Employee was running a walk behind skid steer removing snow around the scale-house. Employee was backing up and backed into a concrete bollard hitting right leg causing a large hematoma. Employee was sent to hospital via ambulance.

EE was cutting electrical tape off the wire connections while wires were in junction box. EE was using a ceramic knife and had on cut level 4 gloves. While cutting the second wire connection the knife blade went across EE's left palm causing a 2 inch laceration.

Employee was installing a blank in the meal pipe. While pushing blank in and didn't realize a finger was in the bolt hole. When blank finally went in it cut tip of right ring finger.

Employee was working on the long reclaimer. Employee went to reposition and while doing so slipped and left butt cheek landed on the point of a metal plate. This caused a 2cm puncture to butt cheek. This didn't become a medical until they went to wound care and they started packing the wound. This started on Oct 29th

EE was closing up a rail car by prying with a bar. While doing this, EE felt a sharp pain in EE's left side. EE then noticed belly button started to swell twice the size. It is bright red and hard to touch. EE does not have a lot of pain in this area. EE will finish the shift out on the locomotive.

Employee was working with a crew installing putlog beams in a vessel. Due to a miscommunication, the crew on opposite side of vessel pulled the beam while employees hand was in a pinch point and the top of the employees right pinky finger became pinched between a beam clamp and the vessel. Employee pulled finger out and noticed blood. Injury required stitches.

Employee was removing cutting edge from 966 loader. While removing the carriage bolts from the cutting edge, the bolt dropped down and started to turn. The employee pushed the bolt back up with right index finger, the bolt turned again cutting EE's finger requiring 7 stitches.

Employee was observing conveyor in motion. Conveyor was being cycled in position for additional bucket change out. Employee took it upon self to get into a closer position near conveyor, lost balance and grabbed rail. Wheel crossed path of right hand at that moment, squeezing right gloved finger.

Stated was fine and felt no pain. The next day said was in pain and wanted to be seen by doctor. Referred to specialist.

EE was changing out a troughing roller at the tail of the F01-003 belt. The new roller was slid in place and while trying to lift the roller up it fell back over catching EE's left hand. We think maybe EE then pulled hand out and this cut EE's hand around the thumb. EE was wearing Cut 2 rated gloves.

EE was installing a containment system for the kiln heat exchangers when EE bent down to weld a piece. Prior to that EE carried a Fronious electric 110 welder which weighs 19.6 lbs. When EE bent down EE felt something pull in the lower back.

Employee was removing a tipping valve and pinched left middle finger between tipping valve employee was holding and the guard bracket. It broke the skin and nail. Employee notified supervisor and was taken to hospital. Employee fractured tip of finger and received two stitches. Employee was released for full duty with attention not to aggravate the injury.

EE was getting on the rock breaker to operate, when EE started up the first rung of the ladder with left foot EE heard a popping noise and felt pain. From that point EE couldn't put any weight on the foot. EE had to get help to get back off and to the first aid room.

Employee was standing on crusher housing to remove a crusher liner & sit on ground. The liner started to slip toward employee. Employee moved quickly out of the way by stepping to a 4ft step ladder. When employee's foot hit step ladder the ladder fell throwing employee to the ground. Employee fell on left shoulder & elbow. Employee had an open wound on elbow requiring 2 stitches.

EE went to work truck to get a drink & grabbed a 1-gallon windshield wiper fluid bottle from the bed of the work truck that had contained antifreeze (the bottle was labeled in "antifreeze" in sharpie) believing it was a Gatorade mix bottle. EE poured some into EE's water bottle and drank half the bottle before realizing something was wrong. Employee was advised to go to hospital.

Employee was passing scaffold board and a splinter entered his left index finger at the base where it meets the hand. The splinter penetrated the rubber palmed gloves he was wearing and jersey gloves he had underneath. Employee removed splinter and continued working. The next morning his finger was swollen and he was taken to get treatment where it was discovered it was infected.

While performing labor duties, a piece of buildup fell from above in vessel (below above headache deck above and scaffold). Buildup hit scaffold above employee and a piece ricocheted between wall and scaffold and struck employee in shoulder. Employee refused medical treatment at time of accident. Employee continued working until Wed morning when asked to be seen by doctor.

Our technician was working on an air compressor and while holding the part in one hand and using a wrench with his other hand, he was turning the pipe to seal an air leak and noticed an immediate pain. He reported the injury to his Supervisor and the next day, went to a clinic and was diagnosed with a back strain.

The ambient noise measured 100db at the high level. The skid loader was working in the area as the injured employee. The injured employee was kneeling down to make repairs. The operator of the skid loader could not see the kneeling employee. The injured employee said he did not hear the back up alarm on the skid loader.

ACCESS TO BOLT WAS VERY TIGHT & WHEN EE SWUNG HAMMER, IT HIT SIDE OF SCREW CON VEYOR CAUSING IT TO STRIKE HAND RESTING ON EQUIPMENT. 5 STITCHES REQUIRED TO CLOSE WOUND. EE WAS WEARING GLOVES WHICH PROBABLY REDUCED INJURY.

EE IS VERY LARGE & HAS HAD PREVIOUS KNEE PROBLEMS.

EMPLOYEE WAS USING UTILITY KNIFE TO REMOVE PLASTIC WIRE TIES FROM WIRE BUNDLE ON A PIECE OF MOBILE EQUIPMENT. WHEN PLASTIC WAS CUT, UTILITY KNIFE CONTINUED MOVING CAUSING A CUT IN PALM OF LEFT HAND. FOUR STITCHES APPLIED. WIRE CUTTERS WERE AVAILABLE AND ALL EMPLOYEES WERE AWARE THAT UTILITY KNIVES ARE NOT TO BE USED. THIS WAS COMMUNICATED AS A LESSON TO ALL EMPLOYEES.

EMPLOYEE WAS USING A LOADER TO CLEAR ROCKS FROM AREA AROUND DRILLING RIG. WHILE BACKING UP, THE FRONT LEFT WHEEL WENT OVER TOP OF 18" ROCK CAUSING THE LOADER TO LUNGE TO THE SIDE. THE EMPLOYEE WAS LOOKING OVER HIS RIGHT SHOULDER AND THE MOTION CAUSED WHIPLASH TO HIS NECK. AN HOUR LATER THE PAIN WORSENED. HE WAS TAKEN TO THE HOSPITAL FOR EVALUATION AND RELEASED WITH RESTRICTIONS.

EE BEGAN TO FEEL BACK PAIN THAT EVENING & WAS SENT TO THE DOCTOR THE NEXT DAY & PLACED ON RESTRICTION FOR PULLED MUSCLE

EMPLOYEE WAS OPENING A 10"X8" DOOR TO CLEAN OUT A CLOGGED CHUTE. STEEL DOOR IS HELD IN PLACE WITH HANDLES AND TABS. HE LOOSENED DOOR AND WHEN HE PULLED IT OUT, THE WEIGHT OF THE DOOR WAS MORE THAN HE EXPECTED AND IT DROPPED SEVERAL INCHES, PINCHING HIS FINGER AGAINST AN ADJACENT STEEL BEAM. THE IMPACT CAUSED BRUISING AND FRACTURE OF HIS RIGHT FOURTH FINGER. DOOR HAS BEEN MODIFIED.
