EMPLOYEE WAS HOOKING CHAIN THROUGH BELT WHEN HE TWISTED HIS BODY CAUSING PAIN IN LOWR RIGHT BACK. EMPLOYEE FIRST SHIFT MISSED DUE TO INCIDENT WAS 4-23-00. EMPLOYEE WAS DIAGNOSED AS HAVING A HERNIATED DISC.
