Employee was running a walk behind skid steer removing snow around the scale-house. Employee was backing up and backed into a concrete bollard hitting right leg causing a large hematoma. Employee was sent to hospital via ambulance.
EE was cutting electrical tape off the wire connections while wires were in junction box. EE was using a ceramic knife and had on cut level 4 gloves. While cutting the second wire connection the knife blade went across EE's left palm causing a 2 inch laceration.
Employee was installing a blank in the meal pipe. While pushing blank in and didn't realize a finger was in the bolt hole. When blank finally went in it cut tip of right ring finger.
Employee was working on the long reclaimer. Employee went to reposition and while doing so slipped and left butt cheek landed on the point of a metal plate. This caused a 2cm puncture to butt cheek. This didn't become a medical until they went to wound care and they started packing the wound. This started on Oct 29th
Employee was shoveling and felt a pull in back. Employee was put on restricted duty for a week.
EE was closing up a rail car by prying with a bar. While doing this, EE felt a sharp pain in EE's left side. EE then noticed belly button started to swell twice the size. It is bright red and hard to touch. EE does not have a lot of pain in this area. EE will finish the shift out on the locomotive.
Employee was working with a crew installing putlog beams in a vessel. Due to a miscommunication, the crew on opposite side of vessel pulled the beam while employees hand was in a pinch point and the top of the employees right pinky finger became pinched between a beam clamp and the vessel. Employee pulled finger out and noticed blood. Injury required stitches.
Employee was working in baghouse without hardhat on. Employee was climbing out when EE struck head on a welding receptacle causing a laceration on head. Employee was taken to local medical facility where EE received 4 stitches.
Employee was standing on a pile of rubble when the bottom of the pile started to drop. Employee jumped landing on their right shoulder.
Employee was removing cutting edge from 966 loader. While removing the carriage bolts from the cutting edge, the bolt dropped down and started to turn. The employee pushed the bolt back up with right index finger, the bolt turned again cutting EE's finger requiring 7 stitches.
Employees were on the support structure of a steel plate. The steel plate fell over on top of the employee pinning EE beneath it.
Employee was trying to lift steel plate off of employee and index finger was cut off at tip.
EE was assisting electricians by lifting bin to make weight change on load cell when EE felt a shock sensation in their neck.
Employee was moving and mounting control boxes on a conveyor. While doing this task EE felt a pull in the groin.
Employee was observing conveyor in motion. Conveyor was being cycled in position for additional bucket change out. Employee took it upon self to get into a closer position near conveyor, lost balance and grabbed rail. Wheel crossed path of right hand at that moment, squeezing right gloved finger.
Employee was passing scaffold boards on scaffold deck and misstepped on scaffold plank, fell to left side and caught self on handrail under left shoulder/armpit and did not fall to ground. Stated was fine and felt no pain. The next day said was in pain and wanted to be seen by doctor. Referred to specialist.
Employee was in contact with COVID positive employee
Employee was walking and fell on left shoulder. Employee kept working until day of surgery. Surgery was on November 4th. This is when it became a lost time.
EE was changing out a troughing roller at the tail of the F01-003 belt. The new roller was slid in place and while trying to lift the roller up it fell back over catching EE's left hand. We think maybe EE then pulled hand out and this cut EE's hand around the thumb. EE was wearing Cut 2 rated gloves.
Strain upper right arm climbing off of drill by cab, 3 step ladder, left hand slipped off of handle and all weight went onto right arm.
EE was installing a containment system for the kiln heat exchangers when EE bent down to weld a piece. Prior to that EE carried a Fronious electric 110 welder which weighs 19.6 lbs. When EE bent down EE felt something pull in the lower back.
While staging grates in the plant kiln cooler area, employee slipped on a piece of plywood that was placed in the access doorway, causing EE's leg to come down at an awkward angle.
Employee was removing a tipping valve and pinched left middle finger between tipping valve employee was holding and the guard bracket. It broke the skin and nail. Employee notified supervisor and was taken to hospital. Employee fractured tip of finger and received two stitches. Employee was released for full duty with attention not to aggravate the injury.
EE was coming from the tower to the core building. EE walked up the stairs by the elevator and when EE stepped off the landing on to the road EE felt left knee give out. This was not a reportable until 2/28/2020 when EE missed time after surgery.
Employee was getting tool bucket from locker and felt sharp pain in lower stomach area. Employee was able to do regular job until day of surgery which was 01/03/2020
EE was getting on the rock breaker to operate, when EE started up the first rung of the ladder with left foot EE heard a popping noise and felt pain. From that point EE couldn't put any weight on the foot. EE had to get help to get back off and to the first aid room.
Employee was moving a vfd cabinet with another employee on a catwalk with steps. When they were going down the steps one employee lost footing and fell backwards into the catwalk.
IE was cleaning the interior of #1fm building with another EE and was running the walk-behind skid loader on the ground level at the time of the incident. As IE was exiting the south side door IE hit head on the door frame and was thrown to the ground.
Employee was standing on crusher housing to remove a crusher liner & sit on ground. The liner started to slip toward employee. Employee moved quickly out of the way by stepping to a 4ft step ladder. When employee's foot hit step ladder the ladder fell throwing employee to the ground. Employee fell on left shoulder & elbow. Employee had an open wound on elbow requiring 2 stitches.
EE went to work truck to get a drink & grabbed a 1-gallon windshield wiper fluid bottle from the bed of the work truck that had contained antifreeze (the bottle was labeled in "antifreeze" in sharpie) believing it was a Gatorade mix bottle. EE poured some into EE's water bottle and drank half the bottle before realizing something was wrong. Employee was advised to go to hospital.
Employee was manually turning a lathe. This caused pain in left leg.
Electrical employee was unloading cable tray from trailer. As EE removed a steel band from under the tray it flew up and lacerated EE's left arm above the elbow.
EE was lowering a slider bed that was vertical on a dolly. While lowering EE felt a pop in EE's left arm. EE found out 06/13/2018 that EE needed surgery to repair torn tendon. Surgery was 06/15/2018. EE will be on restricted duty until released from Dr.
Employee was adjusting a jack bolt on #1 mill motor for alignment when the wrench slipped smashing right ring finger between the wrench and the floor. The contact caused a laceration to the finger
Employee was relieving a laborer for break and was lifting 55 lb bags of refractory onto the gunite machine for 30 minutes. After 20 minutes, employee felt a pain in the lower back.
Employee was struck by a piece of metal that broke off of a hand tool while trying to line up a hole on a liner.
2 employees were grinding on a chain conveyor approximately 6' apart in the dome area of the cement plant, one employee felt a piece of metal fly into left eye. EE was unsure if the debris was from EE's own grinding or from the employee next to EE.
Employee was walking up and down the preheater tower continuously for several days causing pain in EE's left foot.
While task with installing steel door frames in the Jumbo covers on a conveyor belt-roadway, two employees attempted to move the door frame manually to allow a vehicle to pass. The injured employee stated that while the employees attempted to lift the door frame, employee felt a sharp pain in employee's lower back
Employee was grinding on a piece of metal with a grinder when a small shard of metal flew up under face shield and safety glasses and became embedded in right eye.
Injured employee and supervisor were repairing missing grate plate when injured employee fell backwards into hand railing herniating lower back. Injured employee was seen on 11/7 and placed on restriction and provided prescription medication.
Employee was climbing down from a Cat D7 dozer when left foot caught in the step and twisted left knee. Employee has had prior knee surgery. Employee had knee surgery on 2/18/2017
Employee was retrieving a tag line from beneath a belt truss leg by using left hand to lift the truss leg, while using right hand to swipe the tag line from beneath the leg, the truss section slipped out of left hand pinching right middle finger between the truss and the floor beneath causing the injury.
Employee was cutting a piece of air line with a utility knife. The knife slipped causing a laceration to the employee finger

Employee was digging at the toe with the CAT 992 when the bucket tooth slipped causing the loader to lunge forward. The employee's left arm slid forward jamming into the joystick controls. The employee received an injection to arm on 6/29
Employee was removing a bent pulley. While lifting the belt off the pulley the bearing rolled out onto the employees left foot causing a fracture to the left foot

Employee was rolling a 750# steel beam over to check a bolt hole pattern when the beam fell onto his right foot from a distance of less than 2'. Employee steel toe collapsed on his foot from the weight and broke his big toe.
Employee was drilling a hole with an electric drill when the bit caught and spun the drill. The employee's gloves were tacky and stuck to the drill handle. His right pinky was twisted with the drill causing a fracture
Employee was walking carrying a box of bag house bags when his back tightened up. Determined to be work related on 10/1/2013
Employee was investigating a high diff pressure on a bag house. Employee went to wrong location and stuck hand inside a pipe with a moving rotary air lock above. Employee contacted the blades and amputated 3 finger tips on his left hand
Employee was stepping up to the first rung of the ladder and his foot slipped. Employee felt a pain in his left knee. Employee required surgery to repair a torn meniscus on 8/19/2013
Employee was passing scaffold board and a splinter entered his left index finger at the base where it meets the hand. The splinter penetrated the rubber palmed gloves he was wearing and jersey gloves he had underneath. Employee removed splinter and continued working. The next morning his finger was swollen and he was taken to get treatment where it was discovered it was infected.
While performing labor duties, a piece of buildup fell from above in vessel (below above headache deck above and scaffold). Buildup hit scaffold above employee and a piece ricocheted between wall and scaffold and struck employee in shoulder. Employee refused medical treatment at time of accident. Employee continued working until Wed morning when asked to be seen by doctor.
Our technician was working on an air compressor and while holding the part in one hand and using a wrench with his other hand, he was turning the pipe to seal an air leak and noticed an immediate pain. He reported the injury to his Supervisor and the next day, went to a clinic and was diagnosed with a back strain.
Injured employee and two co-workers were removing grating to clean under. While lifting the employee did not follow proper lifting techniques and strained his right lower back
The ambient noise measured 100db at the high level. The skid loader was working in the area as the injured employee. The injured employee was kneeling down to make repairs. The operator of the skid loader could not see the kneeling employee. The injured employee said he did not hear the back up alarm on the skid loader.
Employee was cutting flanges off of pan conveyor buckets. Employee was lifting the bucket to rotate and lost his balance. While falling he twisted his shoulder attempting to push the bucket away and catch himself. The ground was even and level.
Climbing up into a triple 7 euclid (e0072) reaching to pull himself up in to euclid and felt something pull in shoulder blade area of back.
Injured employee was stripping #8 bag house and had cement on his skin. The cement irritated his skin from movement of his clothing causing cement burns to his arm pit and both wrist

Frost in morning creating a slipping hazard on ladder of Mitsubishi 45 ton loader.
Employee was cutting strips of wood down to 12X2 inch pieces using a table saw. The employee's thumb struck the saw blade and cut his left thumb open. Required 6 stitches to close the wound.
Employee was holding onto a air lance while trying to dislodge a piece of buildup off the burner pipe. The piece broke free and jerked the air lance. The employee was still holding onto the airlance and strained his right shoulder. Employee attempted to condition the shoulder but was determined to have a torn labrum. Employee had surgery on 12/2/2010
Employee was clearing ice and snow when he slipped on the snow and fell to his back, breaking 3 ribs. We experienced 2 snowfalls of 24"+ of snow in a week.
Driver hit a rut and bumped his head against the side of the truck and was cut. The cut required three (3) stiches. The driver was wearing his seat belt. Driver returned to work the same day. No lost time and no restrictions.
Employee was holding a rope attached to a heavy cable. The cable and rope were slipping. The employee was forced to grasp harder and strained a muscle in his left hand.
Employee was cleaning under a beam. When he raised his head, he struck his face on a low hanging beam.

Employees was assigned housekeeping duties on the second level of the work site. A hole covered with plywood and wired in place to prevent movement was apparently removed by the injured employee and he fell into it.
Injured employee was found slumped over inside his haul truck with a trauma to the head. Incident is unknown at this time. Investigation continues.
While loading a piece of wood approx. 3' x 4' into the backhoe bucket, EE's RT glove caught on the wood, causing the panel to fall toward's EE. A protruding nail head cut his left forearm.
Employee was assisting another injured employee and aggravated a previous back injury. Employee was assisting lifting a hearbox off an employee's hand using his own strength and strained back.
While skinning out an elecrical cable, employees knife slipped and cut into upper thigh.
An associate was attempting to adjust a cement bag on a Roto Packer. A cement split spilling cement onto the associate.
Associate was manuevering a rachet device. The associate moved the rachet device too close to a rail car causing his finger to be caught between the rail car and rachet.
Slipped on ice, injured left arm.
This employee was using a vacuum hose to clean-out the grit separater in SFG. She was using a small ladded for ingress when it slipped out from under her. She was half way out of the entryway and her shoulder caught on the separater housing causing a dislocation of her shoulder.
Slipped on ice landing on back.
Telehandler window fell on employee's forearm.
Employee struck right hand with a hammer.
Strained right shoulder while trying to weld overhead.
This employee was climbing down a ladder and wearing bifocals - was higher than he thought when he stepped down pinching his sciatic nerve.
This employee was working on a heat exchanger. He was taking off fittings that have been in place for over 6 years. They were extremely tight and the employee was using a pipe wrench that was too small for the job. This incident resulted in a strained right shoulder.
This employee was cutting pipe with a electric hand grinder when the blade caught in the material and "kicked-back" across the top of his left hand. The laceration took 14 stitches and the employee was given a tetanus shot along with antibiotics and modified duty.
This employee was trying to free a stuck drill, when a hydraulic line on the drill ruptured, spraying oil into his eyes. His eyes were washed out and the employee was sent to a clinic for treatment. He had no restrictions. On 2/15/2007, this employee went back to the clinic for eye problems and was put on modified duty indicating no ultraviolet or weld arc exposure.
This employee was picking up a printer box with his right arm while walking forward. While picking it up, he slipped on a small piece of paper on the tile floor in the lobby. The fall caused a sudden pain in his left shoulder to his wrist.
The turbo on a 992 loader caught fire and the employee pulled the fire extinguisher system and climbed off the loader. As the employee went back to shut off the engine, he heard a loud "puff" and as he went to run - his knee gave out and he fell on the ground hurting his ribs.
Employee stepped off a curb and twisted ankle.
Back Strain. This is a tentative Workers Comp Claim that allegedly started 8/2/2004. His attorney, The Maryland Workers Comp. Commission and our Insurance Carrier and an Independant Medical Examiner are trying to sort it out.
The employee was pulling on a pallet when his foot slipped and he fell and injured his back. This accident report was misplaced by others when I was out of state. That's why it is late.
This employee was wearing safety glasses and not the required safety goggles while sweeping material in the bio-solids silo. The employee was also asked to vacuum the area not sweep it. The result was that the employee got dust in her eyes and had to have them washed out. Acc. Rpt. Was misplaced during absence.
This employee was pulling a transport line into position when the cable sling jumped and struck his right hand fracturing his little finger.

Strained Lower Back
This employee was removing a railing to repair it. When he unbolted the railing, it fell crushing his right thumb.
This employee was air lancing a pump in the Mill room. The air from the air lance hit a beam and was directed back at the employee.
Crushed left ring ginger. Caught finger between jackhammer and mounted ladder.
Employee was shoveling material and twisted the wrong way causing a lower back strain. Safety Manager was on vacation then training - this is why the report is late.
Employee was fueling a 777 Cat Truck with a 1" nozzle as the Wiggens nozzle was out of service, when the nozzle came out and sprayed him with diesel fuel.
Cement dust blew into eye
This employee was manually moving material when his finger struck the floor kick plate. He fractured the tip of his left ring finger.
This employee was tightening a bolt on a gearbox and strained his right wrist.
This employee tripped on a toe board and lacerated his right forearm. This incident was not reported during the allotted time as the Safety Manager was on vacation.
Employee was standing in a pile of cement dust while cleaning and received contact burns to ankles. This report was not submitted during the allotted time due to the Safety Manager being on vacation.
Clinker dust caused a contact burn on neck. Incident was not entered into the MSHA database within the allotted period due to the fact that the Safety Manager was on vacation.
This employee was using a pry bar to loosen and close a valve when he lost his footing and strained his lower back.
While cutting Kendor on a 45 degree angle, the end piece swung around and hit the employee's left hand causing a lacerated finger requiring 11 stitches.
This employee was burning holes through newly installed liners in a vibrating tube. When he went to relocate his torch from one hand to another - it spun and burned his left side of his face, head and neck causing 2nd degree burns requiring Silver Sulfadiazine and a tetanus shot.
Two employees were placing a crated roll of wire on a bar when one of them suffered minor puncture wounds to the left side of his abdomen. A Tetanus shot was required.
This employee was cleaning the build-up of material on and around the manhole covers on both the North & South day silo bins. He injured his back trying to lift the manhole cover.
Employee was removing roll brake liners from a large cooler door when coating from the kiln fell and sent hot dust out the door
While unplugging and checking an air blaster on the kiln inlet, the back plate was removed previously from inside the cannon blow pipe and dust and pebble sized material flew into his face.
Employee was cutting the metal grizzly off the # gypsum tank when they cut into rust scale and smelled a strong fume
Employee was laying on back to do the job and had to use his left hand to swing a hammer.
Employee and crew were dismantling a metal beam, the beam knocked loose a smaller metal pipe. The small metal pipe struck the left forearm of employee, causing a contusion--negative x-ray results. Employee has been given work duty restrictions.
The employee was standing on the corner of the limestone feeder, took a step to the left and fell down the step to his left.
When digging and moving large material, the loader bucket slipped off a large rock and bounced the cab. The employee bumped his elbow on the armrest.
Employee was at floor level and was moving ladder. Coworker was working nearby, 10' above floor level. The coworker was handling a handheld power tool (impact wrench). The handle on the impact wrench slipped off the tool and the tool then fell, striking employee (working on floor level) on his hard hat (strained neck).
Employee slipped on ice walking from SFG to the garage. Contusions on hip, shoulder and head.
Employee slipped on snow while exiting a skid loader and fell approx. 1 1/2 ft. to ground.
Employee was overdressed for the weather and became exhausted while moving a vacuum truck hose.

Operating skid loader - clean-up during shut down. Employee failed to inspect work area for obstructions. Lip of concrete was sticking up about 1/4', allowing the skid loader bucket to strike it. EE impacted steering wheel (contusions to ribs & head).
Employee was using hammer & chisel to remove a bolt. EE failed to control chisel and it hit him in area between nose and eye (cut required stitches). Failure to wear proper PPE (face shield).

Stepped through hole in grating (contusions to chest). Grating was on platform of 450' power tower.
Employee was assisting manually several other employees in lifting a piece of duct pipe onto a pipe stand. He felt a sharp pain in his lower back-like a strain.
Employee manually handling chunks of broken concrete, in process of cleaning up. May have strained lower back
While using the pressure washer, hose wrapped around wrist causing a slight sprain.
Using cutting torch to wreck out old hopper; included use of face shield and safety glasses; employee lifted face shield when not using cutting torch, pieces of cement/metal particles sitting on head suspension of face shield fell off of small groove on top of head suspension and down through and behind safety glasses. Eye irritaion from particles falling into eye area.
Injured employee was using digging bar to clean under conveyor belts that were locked out. While striking material with bar, his finger struck against metal frame work. Impact lacerated the finger through the leather gloves. Injury required sutures and splint. Area is cleaned more often to prevent buildup and belt scrappers are better maintained to prevent spillage.
Employees were working on 8" flyash line to unclog it. System was shut down at time. When the removed 2" cap to insert air lance, material flushed out striking employee in face. Safety glasses were worn, but dust flow blew them off. No serious or permanent injury. System has been redesign to reduce clogs and all caps now have valves to allow controlled opening.

Injured employee was shoveling material from under equipment and was twisting while bent at the waist. Caused strain and disc injury to lower back. Employee instructed not to twist but to lift feet and turn when shoveling.
Injured employee was replacing head traction segments on large pan conveyor. Difficult access caused the employee to work at an ackward angle while tightening bolts with torque bar. Caused strain to lower back. Purchased compact torque multipler to use in tight areas.
Employee was relocating a 2 piece, plywood & 2x4 form wood ramp for wheelbarrows used during a concrete pour. The ramp had been un-nailed/separated on one side. Employee thought both sides were un-nailed and attempted to move the ramp straining his right shoulder while picking upward on the ramp.
Injured employee was setting up suction hose for vacuum truck when large diameter hose fell off back of truck striking her in back of head and neck. No permanent or serious injury. Hose fitting to truck was severely worn and not seen from ground level. Hose fitting was replaced and part of inspection program.
Injured employee was riding side of moving railcar when wind gust blew open door on adjacent building. Employee thought she could fit through opening, but door caught her in the side and squeezed her against railcar. No permanent or serious injury but had severe bruising. All swinging doors were removed and replaced with roll-up doors if needed.
Injured was working in high heat area when he felt dizzy and while leaving area fell down steps spraining ankle. Employee ate large lunch and vomited but did not leave area. New policy issued for heat stress with time restraints, required breaks and symptoms of heat exhaustion.
Injured was taking gas readings for open port in hot meal chute when air blaster went off spaying hot gases into face. Employee was wearing safety glasses. Received first and second degree burns to cheeks and lip. Employee has recieved additional training on locking out air blasters before opening port holes.
Injured employee was riding in basket of 130' manlift in lowered and retracted position. While reversing to make sharp turn, rear wheel went into 4" deep hole which caused basket to bounce extremely. Employee was wearing fall protection and was propelled upward several feet. Upon landing in basket, his left leg hit against piece of trash metal causing severe laceration.
Injured employee put hand in air operated valve to clean out materials. Had valve in local control, but not locked out. Valve closed on left wrist causing possible nerve injury. Employee recieved disciplinary action for not locking out system prior to working on it.
Injured was installing metal lagging in feed elevator when he felt strain in back. Condition became worse over weekend and started tightening up. Need to use two men to install this lagging or use hoists.
Injured was operating small crane when he left cab to hook up next item to be lifted. While bent over, strong wind gust swung crane line ball which struck him behind ear. He was wearing hardhat, but it struck just below rim. Was instructed to lower ball to ground to prevent swing.
Walking past display case in hallway, hand rubbed against wooden frame causing a splinter to enter and break off under the skin and wedged in a finger knuckle. Became inflammed and required a physician to x-ray and remove. Cabinet sanded and refinished.
Supervisor was responding to help employee clear a clogged dust silo. Employee had already opened large door and digging out material. Supervisor realized that this was unsafe because material was still above door. While trying to close door, material flushed out and into eyes of supervisor. All employees instructed to verify silo is empty before opening doors.
Employee was shoveling material when he tried to raise up, his back wouldn't straighten out. Due to the fact that he was lifting and twisting to shovel material, this caused an overexertion to his back. Employee was instructed on the proper method to move feet instead of twisting back to turn.
We were installing several side panels on the bucket elevator. The supplied rental equipment erratically moved and hit a panel that was about to be installed, knocking it over on worker's right leg.
Employee was carrying small jackhammer on left shoulder in route to another worksite. Jackhammer started to slip off shoulder and employee caught it causing strain to left shoulder. Employee advised on how to carry tools correctly, to know limitations, and to ask for help or use carts to transport tools.
Injured employee was assisting another in positioning a suspended conveyor belt around rollers when he felt a small pain in his lower back. He didn't believe it was serious and continued working until 1/21/05. On that date, he was working in cold weather and the pain became unbearable. He then was sent to a doctor for treatment.
Employee was trying to carrying empty 55 gallon barrel up steps to drain oil into. Strained back while pulling up on barrel. Employee did not ask for help or use available hand carts. Employee was advised to ask for help. If this becomes a frequent task, a hoist will be installed.
Employee was replacing door on material elevator when door fell out of holding position and hit foot. Another employee was assisting by hitting locking cross bars into place. Cross bar didn't hold and door fell about 3' and struck employee on foot just behind steel toe. Employees should have loosened up cross bar bolt and taken more time to install.
Employeee recieved whiplash on 9/19/03 while backing loader and looking over shoulder when front tire ran over 12" diameter rock causing whiplash, but injury was not serious at time. He continued working for the past year and neck became increasingly painful. Injury detected in 12/04 in upper neck region that required surgery. He pre-op for surgery on 12/9/04.
Stepping down from ladder on loader when employee twisted knee. His foot evidently landed on a small stone or unlevel ground. Wasn't aware of severity of injury until later in day and it became much worse later in week. Eventually had to have surgery to repair torn ligament. Area was fairly smooth and covered with road stone and step was within 2' of ground.
Employee injured lower back while carrying piece of metal out of storage area. Employee was walking backwards while another employee was carrying other end. Truck was not placed close enough to material. Employee instructed to get forklift to move material so truck could park closer and/or use forklift to load material.
Employee was assisting in stemming holes prior to blasting in quarry. Felt pull and pain in lower back. Load being lifted was not heavy, but position of feet was ackward and could have created twist.
Employee was using pry bar to knock down buildup inside vessel while standing outside inspection door. Buildup released suddenly causing stain in groin area. Small jackhammer was available to use, but employee chose not to. Told to use it from now on.
Injured employee was working next to and below another worker who was using air impact to tighten and break off head bolts on liner plates. When worker broke off bolt head, he allowed air impact to swing downward. It struck injured employee in back of hand piercing it and breaking a bone. Tool modified to eliminate hazard and air pressure increased to reduce fatigue.
Employee became dizzy and confused while working in plant. Coworkers responded and found him to be heat exhausted. Transported to emergency room and was severly dehydrated. Employee has been sick for several weeks, was on medication, and didn't drink water. Safety Alert sent to all employees as lessons learned.
Injured employee was walking down outside metal stairs when he slipped on grease. Employee's foot went down several steps while his other leg stayed on the higher step causing an overexertion of the knee. Employee came back to work the next day with restrictions, however the injury required surgery to be perform on 7/7/04 to repair a torn ligament.
Injured employee was removing heavy metal screens in rock separater. When he lifted screen up, it slid down and pinched finger between bottom of screen and support. Broke tip of finger and sliced open finger tip. Hoist in place to assist in lifting screens but could not be used due to location of this screen. Project requested to replace entire screening equipment.
Associate was working on equipment and when he stepped back, his foot twisted on the edge of a step. He was aware of step and has worked in this area many times, but did not look when he stepped back. Lighting was adequate in area and wasn't congested.
Employee was standing to rear of pickup truck that had restricted view in rear window. Truck backed up and stuck employee who was facing other direction. Noisy location and driver assumed employee moved. Employee did not get run over but had knee injury.
Employee was helping to pull heavy metal piece off of conveyor belt table. When he released metal, it fell on his ankle. Employees had hoists in place to lift metal but they were in a hurry. Instructed to use hoists on all heavy lifts and not to drop material.
EE WAS WEARING HARDHAT AND WAS WORKING UNDER SOME PIPES. HE STOOD UP TO LEAVE AREA & HIT HEAD AGAINST PIPE CAUSING A JOLT TO HIS SHOULDERS & NECK.
EE WAS LIFTING 5 GALLON BUCKETS OF OIL INTO BED OF PICKUP TRUCK AND FELT HIS SHOULDER POP. VARIOUS HOISTS AND FORKLIFTS ARE AVAILABLE TO ASSIST IN LOADING AND EE IS INSTRUCTED TO USE THEM. ALS O INVESTIGATING IF SMALLER CONTAINERS WOULD BE APPROPRIATE.
Employee was loading 6" steel pipe 6' long onto forks of telehandler. Lifted end of pipe to center onto forks, when his feet slipped on ice causing him to twist his back. Area of accident is a remote storage location that is not normally cleaned during snow and ice storms. Employee instructed to reposition telehandler instead of manually moving pipe or to get help.
EE IS A LOCOMOTIVE BRAKEMAN AND WAS WALKING ALONG RAILROAD TRACKS TO SWITCH TRACKS. STEPPED ON PATCH OF ICE AND FELL TO KNEES. INVESTIGATING USE OF LIQUID DEICERS NEAR SWITCHES. SALT DOESN'T WORK WELL.
EE WAS REMOVING BOLT ON ARM OF TIPPING VALVE TO RESET. EE POSITIONED HAND TO CATCH BOLT AS IT WAS DRIVEN OUT. HAMMER HIT HANDRIAL CAUSING IT TO DEFLECT & STRIKE HAND. 6 STITCHES TO FINGER. MOT ORS RELAYS REQUIRE RESET TOO OFTEN & ARE AN OLDER TYPE THAT ARE NO LONGER AVAILABLE. MOTORS AND RELAYS WILL BE REPLACED IN MODERNIZATION PROJECT SCHEDULED IN 6 MONTHS.
EE WAS USING HAMMER TO DRIVE OUT BOLT ON BROKEN COUPLING OF SCREW CONVEYOR. EQUIPMENT WAS LOCKED OUT AND SECURED. ACCESS TO BOLT WAS VERY TIGHT & WHEN EE SWUNG HAMMER, IT HIT SIDE OF SCREW CON VEYOR CAUSING IT TO STRIKE HAND RESTING ON EQUIPMENT. 5 STITCHES REQUIRED TO CLOSE WOUND. EE WAS WEARING GLOVES WHICH PROBABLY REDUCED INJURY.
EE WAS EXITING MOBILE EQUIPMENT & FELT STRAIN IN RIGHT KNEE. DID NOT REPORT IMMEDIATELY BECAUSE HE FELT IT WAS A MINOR PAIN THAT WOULD CLEAR UP. HOWEVER, INJURY CONTINUED TO HURT & PHYSICIAN P LACED HIM ON RESTRICTIONS. AREA OF ACCIDENT WAS CLEAR & FREE OF UNLEVEL SURFACES. EQUIPMENT HAD STEPS/HAND HOLDS IN GOOD CONDITION. EE IS VERY LARGE & HAS HAD PREVIOUS KNEE PROBLEMS.
EMPLOYEE WAS USING UTILITY KNIFE TO REMOVE PLASTIC WIRE TIES FROM WIRE BUNDLE ON A PIECE OF MOBILE EQUIPMENT. WHEN PLASTIC WAS CUT, UTILITY KNIFE CONTINUED MOVING CAUSING A CUT IN PALM OF LEFT HAND. FOUR STITCHES APPLIED. WIRE CUTTERS WERE AVAILABLE AND ALL EMPLOYEES WERE AWARE THAT UTILITY KNIVES ARE NOT TO BE USED. THIS WAS COMMUNICATED AS A LESSON TO ALL EMPLOYEES.
EMPLOYEE WAS USING A LOADER TO CLEAR ROCKS FROM AREA AROUND DRILLING RIG. WHILE BACKING UP, THE FRONT LEFT WHEEL WENT OVER TOP OF 18" ROCK CAUSING THE LOADER TO LUNGE TO THE SIDE. THE EMPLOYEE WAS LOOKING OVER HIS RIGHT SHOULDER AND THE MOTION CAUSED WHIPLASH TO HIS NECK. AN HOUR LATER THE PAIN WORSENED. HE WAS TAKEN TO THE HOSPITAL FOR EVALUATION AND RELEASED WITH RESTRICTIONS.
EE WAS TORQUEING BOLTS & NUTS ON KILN TIRE FROM MANLIFT. AS HE TORQUED ONE SIDE, ANOTHER EE IN AN ADJACENT MANLIFT WAS TORQUING THE OTHER SIDE. THEY WERE SHARING THE TORQUE WRENCH & SOCKET & H AD TO REACH REPEATEDLY BACK & FORTH BETWEEN MANLIFTS TO SHARE THE TOOL. EE BEGAN TO FEEL BACK PAIN THAT EVENING & WAS SENT TO THE DOCTOR THE NEXT DAY & PLACED ON RESTRICTION FOR PULLED MUSCLE
EMPLOYEE WAS OPENING A 10"X8" DOOR TO CLEAN OUT A CLOGGED CHUTE. STEEL DOOR IS HELD IN PLACE WITH HANDLES AND TABS. HE LOOSENED DOOR AND WHEN HE PULLED IT OUT, THE WEIGHT OF THE DOOR WAS MORE THAN HE EXPECTED AND IT DROPPED SEVERAL INCHES, PINCHING HIS FINGER AGAINST AN ADJACENT STEEL BEAM. THE IMPACT CAUSED BRUISING AND FRACTURE OF HIS RIGHT FOURTH FINGER. DOOR HAS BEEN MODIFIED.
