Employee was standing in pinch point on deck of semi trailer when driver pulled forward without noticing employee still on deck. Injured employee suffered a fractured fibula and laceration to right lower leg.

Employee had started the engine on the water pump and went to engage the clutch. When clutch was engaged the coupling broke. As the coupling came apart, it distorted the shape of guard, pinching his thumb between the clutch and guard.

Employee was cutting crown bar rubber at the 2-deck shaker (cutting excess off), as he was cutting the blade cut his knuckle on his left thumb. We took him to the clinic for medical attention, the doctor put stitches due to the location of the cut not the depth.

EMPLOYEE WAS PREPARING THE DREDGE TO BE PUT ON SHORE FOR REPAIRS. SOME TYPE OF FOREIGN OBJECT FLEW INTO HIS RIGHT EYE. UPON SEVERAL ATTEMPTS OF WASHING THE OBJECT OUT, HE DECIDED TO WAIT UNTIL HE GOT HOME TO GET IT OUT. THE NEXT MORNING HIS EYE WAS SWOLLEN AND WE TOOK HIM TO THE CLINIC FOR MEDICAL ATTENTION

EE HAD PICKED UP AN ACETELYNE BOTTLE TO PUT ON THE WORK BARGE TO DO SOME MAINT. WORK ON THE DREDGE. WHEN HE TURNED TO PUT THE BOTTLE ON THE WORK BARGE HIS LEFT HAND CAME INTO CONTACT WITH A PAD EYE WELDED ON TOP OF AN ADJACENT PONTOON. THE FORCE OF THE CONTACT CAUSED HIS LEFT INDEX FINGER TO SMASH & BUST OPEN. HE WAS TAKEN TO THE HOSPITAL WHERE 10 STITCHES WERE PUT IN
