Opening the shop garage door and another employee ran over to help him lift it and employee said it took the weight of the door from him by surprise, causing him to hurt his lower back. Doctor took him off work for his back on 3/20/2015.

Employee was changing offside rear tire on scoop. He had pulled the tire loose on top and stuck the bar at the bottom of the tire and pulled up to loosen the bottom of the tire when he felt a pain in his lower back.

3rd shift was belting back and the belt ends were spliced. The splice hit on top of top roller. Injured employee removed top structure and placed on right rib and left foot. Sharp edge on belt structure cut hole in boot resulting in a puncture wound to the top of the left foot, just above his big toe. Open fracture to bone on left foot just above big toe.

Employee was standing on the rear wheel wall of a Mac 3D assisting a mechanic lineup motor bolts. As he exited from this position his right knee twisted causing him to fall onto the shop floor, striking his right elbow and head.

Employee was using 3/4 drive ratchet with 18" of extension to loosen the bolts of a shuttle car trunnion. As employee used his weight for leverage the bolt loosened and he fell forward and struck a fender resulting in a broken tooth.

Employee was carrying tools from his ride over to a broke down man trip when he struck a roof bolt with his head resulting in pain in his neck. This area of the mine is about 44" to 48" high. Employee continued to work up until a doctor took him off work on 2/5/14.

On July 17, 2013, ee filed a hearing loss claim with the Kentucky labor Cabinet, Department of Workers' Claim. As per the report of a Specialist/Audiologist, ee has 46% hearing loss in his right ear and 46% hearing loss in his left ear. There are no accidents associated with this claim.

A small roof fall was discovered during the weekly exam located approx 1100' outby the working section. The roof fall is in the #1 entry off the mains going into a panel that has been pillared. The roof fall appears to be contained to that intersection. There were no injuries associated with this fall. This fall measures approx 20' wide x 20' long x 10' thick.

Employee was hanging miner cable and waterline for the scoop to go by and was moving a pump which resulted in a burning pain down his leg. He did not seek medical attention at that time. On 6/2/2013, he complained to experience pain and numbness in his leg and requested to see a doctor. He started missing work at that time.

An unplanned roof fall occurred two (2) cross-cuts outby the abandoned 4 left panel off 3 west mains in the #3 return entry at survey station #3353. The roof fall is approx 20ft long x 20ft wide x 8ft-10ft thick. Ventilation has not been affected. roof fall area will be dangered off and blocked off to prevent access. There were no injuries associated with the roof fall.

EE was attempting to weld a nut onto a bolt to make a puller. He had just started to weld when he felt the top of his right foot burning. This resulted in a burn to the top of the EE's right foot.

EE was getting out of his coal truck to clean his windows after crossing the truck wash. He said his foot slipped off the bottom truck step landing hard on the ground. He said he fell backwards landing on his right hip. He had severe pain in that hip. He was taken to Harlan ARH emergency room, x-rayed and diagnosed with a large contusion, no broken bones.

On Tuesday 1/24/12, a roof fall occurred on the 003 pillar section. The fall occurred in the #4 entry while cutting the last run from the left pillar block. The fall caught the head of the miner. A call was made to MSHA and also reported to the 800 number. MSHA inspector came to the mine and was present when the miner was pulled out. No injuries occurred during this incident.

EE was roof bolting in the #4 entry on the MMU. As the EE was drilling the hole, the coupling that connects the drill steel together broke causing the 2 sect.of steel to come apart. The upper piece of drill steel fell out of the hole which bounced off the drill canopy striking the EE in the lower left ribs, caused a small burn on upper side of the left bicep,abrasion on left ribs.

The employee bent over to pick up a metal H beam and moved the beam over to the rib. When the EE lifted the beam he twisted his lower back. EE cont. to work and did not rec. any medical treatment until 9/29/10. The EE ret. to his reg. duties on 9/30/10. The EE cont. to hurt and went back to the Dr. on 10/6/10. The Dr. placed EE on restricted duty on 10/6/10.

Roof fall occurred in the No. 5 entry (2) crosscuts inby survey station 2418 on the 3 left panel off the 1-East submains. Roof fall reported to MSHA by phone on 1/08/2010.

On 003 section, a roof fall occurred in the #3 entry - pillar section. All wings on block were mined, ready for butt-cuts. When MRS's were lowered, fall occurred on top and in front of MRS's. Fall approx 35' x 18' W x 8-10' H. Trapped both MRS's. No injuries. Fall reported to MSHA with "J" order issued. Modified to "K" order at 23:45 hours. Fall was between spad 2503 & spad 2509.

EE was drilling for a roof bolt and tried to remove the steel from the hole when the steel got hung up. EE was pulling on drill steel when the steel broke free and the EE hit his right elbow on the drill pot.

Foreman was traveling airways & found roof fall on 11/13/08. Roof fall was in the No 16 entry outby survey spad 1948. The roof fall is approximately 30' long x 18' wide x 3'-4' thick. This area has been dangered off & additional roof support has been installed on the inby & outby end of the roof fall to make area safe. (Roof fall reported to MSHA by phone 11/14/08.)

EE alleges he was loading roof bolts from scoop bucket to roof bolter when his right shoulder began to hurt. He was angry and began slinging roof bolts out of the scoop bucket. He grabbed several at one time & jerked on them causing the alleges injury. We are reporting this accident to meet the 10 days requirement but after our investigation is comeplete.
