on bottom of hopper & impact of clam knocked lever out of EE's hand and knocked him against beam to the ground, causing laceration to EE's left arm.

The employee was greasing an R-35 Euclid haul truck with power greaser. Attempted to grease a fitting on the truck when the grease line busted on end of greaser, causing the grease to enter his left hand pinky finger.

Employee was pounding anvils out of anvil ring with a sledge hammer. Another employee was standing about 25 feet away. While pounding the anvil a piece of metal from sledge hammer, head came off and hit the second employee in the upper left thigh.

EE WAS STANDING ON THE GROUND PULLING ON A 14" PIPE WRENCH WITH A 24" PIPE HANDLE. WRENCH SLIPPED FROM DOUBLE NUTS ON A STUD AND THE EE FELL ON HIS REAR END ON THE GROUND WITH HIS RIGHT FOOT U NDER HIM.
