While reinstalling hinged inspection door on main system fan housing, EE pulled awkwardly resulting in back pain. EE sought medical attention and was prescribed medication and given restrictions. Axiom was notified after the fact and is currently managing the case.

EE was assisting with plant maintenance and got a metallic particle in EE's eye. The particle was removed and the EE was prescribed an antibiotic ointment to prevent infection resulting in the classification being escalated from first aid to recordable.

Employee had both hands in contact with a 20x10x8 metal container box that was being lowered onto the ground from a flat bed truck by the track-hoe. During the transfer, electricity discharged from the overhead power lines and contacted the track-hoe bucket. the employee received burns to both palms and the outside of his right foot.
