Employee was working on the crusher, it was LOTO. EE's arm was pinned between the wall and the head.

Employee was placing new flashing rubber on conveyor when the clamp bar slid down and pinched finger.

EE was working with another EE to jack up the cab of a 980 loader in order to access a hydraulic pump. The loader was being raised implementally with two jacks. During the lifting process the EE noticed a wire was caught and reached in to free it. The jack slipped and caused the cab to drop and pinch the EE's arm.
