Employee was using a Razor Type knife to cut ties from hoses. While cutting away from body he had his left hand in line of fire. The knife went through the ties and struck his left index finger, resulting in an incision that required 3 stitches.

Employee was loading binder on coalburg seam. When attempting to load a bucket of material the bucket caught a hard spot that stopped the loader, jarring his whole body especially his back. He contacted the Foreman and denied transportation to the ER. Was X-rayed and told nothing was broke, He started missing work on the 28th and ruled compensatory on March 14.

Employee said he was sick, had M.E.T. check him out, his B/P was 190/110. He was transported to ER. After arriving at ER employee said that while pulling cap off of Buffalo seam he caught a hard spot that jarred his neck and back. He worked for a time and the pain and stiffness continued until he got sick and vomit. X-ray was negative and Dr. took him off work starting 12/17/2010.

While parking Grader near end of shift, employee noticed the small door on the right rear at radiator was opened. Holding to hand rail, employee walked to door and while bending over to shut door, his feet slipped and all of his weight was on his left arm and shoulder. He went home and next morning went to Dr. He was taken off work.

Injury along with other employee was removing a transmission pump from a D11R dozer. He had hooked the pump with a sling onto the boom of his mechanic truck and was in the process of lifting the pump out of the dozer. The pump caught on something and fell, hitting him on the right side of head and right shoulder. Taken to ER-no broken bones, worked until 6/21/2010, LTA

Employee was sitting in a 375 Komatsu dozer at the beginning of shift. The other equipment was parked in line behind him. A D11 dozer was parked directly behind the fill dozer and in the process of getting out of line and to the work area, his blade hit the ripper shank on the 375 dozer. At 7:49 pm the employee notified the foreman that his neck was hurting and he was taken to ER.

Employee notified foreman about 8:30 pm on July 14, 2009 that his back had started to hurt. He asked the foreman to fill out accident report Jarring of back from truck was mentioned He was offered a ride to the ER but declined. He went to his personal Dr. the next day and was excused from work. He returned the 17th but left after 4 hrs. 8/3/09 accident determined to be chargeable.

Employee stepped in between the grader blade and tire to start up the ladder to the cab of grader. He twisted left knee and fell to the ground. He stated that he heard a popping noise and was unable to put any weight on or stand. He was transported to ER and X-ray showed nothing broke. He seen Family Dr. on May 1, 2009 and returned to work on May 4, 2009. LTA began on May 26, 2009.

The D11 dozer track jumped segment sprockets and jarred operator's back. Foreman made note of complaint by employee, but accident was not reported to company until 08/14/2009. After investigation it was determined to be a reportable on 08/21/2009.

While replacing cutting edges on a D11R dozer, employee was using an air hammer to remove the dirt and other material from bolt heads. A foreign object was blown behind his safety glasses and into his eye. He went home and used some eye wash Friday and Saturday night. The pain increased on Sunday and he went to ER.

Employee was assembling a new 993 wheel loader. Employee was removing bolts that secured rollover protective structure to shipping pallet. When employee cut the first bolt the ROPS shifted breaking the other bolts and fell on employee's foot.

EE & partner were removing wear plates from bed of rock truck. Employee was using an air chisel to clean plates. his partner was using a hammer to knock plates off. While doing this a piece of metal chipped off struck employee over his right eye at the hairline embedding in his scalp. They both were wearing safety glasses and gloves.

Servicing excavator. After refueling he was in the process of returning the nozzle to the grease truck. The nozzle was not swiveling properly and had leaked some fuel on the tracks. The nozzle gave a twist when he pulled it out and caused him to fall. He landed on right knee & fell to the ground, hitting the back of his head on the tracks. He was unconscious for a few minutes.

EE was working in a new shot making a loading pad. The material was on a different level than the previous shot. In order to move material, he was running the loader bucket into solid rock. On one of the passes he felt pain in stomach. The pain increased until he called the foreman and transported to ER. CAT scan found hernia. Started missing work on 7/16/08, history of hernias.

Employee raised left hand to keep boom from powder truck from hitting in the face. They were loading holes to be shot. the boom came in contact with the outside part of left hand causing injury. He finish his shift and continued to work until Thursday June 5, 2008 before going to ER.

Employee was changing teeth on 992G loader bucket (#2366) while hitting teeth with hammer a piece of metal came off of loader and struck him in the chest. It left a small laceration and some bleeding. The foreman was witness and transported to er. x-ray showed no metal present and wound needed 3 stitches. He return to work.

Employee stated that while running the Kamatsu 375 Co. # 064 dozer on the fill he backed over a rock and jarred his back which he had hurt on 2-02-08 about the same way but had not reported it. He was transported to the ER. He was X-ray and told everything looked fine but on a follow-up told that there might be something wrong with lower back.

Employee was removing tailgate from a John Deere articulating truck. He had secured a chain to the tailgate and an excavator to take the pressure off of the pin holding the gate. He decided to cut the bolt that holds the pin. After cutting the bolt the tailgate swung, striking the employee in the back on his right side just above his hip.

Employee had switch trucks earlier in shift and was returning to the first truck to pick-up something he had forgot. Both trucks were CAT 785B Rock Trucks. He pulled up beside of the other truck and was attempting to go from one cat-walk to the other one. He lost his footing and fell to the ground. The fall was app. 11 feet.

Employee was building a road above the Taylor Seam in recently shot material. As he was backing up he ran into a rock that had rolled in his tracks. This caused the dozer to come to a quick stop, that jerked and jarred his back. He finished the shift and on his way home he came back to tell foreman of incident. He worked the next shift then has been off since 7/14/2007.
