Employee was getting out of their truck to use the restroom and missed the first flex step and fell to the ground landing on their butt and lower back. Accident currently under investigation.

Cut right index finger at second knuckle with pocket knife while cutting a zip tie off of hydraulic hose harness to make adjustments to the hose harness.

Employee states EE fell while picking up garbage at ground level.
The employee injured left thumb while helping to secure a V-belt guard. As employee was holding pressure using a wrench another employee used an impact wrench to tighten the bolt of the guard. Not being aware of surroundings employee placed left thumb between the guard bracket and structure framework.

Employee was inspecting an inflated OTR Tire when it released air hurling EE to the ground.

Employee was found unresponsive in loader. Employee had no pulse & was not breathing. Gave him CPR until medical personnel arrived. Employee was air lifted to hospital where he was pronounced died.

Employee was walking toward a conveyor belt to help get it running after the belt had chocked down when he slipped on some ice & fell twisting his left ankle. (There was a severe ice storm 6 days prior to this accident.)
