Employee was operating a Personnel Carrier when he ran over a rock in the roadway, causing the steering wheel to suddenly turn, striking his right hand. The employee sought medical treatment for this injury at a later late & reported to Premier Elkhorn Coal Co. on 2/6/08, that he had suffered a fracture as a result of the accident that occurred on 1/14/08.

EE bent over to pick up and load glue boxes onto the roof bolt machine. He then experienced pain and a popping sensation in his lower back. The injured worked his shift, then visited his family doctor, who advised him to take off work one day on 8/7/07.

Employee was removing the lid from a Fletcher Roof Bolter to replace the hydraulic pump. When he slid the lid off the machine, he experienced pain in his lower back and left leg. The employee finished his shift but later sought medical treatment as a result of this injury.

The EE was loading plates when he turned to throw a bundle onto the roof bolt machine and experienced a sharp pain in his lower back and numbness in his right leg. The EE worked through the following Monday (5/7/07) and began taking off work on Tuesday (5/8/07) as a result of this injury.

The accident victim and witness were in the process of building cribs, when the witness threw down a crib block while the victim was attempting to pick up one. The block struck the victim's right hand (index & middle fingers) resulting in a laceration that required stitches.

The employee was starting to drill when a piece of rock fell from the roof, striking the small finger of his left hand, and cutting the tip of it off completely. Per phone call, the original injury did not cut the bone, but the Dr had to amputate just below the fingernail to graft skin around severed part of finger.

THE FALL WAS APPROXIMATE LY 60 FEET LONG, 18 FEET WIDE, AND 4 FEET HIGH IN THE #2 HEADING. AND APPROXIMATELY 25 FEET LONG, 18 FEET WIDE, AND 4 FEET HIGH IN THE #3 HEADING.
