#1 BELT LINE BROKE. THEY WERE USING SHIEVE WHEEL AND STEEL ROPE PUTTING BELT BACK TOGETHER WHEN PIN IN THE SHIEVE WHEEL BROKE LETTING THE ROPE FLY BACK AND HIT HIM IN THE CHEST.
