RIVER VIEW MINE Coal

The operator dozed off while driving a mantrip, causing the trip to hit the rib. The operator hit EE's left knee on the dash of the ride resulting in sutures to close a cut. No work was missed.

A passenger of a mantrip that hit the corner of a rib was dislodged from the seat and landed on the mine floor. EE was diagnosed with two stable vertebral compression fractures in the spine. Started missing work on 12/02/2025.

When hanging a cable in the belt line during a power move, the employee felt a pop in left shoulder causing pain in an area that had been repaired before. They continued to work. Started missing work on 3/3/2026.

The mechanic was standing on the shuttle car fender that was laying on the ground. While they positioned themselves to fix the problem, they slipped on the side of the fender causing pain in left knee. Started missing work on 11/14/2025.

While lifting the framing at a header with right arm the belt foreman felt a burning sensation in right shoulder. Worked until surgery date. Started missing on 03/25/2026.

A roof fall occurred on the 4th southwest BL seal area. in #11 entry from spad 93+75 through spad 93+10. The fall measured approximately 85'x 19' x 8'. This fall will not be loaded out, and the area has additional support added.

Roof bolter was reaching into pin tray to retrieve a steel when a small head came from the roof smashing the tip of ring finger on right hand. Resulted in a fracture and started missing work on 10/29/25.

The roof bolter was drilling a hole for EE's rib pin when a rock fell hitting the canopy swinging in and striking EE's right shoulder, lower back and EE's right knee. The rock pushed EE's knee into EE's boom causing a contusion and swelling to the knee. Started missing work on 10/17/2025.

The miner operator used EE's index finger to rub EE's right eye causing dirt from EE's glove to enter and scratch the eyeball. Seen the doctor on 10/15/2025 where medication eye drops were prescribed and no work was missed.

While crossing over a supply trailer the tractor operator slipped and fell onto the ground. The operator worked until needing surgery to repair the left knee. Started missing work on 11/26/2025.

while hanging the miner cable and water line that goes to the miner, the shuttle car operator strained EE's left shoulder. Started missing work on 10/03/2025 and returned to work on 10/08/2025.

The mechanic was setting on a golf cart when a thin piece of draw from the mine roof fell striking the mechanic in the scalp. Resulting in staples to close the wound. No work was missed.

While drilling a hole for the roof bolting process, the operator's steels became clogged up causing the steels to bend under pressure. While looking back at the tray and reaching for the steels at the same time the bent steels struck the operator in the base of the thumb on the left hand. Started missing work on 09/24/23025.

Walking around the tailpiece EE stepped in a hole, slipped causing pain in EE's left knee. Started missing work on 09/24/2025 and returned on 09/26/2025.

Employee was helping load some belt framing on the 3/60N beltline. As EE was dragging a 60 inch top roller under some water line, EE felt pain in EE's lower middle back and down EE's left leg. Employee started physical therapy on 9/22/25.

The mechanic was pulling an old piece of high voltage into a crosscut and pulled EE's right bicep. The mechanic continued to work until surgery was needed to repair. Started missing work on 10/22/2025.

Employee was using a utility knife to cut down a piece of back up curtain and cut EE's left index finger. Received 4 sutures.

Employee was plastering a brattice when some plaster got in EE's right eye. Employee was seen by an Eye doctor and was given a prescription eye drop for a scratch on EE's right eye.

Employee jumped off a ledge at the dust hole onto a dust trailer. EE then jumped from the dust trailer onto the top of a diesel scoop and grabbed a trussbolt to catch EE's balance causing EE to strain EE's left shoulder. Started missing work on 09/08/2025.

The examiner was traveling in a golf cart when they stopped for traffic. A piece of slate fell, striking their hard hat and both arms. The slate broke, causing a laceration to left forearm that required sutures. No work time was missed.

As the operator of the roof bolter stood up and twisted at the same time, EE felt a strain in the middle of EE's back. Started missing work on 8/29/2025 and returned on 09/03/2025.

Employee was throwing loops of feeder cable up against a rib and strained EE's right shoulder. Started missing work on 09/11/2025.

The miner helper was cutting a crosscut when a piece of slate dropped striking EE on top of the hard hat and then hitting the back of EE's neck causing a laceration on the base of the neck then hitting the right leg below the knee. Staples were used to close the laceration. No work was missed.

The mine foreman was on a diesel golf cart traveling out on the supply road when a piece of slate dropped on right shoulder, right arm and right leg fracturing two ribs. No work was missed.

The Belt mechanic was in the bathhouse cleaning EE's belt knife when drying the blade of EE's knife the index finger on the right hand hit the blade rustling in 3 stitches. No work was missed.

Shuttle car driver was entering EE's car when a piece of coal rib struck EE in the back and the right ankle. Started missing work on 7/25/2025 for inflammation.

The operator of the bolter was tramming past the supply trailer and pinched EE's 4th and 5th fingers on EE's left hand between the trailer and the roof bolter. Resulting in 3 stitches in the pinky finger and a fracture to the ring finger. The employee started missing work on 7/28/2025.

Shuttle car operator was turning a tight muddy corner, hitting the rib with the bumper of the car it jarred EE's hand loose from the handle on the canopy, causing EE's left arm to get pinched between the car and the rib. Operator received two stitches.

The rock duster was dusting the belt line when EE had carried the dust hose back to the duster, EE felt sharp pain in EE's back. Started missing work on 07/22/2025.

Roof bolter operator was swinging out to install the rib pin when two rocks fell hitting the operator in the back and the left leg. Roof bolter started missing work on 7/22/2025 for inflammation.

The mechanic and miner operator picked up the rub rail for the miner to access the panel. The mechanic felt a sharp pain in abdomen. The mechanic started missing work on 7/30/2025 for hernia repair.

The employee was loading miner cable into the bed of a miner when EE felt pain in EE's right shoulder. No work was missed, continued to have pain, started physical therapy on 09/04/2025.

While raising the lids on a battery scoop the operator strained their groin. The employee started missing work on 7/22/2025 for a hernia.

While ratcheting a come along to pull the miner cat together the mechanic felt pain in right shoulder. No work was missed.

Reclaiming belt rollers, the outby worker was stacking rollers onto the ground when they caught their right ring finger between two causing a small cut. Two stitches in the end of the finger and no work missed.

While drilling a hole for the outside pin a head dislodged from the roof hitting the roof bolter operator in the right leg just above the ankle causing a fracture. Started missing work on 6/12/2025.

While installing a chain guard on a belt drive. The left middle finger of the belt mechanic was caught between the two chain guard handles. This was a two person install, when positioning the guard, it slid into the guard handle next to it. Antibiotics where given No work was missed.

Employee was replacing some hoist cable pads for the slope hoist rope. When EE went to turn and walk EE's left foot stayed planted on the ground causing EE to twist EE's left knee. EE started missing work on 05/22/2025

Employee was dropping EE's canopy with EE's right hand while placing EE's left hand on some boards near the front of the tray. The canopy pinched EE's left pinky finger and when EE reacted with a tug, EE pulled the nail off.

While the mechanic was cutting a pin out of a lift jack on a diesel scoop with a torch, the grease port was exposed letting the grease get hot and shoot out hitting the mechanic in the nose, cheek, and bottom lip causing a burn. Employee started missing work on 5/12/2025

Brattice man was attempting to break a concrete block with a brattice hammer when EE struck EE's left hand below the thumb causing a laceration that required 5 sutures. No work was missed.

While cutting a piece of belt with a belt knife, the knife slipped coming in contact with the right thigh causing a laceration, received 4 sutures. No work was missed.

While in a tight space laying on EE's side working on a bolter boom the mechanic was using a die grinder to clean a hole for a pin when the die grinder jumped back hitting the left forearm causing a laceration that took 4 sutures. No work was missed.

Roof bolter was moving when the reel got tight pulling the cable down hitting the roof bolter in the side of the hard hat causing a laceration that required 4 sutures. No work was missed.

While picking up a trailer hitch to install on a tractor the employee strained their lower back. Started missing work on 04/29/2025.

Air compressor was down on the P3 shaft. This was not an accident. A 7000-1 form is being filled out to terminate citation 9760488.

Employee was installing a bit into the miner head. Driving the bit in with a hammer a piece of carbide came off the bit embedding in the right arm below the bicep. No work was missed.

Employee was cleaning out around a clamp on a waterline and cut EE's right index finger on a sharp rock. Glued the finger and no work was missed.

Employee was drilling a hole for a roof bolt. EE was reaching for a second steel and when EE turned back around the first steel fell out of the mine roof and bounced off the drill pot knocking EE's glasses off causing the steel to hit EE on the left eye. EE was seen by a doctor and was given eye drops and a protective contact lens.

Employee was scattering rock dust out of a scoop bucket with a shovel. As EE went to sling the shovel of dust, EE hit the scoop bucket with the shovel causing the shovel to abruptly stop causing a torn bicep on the left arm. Started missing work on 4/3/2025

While exiting the scoop the scoop operator twisted their left knee. Started missing work on 02/20/2025.

Roof bolter strained EE's lower back loading pin boards onto the bolter. Started missing work on 02/25/2025.

Roof bolter was trying to connect two steels together. The steels were touching each other with a little boom pressure and when EE pulled on the one in the chuck, they popped together hitting the right thumb causing a fracture. Worked on 02/05/2025, started missing on work 02/06/2025.

The roof bolter was attempting to cut a suction hose with a belt knife. Holding the hose in EE's left hand and cutting with the right, the knife came out of the hose and cut the left hand. Received 6 sutures, no work was missed.

While preparing the belt for a belt lay the belt mechanic was cutting the belt, when the knife slipped out of the belt and cut right leg just below the knee. Causing the belt mechanic to receive 10 staples, no work was missed.

While setting in between the air doors the outby set was closing, forcing air/debris up under the inby set causing irritation to the right eye. The employee did not miss any work.

Miner operator was starting to load a car when the face fell in leaving a pile of rock just inby the last row of bolts. A rock then fell out just inby the last row of bolts landing on the pile of rock causing the rock to slide in on the operator's left ankle. Resulting in a fracture the miner operator started missing work 1/24/2025.

While the opposite operator had EE's hand on the T-bar, the operator was spotting a pin outby, dropped EE's boom hitting the ATRS pinching the opposite operator's hand between the pad and T-bar. This caused a laceration that required 12 stitches and started missing work on 1/16/2025.

Opposite operator roof bolter was drilling a hole for a roof bolt when the operator pinched right index finger between a board and the drill pot. Started missing work on 12/20/2024.

Mechanic was cutting the outer jacket on the miner cable to make a splice, slipped and cut the index finger on EE's left hand. The cut required 6 stitches. No work was missed.

While walking to the opposite operator boom to install a roof bolt, the employee slipped and fell on a rock. The employee caught their body with their left hand spraining wrist. Started missing work on 12/12/2024.

Mechanic exited a ride, stepped over a berm in the road and rolled their right ankle. Started missing work on 12/03/2024. Update: Started missing work again for the same injury on 4/1/25.

The miner operator was cutting some hard coal and a piece of steel off of the miner bit hit them in the neck. This required 4 sutures and no work was missed.

While using a cut off wheel on a grinder to cut a lock on a golf cart, the grinder kicked back and cut the mechanic on the top of EE's left hand, requiring sutures.

While installing a roof bolt the operator had left hand on the top of the board and smashed their middle finger upon contact with the roof. Started missing work on 11/22/2024.

Employee got a foreign object in left eye. The eye doctor flushed it out and employee received medicated eye drops, employee did not miss any work.

Employee was riding in the passenger seat of a diesel jake. At idle speed the driver swerved to miss something in the road and the passenger fell off of the ride, causing pain in right shoulder. Started missing work on 11/11/2024

A grinder was dropped by a belt mechanic while the blade was engaged, hitting a two man diesel ride, it bounced off of the jake and hit the index finger on the right hand of a spectator causing three sutures. No work was missed.

Operator dropped the boom when hitting the pin that was made up to put in the roof, the board that was on the premade roof bolt slid over and the boom caught the board which hit the operator in the right jaw causing a decade tooth to break on the upper left side. Missed one day of work on 10/25/2024

The employee was cleaning rocks off of the top of the miner when they felt a strain in their lower back on the right side. The employee started physical therapy on 11/04/2024. No work was missed.

The employee was loading the pin tray with roof bolts and felt a sharp pain in lower back. Started missing work on 10/08/2024

The employee had reached across body to pick up a wheel and tire, as employee picked it up to install it onto the golf cart employee's left shoulder popped. Started missing work on 10/04/2024.

While trimming a piece of metal off of a scoop with a cutting torch a piece of hot metal bounced up getting behind the cutting glasses burning the left eye.

Employee was helping put a spool of leaky feeder cable on a spool cart and pinched right index finger between the spool bar and cradle. Employee received 4 sutures.

Employee was a passenger in a mantrip when the operator of the mantrip hit a rough spot in the road causing the injured employee to hit their lower back on the edge of the seat. Started missing work on 09/26/2024.

Employee was climbing a bank out of the dust hole and slipped falling onto the side of a dust wagon landing on EE's right side under EE's right arm resulting in two fractured ribs.

The employee was standing on the opposite side of the bolter and the operator repositioned the bolter running over the toes on the right foot of the opposite operator causing a fracture to the big toe and the third toe. Started missing work on 09/25/2024.

While setting in the shuttle car at the feeder the corner of the rib rolled off and some of the coal fell into the cab pinching the skin on the back of the right arm causing two small lacerations. 3 sutures in one and 5 sutures in the other.

A roof fall occurred in the 4th south west sub mains, number 1 entry between xc 159 and xc 160 at spad 108+80 and 109+55. The fall measured approximately 65'x19'x8'. The fall will not be loaded out and the area has additional support added.

The employee knelt beside the tool slide doing inventory, when a rock fell out and struck EE in the top of the head and then the right knee. 3 staples were required in the cut on the head. no work missed.

While installing the outside roof bolt, a rock fell out hitting the canopy of the bolter. The rock then fell off of the canopy hitting the operator in the right ankle. Taken off work on 09/05/2024 with a facture to the ankle.

The employee exited the shuttle car and tripped over a piece of coal. Falling to the ground employee's rib cage on the left side landed on a chunk of coal that had fallen out of the rib, resulting in a fractured rib. The employee did not miss any work.

The employee was changing a 72" bottom roller, when the employee picked the roller up to put it in the stand employee felt a burning pain in lower back. The employee was taken off work on 8/21/2024.

Employee was drilling the second steel up. The second steel broke and a piece of the steel hit the rib and then hit EE in the back causing a 1" cut that required 5 sutures. No work was missed.

The employee was walking through a curtain while the mine roof was lower on the other side hitting EE's head on a low hanging damaged roof bolt. Jamming neck causing pain in EE's neck and shoulders. Started missing work on 8/9/2024.

Employee was stripping the wires for a light on a roof bolter when the knife EE was using slipped causing EE to cut left pinky finger resulting in the employee receiving 4 sutures.

Employee was in the front passenger seat of a mantrip that was traveling the supply road to bottom when it drove through a clear curtain and struck the bucket of a diesel scoop that was backing into a crosscut. The injured employee hit their head on the mantrip resulting in a laceration on EE's forehead. Employee was taken to the ER and received glue on the wound.

Employee was pulling a trailer off the cage with a diesel tractor, the employee had hand resting on the bump stop in the pivot point of the diesel tractor. As EE made a turn, the ends of the tractor came together, EE's right pinky finger was caught in between the bump stops. This resulted in a distal fracture in the right pinky finger and also required 14 sutures.

The employee was drilling a hole in the mine roof and a donut shaped rock slid down the drill steel and flew off striking the employee just above employee's top lip causing a laceration resulting in the employee getting 5 sutures.

While setting bits on the miner, hitting a bit with a hammer a piece of metal 1/4inch long from the hammer imbedded in EE's right forearm causing infection.

The employee was loading pin plates onto the bolter. Employee swung around to place some more plates on the bolter and hit right elbow on the plates causing a laceration receiving eight sutures.

Employee was pulling a yield prop across the belt when it dropped of the belt and onto the belt framing causing a laceration to EE's left middle finger requiring sutures.

Employee was loading gob with a continuous miner and a rock that was on top of the miner slid down the tail of the miner and onto the shuttle car and then slid off the car onto the employee's left foot. Started missing work on 6/21/2024 due to a fracture in left foot.

Employee was picking up crib block out of a scoop bucket and strained lower back.

While driving a golf cart, the front tire hit a rock causing the steering wheel to spin out of control and stopping the cart. The steering wheel caught EE's right hand dislocating EE's ring finger.

The roof bolter operator was cleaning the dust box on the opposite side of the bolter. While squatting down a rock struck the roof bolter operator on top of the head causing a headache. The rock fell between the rib and the rib pin. The roof bolter operator was taken off work starting 6/17/2024.

The employee was eating a sandwich when a thin rock 12 inch circle fell out hitting the employee's right middle finger causing a cut that required 6 sutures. The finger became infected. The employee started missing work on 6/21/2024

Employee was putting EE's drill steels together when moving the boom up to start drilling in the bolting process, the steels popped together with EE's hand close enough to the steels hitting EE in the base of EE's left thumb resulting in a fracture. Started missing work 6/13/2024

With very little space between the grader and tractor the grader went to back up as the employee proceeded to move between the two pieces of equipment. The grader turned around to look behind and accelerated in forward instead of reverse, catching the employees' leg between the two pieces of equipment. Resulting in a fracture to the right leg. Started missing work 6/13/2024

Employee was putting steels together in the bolting process when they did not match up causing it to pop EE's right hand. The steels hit the webbing of EE's hand between the thumb and index finger resulting in 8 sutures. Employee started missing work on 6/11/2024.

Employee was swung out to put rib pin up when a rock fell hitting them in the calf/knee area causing them to shift their weight on their knee. Started missing on 6/7/2024

Employee popped the steels together with the boom pressure, in their left hand. Employee worked the next day and was taken off, started missing 6/6/2024.

Employee was organizing the supplies on the top of the bolter, when sliding off the side of the bolter employee's feet hit the ground twisting left knee. Employee started missing work on 05/29/2024.

Employee was loading cable bolt plates in the tray of EE's roof bolter. Reaching to put the plates into the tray EE slips and smashed index finger between the plates and the steel tray. Resulting in a fracture.

Employee was helping chain up a foot jack and boom on a roof bolter that was being supported by a scoop bucket when EE's right hand was smashed between the boom and scoop bucket. EE had surgery on 5/13/24.

Employee was operating a continuous miner cutting a crosscut when a piece of metal pierced employee's left bicep. The piece of metal was removed and required 1 butterfly strip. Employee returned to work on employee's next shift.

Employee was using a porta power trying to break the cab canopy loose on a roof bolter. The electric porta power was loading up when it shot out of the cab contacting the employee in the mouth. Resulting in a laceration to the upper lip with 3 sutures and 3 teeth fractured.

Employee was loading a belt roller into a scoop bucket when employee turned around to get another roller and struck cheek on the roof causing a small cut resulting in one stitch. Employee returned to work the next shift.

Employee was straightening supplies on the idle bolter just inby the last open. While bending over a rock fell hitting the employee in the head and neck area, causing a strain to the neck. Started missing work on 4/9/2024.

Employee tripped over a tray made of boards and hit EE's right shoulder on the supply trailer. Employee started missing work on 04/22/2024.

Employee was walking across the last open crosscut when EE hit head on a low hanging brow causing EE to jam neck. Started physical therapy on 10/22/2024.

Roof bolter was reaching for the steels, when a rock slid down the steels smashing index finger between the rock and drill pot. Resulting in an open fracture. Worked shift on 4/5/2024, taken off work on 4/8/2024.

Employee was loading an oxygen bottle on the back of the ride when EE felt a pain in lower back, EE finished shift and went to the doctor and was taken off of work.

Miner was struck by a damaged roof bolt hanging from the roof in the left ear causing a laceration that required 7 stitches. Miner was injured on 03/07/2024.

Miner was struck in the right ear by a rock falling from the roof, causing a laceration requiring 5 stitches. Miner was injured on 03/05/2024.

Miner was strapping supplies onto a trailer, using a small pry bar to tighten straps. The pry bar came loose and hit miner in a tooth, causing it to chip.

Miner lifted the back of the mantrip off the ground with a jack, stepped between the ride and the rib to place boards under the rear tire. The jack fell over catching the miner's leg between ride and the rib, resulting a fracture to the right leg near the ankle. The Miner was taken off work starting 2/16/24.

Employee was bending a roof bolt when they felt a pain in their left shoulder. Employee went to the doctor on 02/14/2024 and was taken off of work for 02/15/2024 then returned to work on 02/16/2024, missing 1 day of work.

Pushing a rock off the fender of the car, when operators hands slipped causing the rock to land on their right hand.

Employee was walking through the unit regulator when they moved a hollow concrete block out of their way and felt a pain in their left bicep. They continued to work until 02/12/2024 when they went off of work for surgery.

Employee stepped on a piece of rock or coal twisting the left ankle on 01/24/2024.

Employee was backing a mantrip and struck EE's foot.

Employee was swinging the boom out when a piece of rock fell out hitting the boom causing it to bounce off striking them in the right knee. Employee finished the shift and went to see their doctor the next day who put them off of work on 01/17/2024.

Employee picked up a bucket of bits with their right hand and tossed it onto the scoop resulting in pain in the right shoulder and neck. Injury was on 01/16/2024 and started missing work on 02/01/2024

Employee was loading structure and felt pain in right shoulder. Employee finished shift and started missing work on 01/08/2024.

A roof fall occurred in the 9A panel, No. 5 beltline entry from crosscut No. 7 to No. 8 at spad 4+23 to spad 5+09. It measured approximately 70' x 19' x 8'feet. The fall will be loaded out in accordance with the approved action plan and rehabilitated.

EE was hauling rock dust in a ram car, when EE ran over something in the road causing sharp pain in their lower back. EE worked until 4/2/2024.

Employee was starting EE's steel when a piece of rock went over EE's safety glasses and irritated EE's right eye, EE finished the shift and went to the eye doctor the following afternoon. The doctor gave EE restrictions and EE missed work on 12/19 to 12/20/23 returning to work on 12/21/23.

Employee was sitting in a battery scoop when a piece of rib fell off and hit EE on EE's right cheek resulting in 5 stitches. EE received a full duty release the same day.

Employee was helping pull belt on when the clevis broke allowing the pull rope to strike EE on their left forearm causing a fracture to EE's arm. EE started missing work on their next scheduled shift 12/11/2023.

Employee was walking to the cage on the bottom and bumped EE's head on the roof resulting in EE missing two days of work. Employee was off 12/11/2023 and returned to work on 12/13/2023.

A roof fall occurred in the 1C panel, No. 6 beltline entry from crosscut No. 225 to No. 226 at spad 173+04 to spad 173+94. It measured approximately 90' x 19' x 7' feet. The fall will not be loaded out and the area has had additional support added.

Employee was pulling two stacks of belt together when the clevis broke causing EE to receive 3 stitches in their cheek. EE was released for full duty the same day.

Employee was hooking a header cathead up when EE felt a pain in groin. EE continued to work until taken off of work on 12/06/2023.

Employee was walking and stepped on uneven ground causing EE's left knee to hurt, EE continued to work until the doctor took EE off of work on 01/09/2024.

Employee was moving the miner cable out of haul road when employee felt a pain in groin. Employee continued to work up till 12/05/2023 and was taken off of work then.

Employee was checking the fire suppression system on the 3rd 60 North header when the test valve slipped out of EE's hand and struck EE on the left forehead resulting in EE receiving 2 stitches. EE returned to work the next shift.

Employee was moving pin plates and cut arm on one requiring EE to receive 4 stitches. EE returned to work their next shift.

Employee was pulling a roof bolt out of the tray and felt a sharp pain in elbow. EE finished the shift and went to the doctor the next day resulting in EE being taken off of work.

EE was rehanging a miner cable when EE pulled their groin. EE worked until 4/2/2024.

Employee was pulling a pin out of the tray when EE hit left pinky finger resulting in 3 stitches. EE returned to work next shift.

Employee was cleaning the duct work out on the miner when the duct work door fell on EE's left hand. EE went to the doctor and was given restrictions leaving EE not able to do EE's regular job.

Employee was riding in a mantrip when the mantrip slid into the rib causing EE to bump the SCSR against the inside of the mantrip resulting in a bruise of EE's left hip. EE went to the doctor and was given restrictions and started missing work on 11/02/2023.

Employee was cutting some high voltage guarding when the belt knife slipped and cut EE's leg resulting in 3 stitches. EE returned to work next shift.

Employee was bolting when some gob fell out hitting their right middle finger causing them to receive 4 stitches. They returned to full duty 10/25/2023.

Employee was putting extra support in when the cable bolt they laid out flipped up hitting them in the left ear which required 5 stitches after they finished their shift.

Employee was loading pins on the roof bolter when they felt a pain in their right shoulder, they finished the shift and went to the doctor the next day and received restrictions which they worked with for 3 days returning to full duty on 10/23/2023.

Employee was driving their car and ran over a rock in haul road and felt their back hurting, they finished the shift and went to the doctor the next day and was taken off of work on 11/15/2023.

Employee was hanging a curtain when some gob hit their neck EE finished shift and neck was hurting the next day on 10/05/2023 so they went to the ER and received a full release to return to work the next day. EE continued to work until they went to their doctor and was taken off of work on 11/13/2023, they returned to work on 11/21/2023 with a full duty return to work slip.

Employee was splicing a miner cable in the LOCC when a battery scoop backed into them resulting in them being taken to the hospital and all tests were negative. Employee returned to full duty on 10/02/2023.

Employee was changing a bottom roller on the slope belt using a chain hoist when EE's right leg felt a pain in the right calf. EE finished EE's shift but was taken off of work on 09/25/2023.

Employee was building a brattice and when EE threw a bag of plaster down EE felt something pop on EE's left groin area resulting in EE having a small hernia. EE started missing work on 9/23/2023.

Employee was walking through the revolving door on bottom with hands full, EE was pushing with one arm & felt a pain in right shoulder. This was originally deemed a non-work related accident and was denied by workers compensation. On 6/17/2024 when the employee started missing work it was picked up by workers comp. Employee was off for 37 days 9/21/2023 to 11/13/2023

Employee was watering EE's ram car batteries when the battery lid fell on EE's right index finger resulting in a fracture to EE's finger. EE continued to finish EE's shift but started missing work on 9/20/2023.

Employee was connecting the pinner steel under pressure when they popped out hitting the employee's left hand resulting in a fracture to the left thumb. Employee started missing work on 08/28/2023.

Employee was riding on a 10 person diesel mantrip in the middle compartment behind the driver facing forward when another 10 person diesel mantrip struck them in the side resulting in the employee being injured. EE became a lost time injury on 08/18/2023.

Employee tripped over a cable causing EE to fall down and now has pain in the right shoulder, resulting in EE seeing a doctor and started missing work on 08/19/2023.

Employee was getting up off of the ground when EE put right hand on a car cable and felt a tingle like EE had been shocked. EE finished shift and had the mine nurse practitioner check EE out and the EKG showed good. EE still felt some pain so EE went to the doctor and received a full release but did not return to work until 08/21/2023.

Employee was bolting when employee held on to the roof bolt and accidentally hit the rotation causing hand to be twisted resulting in hand being sprained and was taken off of work on 08/16/2023.

Employee was adjusting a roller on the beltline when EE twisted back, resulting in EE having surgery on their back. EE started missing work on 08/15/2023.

Employee was holding a 100' metal tape line when a car hung into the tape line causing it to cut their right ring finger and left middle finger resulting in them receiving 6 stitches. They returned to work on their next shift.

Employee was unloading a pinner pot when they caught their left index finger between the pot and the golf cart resulting in them having to get 6 stitches in their finger. They returned to work their next shift.

Employee was backing car up when EE's left got caught between the cab and the rib resulting in EE having a small fracture in their foot.

Employee was opening the gate on the cage when the latch pinched their right middle finger resulting in them having 4 stitches put in. Employee returned to work the same day.

Employee was bolting when EE bent the pinner steel causing it to strike EE's right hand. EE went to the doctor and was given restrictions. EE started missing work on 07/28/2023.

Employee bolting when EE's drill steel came out of the roof landing on EE's rotating pinner pot causing the steel to swing around hitting EE under EE's right eye resulting in EE receiving 6 stitches.

Employee was bolting when the pinner steel bent striking employee on right wrist. Employee finished the shift and decided to go to the doctor on 07/24/2023 resulting in the doctor finding nothing broken but putting employee on restrictions taking the employee off of work at that time.

Employee was replacing a 72" belt roller when EE felt a pain in EE's groin and back. EE continued to work till EE went off of work for back surgery on 01/04/2024.

Employee was bolting when EE was guiding the roof bolt into the chuck and EE accidentally hit EE's rotation causing EE's glove to get caught resulting in a strain to EE's right hand and being taken off of work with doctor restrictions on 6/30/2023.

Employee hit left index finger with a hammer while hanging a curtain, resulting in employee having 2 stitches in the finger.

A roof fall occurred in # 10 entry on the 9C panel starting at spad No. 43+20 - 44+50.The fall will not be loaded out. The examiners route has been moved.

Employee was working on a new header when EE's left foot slipped causing EE's left ankle to swell up. EE continued to work until 07-10-2023 when the doctor took EE off of work.

Employee was pulling an old roller off of the beltline and felt a pain in lower back. EE continued to work until the doctor took EE off of work on 06/01/2023.

Employee was lifting a shear jack and felt pain in lower back. After going to the doctor, employee was given restrictions and taken off of work on 05/01/2023.

Employee was washing a piece of equipment when they stepped in a hole hurting their left knee. They continued to work until the doctor gave them restrictions and they started missing work on 05/10/2023.

Employee was changing out the ATRS T-Bar pin & bushings when the ATRS rolled off striking right thumb resulting in them having surgery and being off of work starting on 04/26/2023.

Employee was letting the belt blocks off when the block slipped and hit employee in left shoulder. Employee was seen by the doctor on 04/20/2023 and employee's restrictions keep employee from working.

A roof fall occurred on # 5 unit in the # 2 entry from crosscuts 17-19. The fall will not be loaded out. The # 5 unit moved two panels outby to a new panel.

On 4/14/2023, the employee was walking through the airlock on the unit catching EE's foot, EE fell to the mine floor, landing on EE's left shoulder causing pain. The employee continued to work, started missing work on 8/12/2024.

Employee was closing the lid on a large tool box when the lid fell on to EE's right hand causing a fracture of the little finger and EE was taken off of work.

Employee was bolting when they got their right hand between the bottom steel and the roof causing them to fracture their little finger. They started missing work on 03/27/2023.

Employee was loading roof jacks into a scoop bucket when they felt a sharp pain in their right wrist. They went to the doctor on 3/23/23 and was taken off work. They returned to work on 3/28/23 missing 3 days of work.

Employee was shoveling on the beltline when right knee popped out of place on the employee resulting in employee being taken off of work.

Employee was walking around EE's car when EE tripped on a cable and fell into the car causing EE to fracture two ribs resulting in EE missing work.

Employee was drilling a hole for a cable bolt when the bottom steel bowed and popped employee's left hand resulting in employee receiving 5 stitches in the hand and employee returned to work full duty on 03/13/2023.

Employee was drilling a hole for a cable bolt when the bottom steel bowed and popped employee's right hand resulting in employee receiving 5 stitches in the hand. Employee did not miss any work and returned full duty.

Employee was helping the mechanic pull the cable off of the bolter when a piece of gob hit them on the arm resulting in them receiving 3 stitches. EE did not miss any work and was back at full duty.

Employee was swinging EE canopy when a rock slid off and bounced up striking EE in EE's mouth chipping EE's front tooth resulting in EE having EE's tooth repaired. EE did not miss any work.

Employee was holding up a cable to let a low trac go past when the rear tire of the low trac caught the back of EE's left heal causing EE to go to the doctor where x-rays showed no broken bones just a contusion. The doctor had EE with restrictions and EE is not back to work.

Employee was pulling a top steel out of the hole when EE felt EE's shoulder pop. The employee had EE's shoulder looked at and was put off of work for two shifts returning to work on 02/14/2023.

Two roof falls occurred in the No. 10 entry. The outby fall went from just inby crosscut No. 224 to just inby crosscut No. 226. It measured 160'x 18'x 9'. The inby fall went from just inby crosscut No. 228 to just inby crosscut No. 229. It measured 80'x 18'x 8'. The fall area will not be loaded out and the area has had additional support added.

Employee was shoveling the belt line when EE felt a knot on the groin area and went to a nurse and was treated for a infected lymph node, then was later diagnosed with a hernia and was off work on 03/29/2023.

Employee was taking a fire suppression fitting apart when the pipe wrench hit employee in the mouth causing a tooth to be broken. Employee did not miss any work.
