Kinder Sand Company LLC · Caught in, under or between a moving and a stationary object

Employee shut down Megatex then using a pry bar, pushed Megatex over and reached into pipe with right hand to remove obstruction. Pry bar slipped causing Megatex to slide back over employee's right hand. Right wrist and hand were lacerated and smashed.

EMPLOYEE WAS WALKING UNDER A PART OF THE PLANT TO GO TURN OFF A VALVE. HE DUCKED UNDER A PIPE. HE DID NOT DUCK DOWN FAR ENOUGH AND STRUCK HIS HEAD ON THE HANDLE OF A VALVE. THE HANDLE WENT UND ER THE BRIM OF HIS HARD HAT AND STRUCK HIS FOREHEAD. HE RECEIVED A 2" LACERATION WHICH REQUIRED SUTURES.
