Employee was setting roof supports in addition to permanent supports, when a rock fell & struck the victim's left arm, causing him to fall & also strike his head on the rock.
EE was employed by Premier Elkhorn Coal Company from 1/19/02 until 7/19/07. ON 3/31/08, EE was awarded Worker's Compensation due to being diagnosed with Coal Worker's Pneumoconiosis.
A co-worker pitched a bent anchor pin from the tail piece over to the rib. The pin glanced off the top & struck the injured employee's Right index finger while he was handling structure.
Employee was operating a Personnel Carrier when he ran over a rock in the roadway, causing the steering wheel to suddenly turn, striking his right hand. The employee sought medical treatment for this injury at a later late & reported to Premier Elkhorn Coal Co. on 2/6/08, that he had suffered a fracture as a result of the accident that occurred on 1/14/08.
An unintentional roof fall occurred 1700 ft. outby the section in the #6 (Track) Entry. The approximate size of the roof fall was 15 ft. long x 18 ft. wide x 4.5 ft high.
An unintentional roof fall was discovered at 6:15 AM on 12/26/07. The fall was located at Break #31, #4 Entry on SW Main at Spad #174. The approximate measurements were 20' wide x 25' long x 5' high.
The general Mine Foreman discovered the fall at 2:50pm. It was 3,400' from the active section in the return at Spad #3252, in the #2 entry. The roof fall measured approx 20' wide X 25' long X 5' high.
An unintentional roof fall occurred at break #58 in an intersection. The fall measured approximately 25 feet long X 25 feet wide X 4 1/2 to 5 feet high.
Employee was changing thread bar on a shuttle car. The new thread bar to be installed slid off the shelf and struck behind the steel-toe portion of his right foot, resulting in a fracture.
EE bent over to pick up and load glue boxes onto the roof bolt machine. He then experienced pain and a popping sensation in his lower back. The injured worked his shift, then visited his family doctor, who advised him to take off work one day on 8/7/07.
The employee was lifting a glue box when he felt pain/burning and heard popping sounds in his left hand/wrist. The accident was not deemed reportable, however, until the ee began taking off work due to complications from the injury beginning on Sept. 19,2007
Employee was removing the lid from a Fletcher Roof Bolter to replace the hydraulic pump. When he slid the lid off the machine, he experienced pain in his lower back and left leg. The employee finished his shift but later sought medical treatment as a result of this injury.
The EE was loading plates when he turned to throw a bundle onto the roof bolt machine and experienced a sharp pain in his lower back and numbness in his right leg. The EE worked through the following Monday (5/7/07) and began taking off work on Tuesday (5/8/07) as a result of this injury.
An unintentional roof fall occurred in the track entry at Break #66. The fall measured approx 41/2'-5' high, 20' long & 20' wide.
An unintentional roof fall occurred at Break #61/Spad #1622. The fall measured approx 7' high X 30' wide X 40' long. It was discovered by the 3rd shift mine foreman, who was conducting a pre-shift examination.
An unintentional roof fall occurred in the #6 Entry at Break #39 intersection. The fall measured approx 5'-7' high X 30' wide X 45" long.
The accident victim and witness were in the process of building cribs, when the witness threw down a crib block while the victim was attempting to pick up one. The block struck the victim's right hand (index & middle fingers) resulting in a laceration that required stitches.
An unintentional roof fall occurred in the intersection at break #85. The fall measured approximately 8 - 9 ft. high x 20 ft. wide x 30 ft. long.
An unintentional roof fall occurred in the #3 intersection at the break inby Spad #2793. The fall measured approx 8'-10' high X 20' wide X 40' long. The fall also covered approx 2/3 of a Fletcher roof bolt machine, as described below.

The injured employee was drilling a hole to install roof support when a piece of rock fell from the mine roof & struck the EE on the rt. wrist, resulting in a fracture.
The injured EE was changing the discharge roller on the #2 head drive. When he removed a bearing off the roller, it fell onto the middle finger of his right hand, resulting in a fracture.
AN UNINTENTIONAL ROOF FALL OCCURRED AT BREAK #81, #8 HEADING OF THE RIGHT RETURN. THE FALL MEASURED APPROX. 30' LONG X 19' WIDE X 5' HIGH.
The injured employee had been driving a pin in the ripper chain at the miner when he stepped approximately 6 - 8 feet away to clean his glasses. Another employee was hammering nearby, causing a piece of metal to strike and puncture the injured employee's right eye.
An unintentional roof fall occurred in the face area of the E3-5B mine at approx. 9:30 pm. The fall measured approx. 30'long x 18'wide x 6'high & was slick-sided rock.
An unintentional roof fall was discovered one break inby spad #2017. The fall measured approximately 20' wide by 20' long by 4'-5' high.
THE FALL WAS DISCOVERED BY GENERAL MINE FOREMAN DURING A WEEKLY EXAM. THIS AREA HAS OTHER FALLS NEARBY. THIS PARTICULAR FALL MEASURED APPROXIMATELY 20' WIDE X 40' LONG X 4'-5' HIGH.
The injured employee stood up, causing his right ear to come into contact with a hanging strap. A laceration requiring stitches resulted from this accident.
AN UNINTENTIONAL ROOF FALL OCCURRED IN THE LEFT ENTRIES OF THE INTAKE AT A BREAK INBY SPAD @2036. THE FALL HAPPENED IN AN ABANDONED AREA. IT MEASURED APPROXIMATELY 100' LONG X 18' WIDE X 4' TO 6' HIGH.

An unintentional roof fall occurred in the No. 6 track entry one cross-cut inby survey station 1917 measuring approx. 20' w x 20' l x 6' h. The fall appeared to be a horse-back, slick-sided rock.
A small piece of draw rock with a sharp edge fell, striking the back of the employee's left hand. The laceration required stitches.
The injured employee was drilling a hole for a roof bolt when a piece of rock fell on the back of his right arm, resulting in a laceration that required stitches.
The employee was starting to drill when a piece of rock fell from the roof, striking the small finger of his left hand, and cutting the tip of it off completely. Per phone call, the original injury did not cut the bone, but the Dr had to amputate just below the fingernail to graft skin around severed part of finger.
An unintentional roof fall occurred in the crosscut between the No. 8 and 9 entries at Survey Station 1995, measuring approximately 18' wide x 40' long x 5 to 8' high. The slate roof that fell was a slick sided piece of roof.

AN UNINTENTIONAL ROOF FALL OCCURRED BETWEEN TWO INTERSECTIONS OF THE NO. 6 ENTRY, 40' OUTBY SURVEY STATION 2022. IT MEASURED APPROX. 18'LONG X 18'WIDE X 6-7'IN HEIGHT.
AN UNPLANNED ROOF FALL OCCURRED IN THE BELT LINE ENTRY OVER AND OUTBY 1 CROSSCUT FROM SURVEY STATION 30. THE FALL MEASURED 40' X 20' X 6'. THE AREA WAS BOLTED WITH 4' FULLY-GROUTED BOLTS AND ROOF MATS WERE USED
Thr roof fall was approx 4-5 feet high, by 20 ft wide and 20 ft long. It was discovered one crosscut out by spad #193 in the #7 entry.
THE EMPLOYEE WAS POSITIONED OUTBY THE 2ND ROW OF BOLTS & WAS BOLTING THE TOP IN THE NO.3 LEFT CROSS-CUT. WHEN THE ATRS WAS LOWERED, A PIECE OF ROOF ROCK FELL & STRUCK HIS FOOT.
THE SCOOP BUCKET THAT THE EE WAS RIDING IN RAN OVER A ROOF BOLT ON THE MINE FLOOR. THE ROOF BOLT THEN FLIPPED UP, AND STRUCK THE EE'S HEAD AND NECK.
A ROCK FALL WAS DISCOVERED IN THE #5 ENTRY ONE CROSSCUT INBY SPAD #1550. THERE WERE NO INJURIES AND THE TRAVELWAYS AND AIRWAYS WERE NOT AFFECTED. THE FALL MEASURED APPROXIMATELY 1 FT TO 7 FT HIGH, 18 FT WIDE AND 50 FT LONG. IT APPEARED TO BE A SLICK SIDED ROCK.
A ROCK FALL WAS DISCOVERED AT THE NO.3 ENTRY IN BY SPAD #1455. THERE WERE NO INJURIES & THE TRAVELWAY & AIRWAYS WERE NOT AFFECTED. THE FALL MEASURED APPROX. 5'HIGH X 18'WIDE X 30'LONG.
EMPLOYEE WAS INSTALLING A BOLT WHEN HIS THUMB ON HIS RT.HAND SLIPPED UNDER THE BOLTHEAD AND WAS PINCHED BETWEEN THE DRILL POT & BOLTHEAD CAUSING A FRACTURE TO HIS RT.THUMB & REQUIRING STITCHES.
EE WAS PUSHING A BENT 4' ROOF BOLT UP IN A DRILLED HOLE AND ACCIDENTALLY HIT THE DRILL ROTATION CAUSING THE ROOF BOLT TO ROTATE AROUND AND STRIKE EE ON BACK OF LEFT HAND CAUSING A FRACTURE.
WHILE EE WAS INSTALLING A 4'METAL STRAP & WAS SLIDING HAND DOWN STRAP TO LINE UP WITH THE NEXT HOLE TO BE DRILLED, HE ENCOUNTERED A SHARP EDGE ON THE STRAP THAT CAUSED A LACERATION TO THE RT. FINGER REQUIRING 6 STITCHES.
EE WAS LIFTING A MAT CHAIN RIPPER PAD ONTO THE MAN BUS AND EXPERIENCED PAIN IN THE LEFT SIDE OF HIS LOWER BACK.
Employee said he was moving miner cable when he turned and twisted his left knee.
DURING A WEEKLY EXAM, THE ROOF FALLS WERE DISCOVERED IN THE RETURN ENTRIES AT SPAD 622 AND 646. THERE WERE NO INJURIES AND THE TRAVELWAY AND AIRWAYS WERE NOT AFFECTED. THE FALL WAS APPROXIMATE LY 60 FEET LONG, 18 FEET WIDE, AND 4 FEET HIGH IN THE #2 HEADING. AND APPROXIMATELY 25 FEET LONG, 18 FEET WIDE, AND 4 FEET HIGH IN THE #3 HEADING.
EE WENT TO FIND A SCOOP OPERATOR TO MOVE A ROCK, STARTED BACK TO DINNER & CAUGHT A STRAP HANGING DOWN & CUT HIS BACK REQUIRING STITCHES (NO LOST TIME).
EE WAS LOADING BELT STRUCTURE WHEN DRAW ROCK FELL, STRIKING EE ON LT. SHOULDER CAUSING BRUISE ON SHOULDER.
EMPLOYEE WAS HANGING MINER CABLE, FELT PAIN IN LOWER BACK. EMPLOYEE'S LAST DAY WORKED WAS 8-16-03.
WHILE INSTALLING ROOF BOLTS, DRAW ROCK FELL CATCHING THE OPERATOR'S RIGHT MIDDLE FINGER AGAINST THE DRILL POT ON THE MACHINE (REQUIRED STITCHES).
AN UNINTENTIONAL ROOF FALL OCCURRED IN THE NO. 1 ENTRY 6' OUTBY FACE MEASURING APPROXIMATELY 5' HIGH X 30' LONG X 18' WIDE.
IN JANUARY EE WAS TAKING A FILL PLUG OUT OF MINER WHEN RATCHET SLIPPED HURTING HIS LEFT HAND AND FINGER, AND HE STATES NOW THAT IT IS DUE TO THAT INJURY THAT HE IS HAVING SURGERY ON 8-21-03 FO R CARPAL TUNNEL.
AN UNINTENTIONAL ROOF FALL OCCURRED ON THE TRACK ENTRY 30' INBY SPAD #44 MEASURING APPROX. 10' HIGH BY 18' WIDE BY 40' LONG. THE AFFECTED AREAS WAS SUPPORTED WITH 4' RESIN GROUTED BOLTS AND 4' STRAPS ROOF MATS.
AS THE EE BEGAN TO DRILL, A SMALL PIECE OF DRAWROCK FELL STRIKING HIM ON THE RIGHT HAND. HE WAS TAKEN TO THE HOSPITAL AND RECEIVED STITCHES TO HIS RIGHT HAND,.
EE WAS CUTTING TAPE OFF SHUTTLE CAR CABLE USING A UTILITY KNIFE WHEN KNIFE CUT THROUGH TAPE, IT CUT LEFT INDEX FINGER THAT REQUIRED 4 STITCHES.NOTE:EE WENT TO HOSP. HAD 4 STITCHES PUT IN THEN RETURNED TO WORK.
AN UNINTENTIONAL ROOF FALL OCCURRED IN THE NO. 5ENTRY 40' IN BY SURVEY STATION 470 MEASURING APPROX 3 1/2 - 4' HIGH BY 12' WIDE BY 25' LONG. THE AFFECTED AREA WAS SUPPORTED WITH 4' RESIN GROUTED BOLTS AND 4' STRAPS (ROOF MATS). IT APPEARED TO BE A HORSE WITH A SLICK SIDED ROCK.
AN UNINTENTIONAL ROOF FALL OCCURRED ON THE SURFACE HIGHWALL AT THE NO. 1 PORTAL MEASURING APPROX. 10'WX6'H CAUSING DAMAGE TO THE FAN HOUSE CANOPY.
EE WAS EXHANGING BOLTS WHEN A PIECE OF DRAW ROCK FELL OUT FROM BETWEEN ROOF BOLTS AND STRUCK EE ON TOP OF HIS HARD HAT.
OPERATOR WAS BOARDING BUGGY, HIS FOOT SLIPPED ON FOOT PADDLE AND TWISTED HIS LEFT KNEE.
EE WAS OPERATING SHUTTLE CAR AND WAS RETURNING TO THE MINER. THE SHUTTLE CAR BRAKES FAILED AND EE STRUCK MINER WITH SHUTTLE CAR-EE EXPERIENCED PAIN IN BACK.
EE WAS BENT DOWN ON RIGHT KNEE WATCHING MINER CABLE WHEN A PIECE OF DRAW ROCK FELL OUT FROM BETWEEN ROOF BOLT AND RIB STRIKING THE TOP OF HIS LEFT FOOT FRACTURING THE SMALL TOE.
A SMALL AMOUNT OF LOOSE ROCK FELL AT THE PORTAL OF THE BELT ENTRY STOPPING PRODUCTION UNTIL THE ROCK WAS REMOVED AND THE BELT LINE CLEARED AND REPAIRED AND CANOPY RE-SET.
EE WAS BOLTING TOP AND A PIECE OF DRAW ROCK FELL STRICKING EE ON HIS RIGHT FOOT CAUSING A BRUISE.
