EE was working on taking batteries off a scoop, the battery pin was stuck. EE was using a bar to try to pry the pin loose. The bar slipped and struck EE in the jaw causing a broken jaw.

EE was standing on a shuttle car cable with conversion valve out. As EE put the valve back in the cable took up while EE was standing on it, and it flipped EE over on right shoulder. X-rays showed no breaks and EE is home waiting an MRI.

Injured was cutting entry when a piece of draw rock (2" to paper thin) fell from in between rib and bolt striking them on lower back area. The X-Ray and CT Scan taken at ER were negative and employee was suppose to return to work. Another doctor took them off pending MRI.

Employee was putting up outside roof bolt using ee body to push it into hole. EE stated knee popped and started burning. EE continued working until 7/20/18 giving an excuse to be off until 7/24/18. EE did not return to work the next week and when contacted ee had doctor send excuse stating ee would be off until 8/28/18 due to knee.

Employee was putting up a roof bolt when EE mashed EE's right pinkie (little) finger between a roof bolt and pot of the roof bolter. On 08/11/2017 EE worked 5.5 hours then left to go to the doctor for a follow-up on EE's finger, at which time the doctor took EE off work due to a minimal distal fracture on the tip of EE's right little finger. EE began missing work as of 08/12/2017.

On 7 section in # 7 entry the Continuous miner hit a core hole in the top right of entry. Water came in the mines. No one was hurt, and MSHA 800 number was called. It was investigated by MSHA, and after grounding the hole. We are back to normal production.

Employee was helping load bolts on roof bolter on 1 section. As he loaded the bolts he said he felt a pain in lower back. He did not go to see doctor, and continue to work every day till Oct 6. He never said anything the whole time or on Oct 6 about back hurting. He called us Oct 7th, and said his doctor had him off on the strain from the 9-14-2015 incident.

2 ee working with a DC powered welder and oxygen/acetylene touches to repair a cutting bit lug on a continuous miner. Rubber hoses of the cutting torches accidental came in contact with the hot case of the bonder, melting the hoses and igniting the gases, causing a fire. Fire lasted 10 to 12 min was extinguished with water. No inj were reported due to this fire.
