Measuring the height to set a timber, when the tape line reeled back up and cut his index finger on his right hand.

Foreman was making his weekly examination on the primary escapeway. When he came across a rockfall. The fall was located 15 feet inby the fanhouse in the #4 entry, the fall was 10 feet in length 16 feet wide and 5 feet to 6 1/2 feet high.

EE was lifting his tool box onto a "buggy". When he twisted to load the box he experienced pain in his groin,left work due to this pain and since then he has complained that he had pain in his lower back.

A roof fall measuring 40 feet by 27 feet by 5 feet was found in an intersection near survey station No. 6153. There were no injuries and the mines ventilation system was not interuppted.

Employee was working near the roof bolter cable plug on the section power center, when the cable inside the plug became grounded and blew out. EE did not receive an electrical shock, but did get a second degree burn on his hand from the flash.

Employee stated that when he picked up a bucket of oil and then later unloaded his tools his back became aching and stiff. He had a previous back injury and this incident has been under investigation. It has now been determined to be a new injury and is now reportable.

Employee, was removing a metal cover from a front end loader when he received a cut on the palm of his right hand that required 6 stitches to close.

Employee was operating the continuous miner when a piece of draw rock measuring 6 feet by 4 feet by 6 inches fell striking him on his right leg. He was taken to a local hospital where he was diagnosed with a bruise and sprain to his foot. He is working restricted duty.

During a pre-shift examination of the mine a roof fall was found on the # 2 belt system. The roof fall was approximately 20 feet by 18 feet by 6 feet. The fall was cleaned up utilizing a continuous miner and a set of arches were constructed as support.

Employee was lifting pieces of belt structure when he injured his back. He continued to work until 8-11-05 at which time he felt he needed to see a doctor due to the fact that his back was continuing to hurt. He was diagnosed with a pulled muscle and was told to take a couple of days off. He missed two days of work.

During a pre-shift of the mine following a weekend idle period a roof fall was found on the No. 2 belt conveyor. The fall measured approximately 18 feet by 24 feet by 10 feet and was located in an intersection 60 feet inby survey check point # 8074 The area had been bolted with 48 inch resin bolts.

A ROOF FALL WAS FOUND 60 FEET INBY SPAD #2825 DURING THE FOREMAN'S INSPECTION OF TRAVEL WAY. THE FALL WHICH IS LOCATED OUTBY THE WORKING SECTION MEASURED 6' X 18' X 35' AND WAS IN AN AREA THAT HAD PREVIOUSLY BEEN DANGERED OFF.

DURING A WEEKLY INSPECTION OF THE INTAKE AIR COURSE A ROOF FALL WAS FOUND IN THE #7 ENTRY ADJACENT FROM SURVEY POINT 2243 ON THE 001-0 SECTION. THE FALL MEASURED 6' BY 80' BY 19'. VENTILATION WAS INTERRUPTED BUT STOPPINGS WERE MOVED TO OPENTHE PRIMARY ESCAPEWAY.

THE EE WAS HELPING ANOTHER EE HANG A CONT MINER CABLE. AFTERWARDS HE REPORTED TO HIS SUPERVISOR THAT HIS LOWER BACK WAS SOAR AND AN AREA OF IT WAS SWOLLEN. HE FINISHED WORKING THE SHIFT AND HAS NOT RETURNED AND IS UNDER PHYSICIAN'S CARE.

A ROOF FALL OCCURRED IN THE LAST OPEN CROSSCUT IN THE #3 ENTRY. THE FALL WAS 18' X 18' X 6'. THERE WAS ANOTHER FALL FOUND OUTBY THE FACE IN THE NUMBER 1 ENTRY APP 150 FEET. IT MEASURED 18' X 1 8' X 4'.
