Employee was bolting in #5 entry at Spad # 17+60 and as employee was walking back to pull up the bolter, ee hung the right foot under a rock lying on the bottom, tripping ee causing ee to fall twisting ankle as ee fell to the ground. Employee finished the shift and didn't go to the Doctor until that afternoon.

The right side pinner was moving from #7 to #8 entry. The Ram Car was setting idle with the pump motor on waiting for the other car to come out of #9 entry. Employee was moving cable out of the way for the pinner to pull into face of #8. EE was standing behind ram car when the car released brake causing ee to rollback striking employee in back of knee.

Employee was operating a Diesel Scoop about 30' from the top of the slope. The scoop died and rolled down the entire slope and hit a concrete wall at the bottom. The employee had a bump on head and was complaining of back pain. Employee was taken to hospital seen and released. This accident did not become a lost time until 11/28/17 when Doctor took employee off work.

Employee was cutting #6 entry when a rock measuring 12" wide X 30" long X 1/4" to 1.5" thick broke from the top striking employee on the head and left shoulder area. Employee said neck and shoulders were hurting so after injuries were assessed employee was taken outside and transported to hospitals.

EE went to move the 4 man diesel, pulled it up about 8-10 ft. to get it out of the way. EE said that a piece of rock fell and hit them in the head and left shoulder. This happened between 2:30 and 3:30am and they continued to work until around 4:oo when they were transported outside. Also MSHA was called,because the ambulance EMT called life-flight. EE was sent to St. Vincent.

EE was hanging an A-Frame and was holding it up with left knee. The A-Frame slipped off the stud and came down on EE as EE fell their right foot did not turn when their body twisting their knee. EE was treated at Muhlenberg Community Hospital and diagnosed with a fracture to the right medial tibial plateau.

Had a roof fall in the Main East. Time is unknown, Examiner and inspector found the fall while making the return air course on the weekly. Fall was at 39+20 from 2-9 in the return air course, it was about 480 feet long. The fall was inby unit # 1 in the return air course.
