The employee was installing a lower tool bushing in the primary crusher Rammer E68 hammer. EE was using a (hand held) hammer to drive in the lower tool bushing retainer pin. One of the times EE struck the pin sliver broke off the end of the 1 ¼" diameter lower tool bushing pin, and embedded in the employee's arm.

Employee was lifting a bucket containing an aggregate product sample for QC testing onto pick-up truck bed. While lifting, EE incurred a thoracic strain, resulting in restricted work duty.

After installing new forks, EE used forklift to facilitate loading of damaged forks into service truck. As EE slid and shimmed the second fork into the truck, a fork on the forklift shifted, allowing the heel of the damaged fork to drop in between the forks on the forklift. As it dropped, a bur on the damaged fork caught sleeve of EE pulling arm into the crush point between forks.

EE WAS CUTTING BOLT OUT OF A TRUSS FOR DEMO, AFTER THE LAST BOLT WAS CUT AND STARTED TO REMOVE THE TRUSS IT HUNG UP, ONCE IT BECAME FREE THE SHEETING BENT BACK AND CAUGHT EE IN THE MOUTH. HE WAS TREATED AND HAD STITCHES IN HIS UPPER LIP.

We told a contractor that we would no longer need their services on 01/29/2015. We saw this contractor 2 times after this date and there was no mention of an injury. We asked him to return our companies equipment on 02/02/2015 and he then told us that he hurt his back and stomach. I do not have anymore detail on the injury because this is all the detail that the contractor gave us.

**THIS IS A DUPLICATE REPORT OF THE CONTRACTOR'S 7000-1** Removed power to main switch of 4160 volt in MCC substation and locked out removing power to building. Removed buss bars cover to show other electrician what the scope of work was. While showing him, the injured contractor employee's hand touched the buss bar and arced through a wrench he was holding causing a flash.

Employee was removing and replacing the worn teeth on a Caterpillar 992G loader bucket. Employee struck the tooth with a sledge hammer and a piece of the tooth came off and punctured the employee's stomach. He was taken to a local ER where they did an MRI and decided to leave the small piece of metal in him. He received stitches and was released to full duty.
