While installing lagging on the head pulley, the pulley was being rotated using the drive belt. During rotation, employee's fingers were positioned too far back on the drive belt and motor pulley and two fingers became pinched between the drive belt and motor pulley, the tension prevented immediate release of the fingers. Drive belt was cut.

150 ton track press was being used to press track pins out of a 390. Press back plate had to be removed first. 2 cranes were used to support track press about waist high. Tech was positioned at back plate. Back plate was removed by sliding off guide rods manually by tech. When back plate came off guide rods, the press section cantilevered up striking tech's lip causing laceration.

While removing bowl assembly was nudging cylinder to make contact. Employee completed task 20 times, each time removing hand prior to pushing. He then rested hand on the rear of the cylinder. As it pushed, his finger was between it and another cylinder. It rotated pinching the end of the pinky finger. He lost the end 3/4" tip of his finger.

Employee was using a pry bar to hold a guard in place near a conveyor belt so that he could repair the guard. The bar came into contact with the moving belt causing the bar to spin and be pulled from his hands.

While standing on a rock face, started to turn around, lost footing, right leg got stuck between two rocks, causing the rt ankle & knee to twist, fell to the ground, and hurt rt ankle.
