Miner was turning drive shaft on mobile bridge carrier when hand slipped off pipe wrench he was using to turn shaft causing the pipe wrench to strike him in the jaw.
Miner was lining up two pieces of metal one piece of metal slipped mashing miners left index finger between the two pieces of metal.
