When entering a lube truck cab to disengage the PTO pump. As they used the grab/handrail to pull themselves into the truck, their shoulder popped. Employee continued into the truck, but, when they got out of the truck shoulder started hurting.

working under the drill cab, machine was partially raised too high to kneel not high enough to stand - ground was muddy when employee came out from under machine they raised up too fast striking edge of cab with neck and shoulder.

Exiting truck bed, foot slipped causing fall. Landed on right side. Employee was taken to doctor and released. No work missed until 11-1-17. 10-30-17 employee visits doctor. Doctor states there may be inflammation in right arm & prescribes medication. Employee returns to doctor the evening of 10-31-17. Doctor admits employee for IV fluids & ultrasound. 11-3-17 Employee is released.

EE was stepping off the left side of a Caterpillar Dozer D9N as he stepped onto the push arm step twisting his ankle and fell to the ground, injuring his mid to lower back.

Coalgood Energy Co · Inhalation of radiations, caustics, toxic and noxious substances

EE was pushing in a shot in an open pit when the motor of his D11R Caterpillar Dozer caught fire. EE disengaged the fire suppression system and the fumes began to fill up the cab before EE could exit the dozer. EE suffered an inhalation injury with internal chemical burns and was released from the hospital 03/18/11.

The claimant stepped out of the excavator to adjust his driver side mirror, when he pushed on the mirror it broke. The mirror busted in the claimant's hand, cutting his right hand.

EE was stepping off the operator side of his dozer. As he stepped on the ground, his left foot landed on a rock and he crilled his left ankle. He was taken to the ER at Harlan ARH, where he was diagnosed with a sprained ankle and possible hair-line fracture. He returned to work the same day.

ABDOMEN, HE GOT SICK TO HIS STOMACH & PULLED OFF THE ROAD. EMPLOYEE STATED HE FELT THAT HE COULD WORK. AROUND 6:30AM HE LIFTED A PRIMER BOX & THE PAIN IN HIS LOWER RT. ABDOMEN WAS SO SEVERE THAT HE WAS TAKEN TO THE HOSPITAL.

EMPLOYEE PLACED A SHEET OF METAL ON A WOOD BLOCK TO MAKE A RAMP FOR A 330M 100 TON ROCK TRUCK TO PULL ON. AS THE TRUCK PULLED ON THE WOOD BLOCK SLIPPED OUT STRIKING HIS RIGHT UPPER LEG CAUSING TISSUE DAMAGE.

HE DID NOT MENTION THE PAIN TO HIS SUPERV ISOR UNTIL HOURS LATER AT WHICH TIME HE STATED HE EXPERIENCED PAIN AT THE SITE OF A PRIOR INJURY IN HIS LOWER BACK.
