Had a hoist hooked up to a speed reducer to a scoop. The arm of the hoist broke hitting employee on the arm.
Employee was tamping when he went to foreman stating this his side was hurting bad and was getting weak. He said he was going to have to go home and go to the doctor next day. He left at 1 am.
Was operating roof bolter. A rock 1 foot round and 1.5 inches thick fell hitting his finger and cutting it.
Employee was backing roof bolter out of section three left. He had his left arm out of deck, while holding cable out of way. His left arm got caught between the roof bolter and rib.
Injured was observing work being performed by co-worker on cutting machine. Grinding dust and metal went into injured's left eye.
EE was spinning steel out of top of bolter. His arm accidentally hit the up level on the equipment, pinning his index finger against the roof and the machine.
EE was using a hammer on a piece of metal, rust flew into his right eye.
Employee stated he was attempting to make corrections in the Pemco 1500 KVA section power box. While he was working in the section power box he came in contact with an electrical current, resulting in an electrical burn to his right hand palm and elbow.
An inundation of gas occurred on MMU 001-0 Section. While advancing Panel #16 cutting Gob-I on across from #6 right crosscut at #27 break. East Mains sealed area was cut into at #27 break on panel #16. This was located approx 100' from #6 entry survey station #3325, w/last 25' unsupported. No injuries reported, temp vent.controls constructed & seal built according to plan.
EMPLOYEE FELT PAIN IN LOWER BACK AFTER PICKING UP A PIECE OF BELT STRUCTURE IN SUPPLY YARD. EE INFORMED ME ON 4/7/05 THAT HE STARTED MISSING WORK ON 4/1/05.
AN INTENTIONAL ROOF FALL RODE OUTBY THRU BREAKER POST INTO ACTIVE RETREAT MINING SECTION. THE OUTBY EDGE OF FALL CAUGHT THE FRONT MBC OF A CONTINUOUS HAULAGE UNIT. ALL PERSONNEL WERE PULLED OUTBY BEFORE FALL OCCURRED. NO INJURIES RESULTED FROM THE FALL. APPROXIMATE SIZE OF FALL IS 100'L X 10'-20'W X 12'H. LOCATION IS ON S. MAINS #11 X-CUT #3 ENTRY.
A fall of supported roof has occurred in by spad # 2589, 001 section. Number 15 crosscut, #2 entry. No injuries resulted from this fall. The fall measured 45' l by 15-20' w by 10' h.
A fall of supported roof occurred in no. 4 entry between crosscut #7 and 8 on south mains, parallel entries on 001 section. The fall measured 18' wide by 70' l by app 6' high. No injuries resulted from the fall.
A fall of supported roof occurred on No. 5 belt (south mains) No. 4 entry (belt enrtry) at No. 14 crosscut. The fall measured approximately 35' long x 20' wide x 12' high. No injuries resulted from this fall.
A fall of supported roof was found during weekly exam in an abandoned area of the mine. The fall was located on No. 8 panel #3 crosscut off south mains. The fall measured 20'W x 300'L x unknown height.
A fall of supported roof was found during weekly exam in an abandoned area of the mine. The fall was located at #66 break on south mains #5 & #6 entry. The fall measured 20' x 90' x unknown height.
A fall of supported roof has occurred out by the active section on #6 belt entry in by spad station #1493. The fall measured approx 20' by 60' by 10' high. No injuries resulted, however the conveyer belt was damaged.
A fall of supported roof was found by examiner on above date and time. the fall was located in an intersection at #4 break, #4 entry, on east mains #3 belt. the fall measured 20'x20'x approx 18' high. no injuries occurred, no equipment damaged, ventilation was not affected.
EMPLOYEE STATED THAT HE JARRED HIS BACK WHILE OPERATING A SCOOP. FELT PAIN IN LOWER BACK. DID NOT START MISSING WORK UNTIL 11/23/04.
A FALL OF SUPPORTED ROOF HAS OCCURRED IN AN ABANDONED AREA OF THE MINE. THE FALL IS LOCATED ON THE #7 PANEL #1 ENTRY BLEEDER RETURN #11 CROSS-CUT. THE FALL WAS FOUND BY MINE EXAMINER AND MSHA INSPECTOR. THE FALL WAS APPROXIMATELY 20'X 20'X 18'HIGH. NO INJURIES OR EQUIPMENT DAMAGE RESULTED.
EE notified us that he was going to the doctor for an incident that occurred on 11-7-04, ee stated he felt pain in his lower back while working on miner ripper head. EE has had ongoing back pain. Did not start missing work until 11-29-04.
A FALL OF SUPPORTED ROOF OCCURRED OUTBY THE ACTIVE SECTION ON THE NO.7 PANEL CROSSCUT NO.6, STARTING IN NO.5 ENTRY EXTENDING TO NO.7 THE AREA STARTED WORKING ON 10/13/04 AND WAS CRIBBED OFF. NO INJURIES RESULTED FROM FALL, THE FALL MEASURED 20'W X100'L X 18'HIGH
A FALL OF SUPPORTED ROOF OCCURRED AT SPAD STATION NO. 447 IN THE #3 ENTRY ON #4 BELT ON SOUTH MAINS. NO INJURIES RESULTED FROM THIS FALL, NO EQUIPMENT WAS DAMAGED, VENTILATION WAS NOT IMPAIRED. THE FALL MEASURED APPROX. 18' X 20' X 7' HIGH.
A fall of supported roof occurred on the 001 section, 15th crosscut, no. 6 panel, in entries no. 1,2,3,5,6,& 7. The fall was appx. 10-18' thick, rib to rib. No injuries occurred and no equipment was damaged.
EMPLOYEE RIDING IN SCOOP WITH CONCRETE BLOCKS. SCOOP RAN INTO MINER TAIL, MOVING BLOCKS OVER ONTO HIM.
EMPLOYEE WAS STRUCK BY A PIECE OF DRAW ROCK WHILE INSTALLING A SCRAPPER AT A HEAD DRIVE. THE ROCK STRUCK HIM ON THE HEAD AND BACK CAUSING A COMPRESSION OF T7 DISC.
A FALL OF SUPPORTED ROOF OCCURRED ON THE 001 SECTION #5 PANEL #2 ENTRY BETWEEN CROSSCUTS #15 & #16. NO INJURIES OR PROPERTY DAMAGE OCCURRED AS A RESULT. THE FALL MEASURED 80' LONG X 18-20' WIDE X 6' HIGH.
A fall of supported top occurred at the no.8 crosscut, intersection of headings 5,6, and 7 on the no.5 panel, 001 section. No injuries resulted from this fall. The fall measured 150' long by 18' wide by 15' high.
Employee was drilling a roof bolt hole, while doing so, he lost his balance and caught his hand on rotating chuck of drill, resulting in twisting injury to ee's right forearm.

AREA HAD BEEN SUPPORTED WITH 6' FULLY GROUTED RESIN BOLTS WITH SUPPLEMENTAL SUPPORT CONSISTING OF 10' CABLE BOLTS. NO INJURIES, VENTILATION WAS NOT INTERUPTED, BELT WAS CAUGHT. FALL WON'T BE CLEANE
UPON MEDICAL EXAMINATION EMPLOYEE WAS DIAGNOSED THAT HE HAD SUFFIEIENT DEGREE OF PNEUMOCONIOSIS (BLACK LUNG).

SIZE OF FALL WAS ESTIMATED TO BE 12' H X 30' L X 18'-25' W AREA HAD BEEN SUPPORTED WITH 4' FULLY G ROUTED RESIN BOLTS SUPPLEMENTAL SUPPORT CONSISTING OF 10' CABLE BOLTS. NO INJURIES, NO EQUIPMENT CAUGHT, VENTILATION WAS NOT INTERRUPTED. FALL WILL NOT BE CLEANED UP.

AREA HAD BEEN SUPPORTED WITH 6'TENSION REBAR BOLTS WITH SUPPLEMENTAL SUP PORT CONSISTING OF 10' ROPE BOLTS. NO INJURIES, VENTILATION WAS NOT INTERRUPTED, BELT LINE WAS CAUGHT. FALL WILL BE CLEANED UP.

AREA HAD BEEN SUPPORTED WITH 6' TENSION REBAR BOLTS WITH SUP PLEMENTAL SUPPORT CONSISTING OF 10' ROPE BOLTS. NO INJURIES, VENTILATION WAS NOT INTERRUPTED, NO EQUIPMENT CAUGHT. FALL WAS CLEANED UP.
AN UNINTENTIONAL ROOF FALL OCCURRED ON THE #5 BELT (#4 ENTRY) SIZE OF FALL WAS ESTIMATED TO BE 12' HIGH BY 17-19' WIDE BY 300-400' LONG, AREA HAD BEEN SUPPORTED WITH 4' RESIN BOLTS WITH SUPP LEMENTAL SUPPORT CONSISTING OF 6-9' CABLE BOLTS IN INTERSECTIONS. NO INJURIES OCCURRED, VENTILATION WAS NOT INTERRUPTED, FALL WILL NOT BE CLEANED UP.
EE STATED WHILE OPERATING MOBILE BRIDGE HIS LEFT KNEE STARTED HURTING. EE SUSTAINED A SPRAIN/STRAIN TO HIS LEFT KNEE.
EE STATED WHILE LIFTING COUPLER ON RAIL CAR TO HOOK UP TO MOTOR HE FELT PAIN IN HIS LOWER BACK. EE SUSTAINED A LOWER BACK STRAIN.
KNEELING TO WELD WHEN MOVING TO STANDING POSITION RIGHT KNEE POPPED.
