Installing attachments to Hi-Wall Miner. Employee had finger between Miner and attachment. Cut tip of finger #4 (ring finger).

Employee got too close to berm on right side striking berm, losing control & crossing road striking left berm before regaining control of rock truck. Hit right arm on glass causing laceration. He bruised hip on seat frame. Employee told company official & foreman he was driving too fast. Employee tested positive on drug test.

Employee alleged he backed over a rock & the dozer jerked. Alleged his neck hurt later. Worked on 5/29/09, told no one of any injury. Employee was absent on 6/1/09. Was contacted by company official and alleged he had hurt his neck.

As EE was returning to equipment after lunch, a gust of wind blew dust into his eye. EE rubbed his eye to remove dust particle, resulting in a scratch on surface of eyeball.
