Employee was in the surface shop and bumped a hand against a circular saw blade causing a cut on right hand. The cut was on the knuckle between the index and middle finger. Employee was taken to a medical clinic in Hazard, KY and received 4 stitches.
Employee was on the operator side of roof drill installing roof strap. It was the first row of bolts in #4 entry, and employee was installing outside bolt and was trying to install strap to hold loose rock. When the T-Bar was lowered a piece of rock measuring 7' long 3' wide & 3 inches thick slid down the T-Bar and struck EE in the left lower leg and ankle.
Employee was struck by a piece of draw rock 3' long by 2' wide 4 to 6 inches thick. The rock glanced off the employees hard hat and struck right hand. Employee felt pain in neck and right hand.
Brake stopped working had to run new wiring.
While installing a roof bolt, a piece of rock dislodged from the roof and struck IE in hand. Incident was not the result of a training or compliance issue.
Employee was using hand to steady the drill steel and got glove causing in a rotating steel. Employee's finger twisted around the steel causing injury.
Rockfall Occurred between breaks #67 & #68 of the 1 West Mains
Rub Rail of Continuous Miner Fell on Employees Right Foot
A rockfall was found by second shift at break 67 on 1 West Mains.
Employee was in a scoop bucket going to the batteries first and the breaker knocked on the scoop which in turn locked up the park brake striking EE up against the ejector blade.
Employee was helping wash slope when employee fell on foot hurting ankle.
Employee bent down to check water line coupling when EE stood up the left knee popped and EE felt pain in knee. However EE came back to work the following day and worked 5 days before starting to miss work on May 1st. EE did not notify us of absence nor did the mines.
Employee was cutting a hole in a bit bucket. Knife slipped and cut employees left hand.
Employee taking vent pipe down from old charger barn and when employee went to pitch vent pipe the edge cut through employee's glove and cut the palm of the hand.
Employee was bolting top when glove got caught in the drill steel, twisting hand.
Employee was throwing roof bolts off of the bolter toward the unbolted face when employee felt pain in lower back.
Employee was unloading jacks from supply cars when the jack got hung in jacket pocket twisting employee's arm causing pain in left shoulder.
Employee was pulling conveyor chain over apron assembly when ee felt sharp pain in back and legs.
Employee was shoveling V wipe at 2B head drive. They had right hand on the belt rail when a piece of draw rock fell from the top and hit the hand mashing their two middle fingers.
Employee was walking and draw rock fell and hit employee in the back as employee was walking toward the LoLo the equipment was backing in the hole and caught softline against Lolo causing softline to strike employee in the ribs.
Hoist car was down due to broken brake band.
Employee was walking up belt entry when employee hit head on brow in a dip causing pain in neck and down back.
Employee got right index finger caught between coupler on slope car.
Employee was making EE's weekly fan exam, when EE got off EE's buggy to get an air reading and once returning to EE's buggy EE slipped and fell.

Employee was tightening outer drum bolts on a continuous miner head, after completing this task EE then went and sat on a personnel carrier, after sitting for a while EE got up and stated back was stiff and hurting. Employee was using foot and leg to push down on the ratchet.

Employee was helping splice 1A belt. EE was checking brads, rubbing fingers across the buckled when EE's coworker struck EE with a 4 pound hammer.
Employee was putting top roller in section belt at the back of the lolo. When EE lifted the roller to put it in, EE felt pain in EE's lower back.
Employee was shoveling and when EE turned, EE twisted knee.
Tracking system was down due to a grounded reader.
Employee was crossing a crossover at 3A tailpiece. Employee was dusting around 3A drive backing off of cat walk and fell. Employee was complaining about stomach, right elbow, right leg, low back, and tailbone.
Employee was getting into the mantrip and twisted knee.
Employee was letting pressure off fire valve and when employee shut it off the hard line blew apart, knocking employee's feet out from under employee causing employee to land on employee's left elbow and shoulder.
Employee was putting primary scrapper on 1D drive. EE was in a tight spot in between the back board and nose roller when ee strained the right side of low back.
Employee was shoveling on 3A belt when they felt pain in the lower back, right side.
Hoist car was down due to faulty batteries.
While loading timber into scoop bucket, the timber slipped on employee's hand, cutting their left small finger.
Employee was pushing lolo back and when they turned around they felt pain in the right lower leg.
Employee was strapping a kettle bottom when employee pulled a muscle.
Employee was scattering structure for belt setup. Top was low and employee was bent over carrying structure. Employee then put structure in place during belt setup. Employee started missing 04/02/18.
Hoist car was down due to mechanical failure.
Employee was walking up 1A belt and hit their head on the head of rope bolt. This caused their head to snap back and resulted in pain in the neck and pain and numbness in the left arm and hand. Employee started missing 08-15-17.
Employee was cutting tape off of wires in head drive junction box when knife slipped across left wrist.
Rock fall 4E belt line from break 11-14. Fall measured approximately 20' wide x 200' long x up to 6' high.
Employee was pulling an old belt pin out of a splice when a piece of the pin or the vise-grips hit employee in the eye. Employee was wearing PPE.
Employee was working at the back MBC when foot was stuck in the mud. As employee turned to walk away felt something pop in right knee. Employee started missing 07/24/17.
Employee was bolting in 3 heading when glove got hung up on the bolt and twisted arm around the bolt.
Employee was getting belt pins off the dolly when a rock fell and struck them on the back of the head and knocked their face against the lolo dolly.
Employee was going through doors on track when EE hit head on beam at overcast in between doors at 1C belt.
Slope car was down on magnetic brake.
Employee was riding in a scoop going to the section due to the super steer being down. The scoop hit a hole in the roadway causing employee's back to be jerked.

Employee was bolting in the #3 left crosscut when a piece of rock fell off the canopy of the bolter, striking EE on the back of EE's leg.
Employee was lifting a drill pot out of a mantrip to load onto another mantrip, with assistance from another employee. When the assisting employee picked up one side of the drill pot shifting all the weight to the injured employees side and EE stated that felt a tearing and ripping sensation in left arm, shoulder, neck and back.
Employee was at the bottom of slope uncoupling the supply cars from the slope car. When he pulled the lever to uncouple the cars he felt a sharp pain in his lower/mid back and he had numbness in his left leg.
Employee stated that he was hit by a rock. An initial report was not filled out.
Employee was pulling on belt in the tandem roller when his feet slipped out from under him.
Employee put a regulator hole in brattice and when he turned around a piece of draw rock fell and struck him on his hard hat.
Employee was tying a pump onto the car with ratchet binder when he slipped and fell off the side of the flat car, scraping his knee and shin bone. Injury required stitches.
Employee was helping to get a 60" top roller out of beltline, lifting it over their heads. The roller got stuck so they pulled it with a slate bar and when it came free it struck him on the knee. The roller was too close to the top to use a chain ratchet.
Employee was shoveling up a coal spill when he felt pain in his groin area.
Employee was throwing crib block under a low boy to unload a mobile bridge carrier and hit tongue on the supply motor with his left hand and ring finger.
Employee moved a blue tub to get a light charger for an employee when he felt pain in his back.
Employee was bending and installing roof bolts in #5 heading, no problem, but when he left bolter to go to return bolter in #2 heading he felt pain in his groin and had trouble walking from place to place.

Employee was straightening a bent bolt, put it in the head of the bolt machine and accidentally hit the rotation. The bolt had a burr that caught his glove and wrapped his hand around.

Employee hit his left thumb with a hammer while trying to hit a cap wedge to install a timber. Injury required stitches.

Employee walked into a bent roof bolt plate resulting in a laceration to his left shoulder. Injury required stitches.
Tracking system was down due to an optic fiber line being cut.
Employee started to get out of supply motor and put his right elbow on top of canopy and his left hand in handle and pulled himself up and felt his right shoulder pop. Employee later required surgery and started missing 03-14-14.
Employee was changing out belt, he was climbing up on structure when he twisted his right knee. Employee started missing 06-23-14 due to knee surgery.
Employee was attempting to get out of his car when he slipped and struck his ear on the corner of his car door. Employee later required surgical debridement and 6 stitches on 11-15-13.
Rock fall was found at break 20 adjacent to #2 belt intake in the #5 entry. Fall measured approximately 20' wide x 20' long x 8'-10' thick.
Employee was working in 3R crosscut approximately 45'-50' from miner which was cutting top in #3 entry.
Employee stated that he knelt down to start bolting when he felt his right knee to start burning. Employee later required surgery on 12-05-13.
Hoist was down due to changing out the brake motor.
Employee was adjusting brake on supply motor when he pulled his hand back he hit a sharp piece of metal. Injury required two stitches.
Slope hoist was down due to wire came off stop switch.

Rock fall was discovered on 4A belt Break 9 at #4 Entry. Fall measure approximately 30' wide x 30' long x 7.5-8' thick.
Employee was walking bent over when he ran into a strap which knocked his hard hat off. The straps edge came down and struck him on the head.
Employee was standing in the #3 at the last open crosscut outby breaker post when a timber got dislodged and struck him on the forearm.
Employee was attempting to slide the track rail over with a slate bar when the rail flipped over and landed on his left foot.
Scoop bumped lo-lo tailpiece catching employees foot.
Employee inadvertently closed his knife on his finger after opening a container of degreaser. Injury required stitches.

A rock fall was found between overcast on 2 West Mains in the #1 entry at mouth of the 2 left submains. Fall measured approximately 20' wide x 30' long x 5' thick.

Loose brake car battery connection set off E-systems on the hoist and brake car. Employee started missing 04-28-11.

Employee was helping miner men to change places when a piece of rock fell and struck him on the arm and hand.
Employee was crawling down the belt on lolo and his foot must have slipped off the side. They were in the process of setting bits in the miner and when they backed the miner up to roll the heads it hit the Mobile Bridge Carrier (MBC) causing it to twist and rubbed employees foot.
Employee was shoveling tailpiece when his shovel hit belt and bounced back and hit him in the mouth.
Employee was struck by a moving object.
Employee laid a bow saw down after using it and when he picked it back up, he grabbed it the wrong way and cut his hand.
Employee tripped over a rock and fell striking his shoulder on the mine floor. Employee started missing 03-14-11.
Employee was installing a draw head on his RRD motor before coupling to the rock duster. A second supply motor was coming up the track and the operator did not notice that the switch was thrown toward the side track where the employee was working. The second motor hit the RRD motor and knocked it into employee.
Rock fall occurred in break 4 off 2 left off 1 West Mains in the #4 entry. Fall measured approximately 20' wide x 60' long x 7' thick.
Rock fall occurred on 1 West Mains at break 99 #5 entry. Fall measured approximately 40' wide x 60' long x 6' high.
Employee dropped rub rail off miner onto his left foot.
Rock fall occurred on 1st left off 1 West Mains intake #4 entry. Fall measured approximately 20' wide x 20' long x 6' high.
Employee was walking on a roadway when he bumped into the end of an installed torque tension bolt.
Employee and another were lifting a motor with a bar and later complained of back pain.
