Sprained right ankle. Caused by employee bouncing on foot to give momentum to climb ladder into water truck cab. Ladder had been previously modified at this employee's request to make access easier.
2" pipe laying on shop floor. Miner broke right wrist.
Employee strained left shoulder and upper arm accessing water truck cab. Restricted duty to no repetitve lifting over 20 lbs, no pushing/pulling over 20 labs force, no reaching over shoulder, prescribed celebrex and vicoden.
While removing a layer of screens, employee strained his back as he was pulling and pushing and climbing around in deck.
EE WAS STARTING EQUIPMENT FOR WARMUP, DOOR HANDLE LEVER BROKE IN HALF CAUSING AND CUT TO RIGHT HAND
CLEARING MATERIAL FROM JAW CRUSHER WITH A PRY BAR, BAR SLIPPED AND HAND SLAMMED AGAINST A PIECE OF ROCK INJURING RIGHT HAND RING FINGER.
PUTTING IN NEW TRAIL PULLY ON CONVEYER, USING PRY BAR THAT SLIPPED AND HIT RIGHT HAND ON PULLYTHAT BROKE RING FINGER.
