FMC employees were attempting to seal the hoist shaft at the Bremen Portal at Ken American Paradise Mine No. 9. Steel beams were welded in place metal decking was secured covering the shaft. Methane accumulated in the shaft and caused a series of two explosions 4-5 seconds apart. The second explosion killed FMC employee. No other fatalities or injuries occurred.
Employee was in the process of examining the beltline and twisted right knee.
Roof fall occurred at xcut 22 entry 9 in the Main East return air course. The roof fall measured 35'L x 20'W x 8'T. The roof fall will not be cleaned up. The roof was supported with 6' fully grouted roof bolts and 8' megabolts. Date of development was 9/2017.
Employee was on man cage while exiting the mine. The employee bent over to pick up lunch box and was struck on the right shoulder with a piece of ice.
Roof fall occurred at x-cut 20 between entries 2 & 3 of the Main East. The roof fall measured 55"L X 18'W X 5"T. The roof fall will be cleaned up utilizing MSHA approved roof fall clean-up plan. The roof was supported with 6' fully grouted roof bolts and 8' mega roof bolts. Date of development was 9/2017.
Employee was in the underground shop area picking up rockdust bags to throw in dumpster when EE struck right ear on a sprinkler head on the fire suppression system. This resulted in a laceration to the right ear causing sutures.
Employee was riding in covered mantrip while wearing safety glasses and felt something in eye.
Roof fall occurred at x-cut 54 between 10 & 11 entries of panel 5. The roof fall measured 50'L x 18'W x 8'T. The roof fall will not be cleaned up. The roof was supported with 5' fully grouted roof bolts. The date of development was 11/2018.
Roof fall occurred in the 9 seam in the 1st Southeast sub main entry 2 between footage markers 5220 and 5140. The roof fall measured 25'L x 20'W x 10'T. The roof fall will not be cleaned up. The roof was supported with 6' fully grouted roof bolts. The date of development was 12/2018.
Employee was operating diesel mantrip on 3B supply road and a piece of rock fell struck employee's left shoulder.
Employee exited mantrip at the end of shift and felt pain in lower back and left leg.
Employee was operating 4 man diesel ride in reverse. Employee struck head on high voltage cable, panicked and instead of applying brake the employee floored the accelerator causing injury to neck.
Employee was examining beltline and felt pain in right knee.

Roof fall occurred in the First North Main #1 entry xcut 29. The fall was L16' x W18' x T8' and was supported with 6' fully grouted roof bolts. The roof fall will not be cleaned up. Date of development was 9/2015.

Employee was removing panel rubrail from a shuttle car. When the rubrail was being placed on mine floor employee injured left thumb.

A roof fall occurred in the 9 seam in the 1st Northeast Mains at crosscut 5 entry 4. The roof fall measured approximately 16'L X 18'W X 8'T. The roof fall will not be cleaned up. The roof was supported with 6' fully grouted roof bolts and date of development was 7/2018.
Employee was walking in the last open between #4 and #5 entries on #3 unit. Rock fell out between roof bolts striking employee's right foot causing injury.
Employee was roof bolting in the face of 5R on #1 unit. Employee was in process of adding pinner steel and was struck by rock from immediate roof on right index finger causing injury.
Mantrip was leaving #1 unit at end of shift. Employee stated allowed right foot to extend partially out of mantrip. Mantrip turned at intersection employee's big toe was injured when right foot came in contact with coal rib.
Employee while gathering coal samples at the clean coal stacker placed right hand on winch. The winch popped causing laceration to employee's hand.
Employee was operating shuttle car #34 on the #3 unit. The employee turned too early in the shuttle car causing the shuttle car to turn into the rib instead of intersection between entries 7 & 8. Restricted duty as of 6/11/18.

Employee was walking to the face of #4 entry of #1 unit and bumped head on roofbolt.
Employee was removing curtain from shuttle car reel and felt pain in lower back.

A roof fall occurred in the 11 seam at crosscut 208 in entry 5. The roof fall measured approximately 20'L x 18'W x 3'T. The roof fall will not be cleaned up. The secondary escapeway was re-established in the adjacent entry. The roof was supported with 4' fully grouted roofbolts and date of development was 10/2007.
A roof fall occurred in the 11 seam between crosscuts 207 and 208 in entry 4. The roof fall measured approximately 25'L x 18'W x 3'T. The roof fall will be cleaned up and re-supported according to the approved site specific action plan. The roof was supported with 4' fully grouted roof bolts. Date of development was 10/2007.
Employee walked thru the underground shop and did not get low enough while walking under fire suppression system and bumped head causing laceration.
An unintentional roof fall occurred in panel 4, entry 6 in crosscut 56 intersection. The roof fall measured 18'L x 12'W x 6'T. The roof fall will be cleaned up and re-supported according to approved site specific plan. The roof was supported with 5' fully grouted resin roof bolts. Dated of development was 12/17.
An unintentional roof fall occurred in Panel 4, entry 7, crosscut 40 intersection. The roof fall measured 8'T x 19.5'W x 35'L. The roof fall will not be cleaned up and immediate area was re-supported. The roof was supported with 5' resin roof bolts. Date of development was November 2017.
An unintentional roof fall occurred in the First Southwest Mains entry seven, between crosscuts thirty-four and thirty-five. The roof fall measured 6'T X 19'W X 60'L. The roof fall will not be cleaned up and immediate area was re-supported. The roof was supported initially with 6' resin/tension roof bolts. Date of development was June 2010.
An unintentional roof fall occurred in the First Northwest Submain number eight entry, between crosscuts seventeen and eighteen in the straight. The roof fall measured 18'W X 16'L X 6'T. The roof fall will not be cleaned up and the immediate area was cribbed off and secured. The date of development was October 2017.
Employee was operating #5 stinger ride on #1 unit traveling outby the last open in the #5 entry. The employee drove thru a line curtain and was struck by a rock. The rock was approximately 4'W X 5'L ranging from 1/2" to 5 1/2" thick. No indicators of adverse roof and roof support was installed IAW approved roof control plan. The rock fell out between middle roof bolts.
IE was repairing bad splice on shuttle car in #5 entry. EE left for 10-15 min, returned & began working on splice. EE had not seen scoop in the area. With cap light & blinking light on, EE was on knees looking down. Scoop carrying supplies in elevated bucket was heading inby in #5 entry at walking speed. Scoop operator rang bell before & while passing through the airlock curtain.
Employee was in the process of attaching bull hose to the unit water tree, the water valve was inadvertently turned on the 1 1/2 inch water valve causing the bull hose to strike the employee in the mouth. The employee received a laceration to the upper lip. The employee received 8 sutures to the injury.
While roof bolting, miner complained of left knee pain and stated heard a "popping" sound. Miner was transported to local hospital, treated and released. On 08/22/2017 miner underwent surgery of unknown diagnosis therefore became a lost time accident on that date.
Miner in process of repairing roof bolter dust skirt out by face area when draw rock fell from roof striking left leg which was positioned on mine floor. Miner sustained an inline fracture of left tibia and ankle.
An unplanned roof fall occurred on the 1st Southwest Mains in the belt entry at crosscut #28.
A roof fall occurred on a worked out section not an active section in #1 entry spad #7850. 2 pins wide, 4 to 5ft high, pulled 5ft fully grouted pins, 20ft long, and 12ft in width.
The miner was helping the certified electrician splice A 300 volt D.C shuttle car cable. The miner placed the cable on employee's leg then cut the insolated jacket of the cable with A razor knife when the knife cut through the cable causing A laceration to the miners leg.
A water leak has occurred on the #2 section in the #1 entry. The water is leaking from the mine roof and adjacent pillars of the #1 entry. No injury occurred during the water leak. MSHA hotline was utilized and district personnel arrived at the mine site shortly following the report.
Miner was pulling up 995 continuous miner cable slack with A nylon strap in the deck of A shuttle car. The miner put hammer handle through the strap, the miner had hand on the hammer head when the cable became tight the miners hand was caught between the hammer and the car deck. The miner received A fracture to the left index knuckle.
Miner was in process of assisting to make a belt splice when employee's hand slipped and was struck on the Right ring finger causing laceration and fracture of right ring finger.
Miner was using sledge hammer on coal chute inside Coal Preparation Plant when the chute was missed with hammer head. Miners hand struck chute causing fracture to Left index finger and laceration to said finger.
A continuous miner operator was injured while operating the continuous miner, when a rock fell from the mine roof and striking small left toe on the side of the foot resulting in a small toe fracture.
An accident (unplanned roof fall) occurred at this operation, between the crosscut #29 and crosscut#30 of the #11 seam. MSHA was notified within 15 minutes of the roof fall being discovered, and issued a 103(k) order upon inspection. an action plan was developed by mine management to provide adequate supplemental roof support in the area.
Miner was in process of moving roof bolter for advancement of the roof bolting process, a piece of loose rock fell between the previously roof bolted mine roof striking employee's left forearm, causing laceration requiring 13 sutures.
Miner repairing shuttle car cable using utility knife, when knife slipped from the cable the miner received a laceration to the left index finger requiring five (5) sutures.

Employee was assisting the unit mechanic with changing shuttle car tire, the tire slipped causing the employee to fall on the tire, striking the wheel rim just above EE's right eye, resulting in 5 sutures.
Employee was making first cut in 4 right, spad +1860 Panel 21B, #1 unit. EE was standing with legs spread due to mining height. A rock, 4' x 4' x 4 inches thick fell from between four roof bolts, hanging on the outby rib bolt's board causing it to swing down and strike the employee's left leg resulting in a lower leg fracture.
During a 6.2 inch rainfall and due to a power outage, water entered the mouth of the mine, rising to approximately 6 to 8 inches for a distance of 10 to 12 crosscuts blocking the secondary escapeway.
While helping plant mechanic install belt on pump, employee's left middle finger caught between the belt and pulley resulting in a slight fracture of the finger.
Employee was cutting the bands off a bundle of top rollers when EE's knife slipped cutting EE's right wrist resulting in 6 sutures.
While stepping out of operator's deck of battery scoop, employee tripped on the seatback and caught them self with their right hand resulting in a slight fracure.
Employee was opening equipment doors when ee Getman tractor rolled into ee lower legs resulting in fractures to both legs.
While reaching for a roof bolt from the top of the bolter, a small piece of rock fell from the roof striking employee on the left thumb resulting in a slight fracture.
A roof fall occurred in the #9 seam in the 1st Nortwest mains (#3 unit MMU-004-0) in the face of #3 Right at spad +2000. No injuries or ventilation short circuit occurred as a result of the roof fall.

Employee was traveling back to the shop on 683 diesel ride. Along the 1SW Primary road at approximately crosscut 30 a roof bolt lying in the travelway was kicked up by the ride. It struck employee in the nose resulting in a fractured nose and multiple sutures.

Employee was walking to his roofbolter when he struck a piece of damaged skin control on a roofbolt cutting his left ear, resulting in 4 sutures.
A roof fall occurred in the #9 coal seam in the 2nd Southwest Mains at crosscut #1 in the #7 entry. No injuries occurred as a result of the roof fall.
A roof fall occurred in the #9 coal seam in the 3rd Southwest Submains at crosscut #80 in the #5 (Belt) entry. No injuries or ventilation short circuit occurred as a result of the roof fall.
Employee had moved shuttle car, while stepping out of the car, he slipped, catching himself with his left hand, resulting in a small laceration and the fracture of his left ring finger.
Employee was pulling up slack of the 5/8 belt sheave rope with a 5/5 belt chain with the back of the cab of #4 diesel scoop. The front tire ran over the belt chain slack and it swung around striking the employee above the right eye, resulting in 7 sutures.
Employee entered entry 1 to follow up on cleaning. 12 - 15' inby the xcut he heard the roof pop, looking up but not seeing anything he went on. A rock 73"x48"x4" fell from between the center bolts, knocked him to the floor and landed on his left foot, fracturing 4 toes and causing the amputation of his pinky toe.
Miner was struck by roof bolter cable after cable reel was engaged, causing a laceration to miner's mouth requiring sutures.
Miner walking and contacted "pizza pan" (surface control bearing plate) with right ear causing laceration.
Miner was unhooking trailer from scoop bucket when shuttle car struck trailer causing it to roll into scoop bucket crushing lower left leg.
Miner was pulling water hose thru a permanent stopping when he tripped over a concrete block on the mine floor. Miner fell to mine floor striking right wrist on additional concrete block, causing a wrist fracture.
An inundation of water occurred after a pilot bore hole was mined thru. the inundation affected the #1 Section, specifically entries #3, #11, and #12.
While positioning a step ladder to inspect a holding tank, the ladder slid forward and started to fall, the employee attempted to secure himself by grabbing the ladder rail, this entangled his wrists in the ladder railing. The ladder and employee fell to the ground.
Employee was trimming side of conveyor belt when utility knife slipped, came in contact with employee causing a laceration to the left thumb. The injury required four (4) sutures for closure of the wound.
Miner was in process of installing a man door in a recently erected permanent stopping when a block became dislodged from the top of the stopping, coming in contact with his right hand. This causing a fracture to the right thumb.
A roof bolter operator, in the process of bolting the #3 entry on the #2 section when a rock fell from the convergence area between the rib and roof. The rock struck the miner in the left waist area causing a dislocation of the left hip.
Employee was loading cans of oil into his scoop bucket when he raised up and struck a roof bolt with his left shoulder causing a laceration that required 10 sutures.
An unplanned roof fall occurred in the mine #11 seam at crosscut #204 in the beltline entry.
Employee was trimming the lead splice on the belt extension when the knife slipped, cutting his left leg resulting in three sutures.
Employee cut left leg with a belt knife while trying to remove a piece of rubber from the bit wrench, resulting in six sutures.
Employee was drilling a bolt hole when a rock broke from the roof onto his drill steel. Before he could stop the rotation, the rock flew from his steel striking him in the mouth, resulting in a laceration that required 5 sutures.
Employee got out of mantrip upon arriving on the section. He started walking toward the face and was bent over. He hit his neck on a roofbolt plate, causing a laceration that resulted in 5 sutures.
While removing transmission from D9L fastened transmission to the boom of the mechanic truck. After attempting to loosen the transmission with the boom, employee stepped forward to see why transmission would not come loose. The transmission "popped off" striking the employee in the chest and knocking him against the side of the mechanic truck, resulting in six fractured ribs.
A roof fall occurred in the #9 coal seam in the panel 11. The roof fall occurred in the primary escape way at crosscut #44 in the #7 entry. No injuries or ventilation short circuit occurred as a result of the roof fall.
Miner was removing wire rope from shuttle car slide anchor when wire rope unwound from clevis striking miner in the face causing a fracture of his nose.
A roof fall occurred in panel 11 #4 entry Crosscut #23, this was in the secondary escape way. MSHA was notified by telephone on 1/18/2014 at 1:17 P.M. The lifeline was re-routed, miners were advised of the change and the escape way map was also revised. All intersections leading to the fall area has been barricaded off and supplemental floor to roof support installed.
A roof fall occurred in Panel 11 at crosscut #34 in the #2 entry. This roof fall was reported to MSHA by telephone on 1/17/2014.
Miner was roof bolting when drill steel became clogged, miner reached for drill steel when a rock fell from mine roof causing a laceration to his left thumb.
A roof fall occurred in the 3rd Southwest Mains in the intake travel way at crosscut #95. The fall occurred in an intersection.
Employee set up a 8 step ladder on unstable ground. The side he was climbing was on concrete at the other side was in loose mucky coal. Employee went to 3rd rung to check footing of ladder and ladder begin to sink. Employee jumped off ladder and broke his right ankle.
Employee was in the process of moving a water hose from the 2nd floor to the bottom floor. The hose was secured to the rail with electrical tape and zip ties. While cutting the tape with a belt knife, the knife slipped cutting the corner of his left thumb, resulting in 3 sutures.
The continuous miner cut into a sealed area from 10 to panel 9.
Employee was cutting belt to make a splice using a belt knife. He was cutting towards himself when the knife slipped off the belt cutting his right thigh resulting in 6 sutures.
Driving up road on the way back from 4th 54 belt drive Didn't see roof bolt and made contact causing a cut on the top of his head resulting in 5 staples.
A roof fall occurred in Panel 7 at X/C #133, this is a worked out area and in an intersection in the examiners travel way.
Employee was on scoop unplugging the batteries when he lost his balance and fell off the scoop. He caught his right knee in the fender well resulting in a meniscus tear.
An unplanned roof fall occurred at crosscut 165, on the 5th 54 beltline in the #11 seam. The roof fall is located in an intersection and will be cleaned up.
While exiting a loader on the surface, the miner scraped his right lower leg on a fire suppression retention bracket causing a skin tear that required 18 sutures to close the wound.
Miner was hanging high voltage cable when cable slipped from his hand striking him in the mouth, causing a broken tooth.
While lifting a pipe with a chain hoist, chain slipped from pipe. The pipe struck the miner in the right side causing (2) fractured ribs.
Employee was in the process of moving an oxygen and acetylene cart when he stumbled and fell. The cart fell across the side of his right knee and leg, fracturing his lower leg.
It was determined that the roof had deteriorated which caused the roof to fall to the mine floor. The fall was 70' long starting in the intersection of cross cut #20 along the 1st Southwest mains in the #5 entry going toward #4 entry. The fall was 18' wide and 15' plus in height.
Employee was removing rock duster hose with belt knife, Knife slipped, cutting tip of left ring finger - resulting in five sutures.
Miner was spreading legs of step ladder when ladder began to fall, miner attempted to restrain ladder and reports lower back strain/sprain. This 7000-1 form is being completed to terminate citation # 8513327. Miner reports the accident occurred on 3-22-13,however did not report accident until 5-22-13. Therefore there were 0 days lost or restricted duty.
Holding bolt on blade edge of D-9 blade while using impact wrench to remove bolt. Bolt head spun, cutting right index finger, requiring 5 sutures.
While rock dusting employee was taking hose off of duster when hose came off, it struck his left ring finger resulting in a slight fracture to his knuckle.
Driving outby, ride ran over rock, swinging ride into rib. Hit left forearm against the rib resulting in a slight fracture and surgery on 1/5/13.
Employee removed the bottom steel from the bolter pot to check the suction. As he placed his left hand over the pot, the remaining steels fell from the bolthole he was drilling and struck his left hand, fracturing his middle finger.
Employee was riding down 6th 54 supply road on mantrip 387 when his hardhat fell off. As he replaced his hardhat, his left ring finger was pinched between his hat and the roof, fracturing his finger.
While tramming the roof bolter in reverse from the cab through a check curtain, employee reached out to put the power cable on the back bumper of the machine when a bundle of roof bolt plates fell off the top of the cab, striking him on the right hand resulting in a fractured middle finger and six sutures to his ring finger.
Walking out of shower, slipped and fell chipping tooth.
Employee was loading rock dust onto a coal scoop. He picked up a bag, turned to put the bag on the scoop and his feet tangled up twisting his lower back.
Roof Fall in the #9 Seam at crosscut 25, spad +2000 in the #1 entry of the 1st West Sub Mains. Fall was approximately 6' High x 9' wide x 12' long. Fall was between cribs, Ventilation was not affected and no one was trapped or hurt.
Loading bolter, fell over pins laying in crosscut. 5' bolts were on the ground. As employee approached the bolts, he tripped on a bundle his right leg landing on another bundle. This caused a 1" x 4" laceration that required 43 sutures.
Drilling a 8' hole and his steel stopped up, he pulled one section of steel out of the roof, placed his right hand over the pot to increase suction, then the other section fell out fracturing his right thumb.
After installing coax cable and as he returned to his ride employee scrapped his right thigh against the spool reel holder on the ride. This resulted in a small laceration that required 5 sutures.
Small piece of rock fell from between roof bolts and cut left arm while employee was attempting to install a roof bolt on the rib side of #7 entry. The laceration required four(4)sutures.
Employee was helping the shop repairman repair A-arm on a diesel mantrip. While releasing the track jack middle finger of left hand was pinched between the jack handle and the wheel well causing a hairline fracture to finger.
Employee had removed the cover over the left miner' s cutter motor compartment and began changing the ball valves for the headsprays when a rock 29"x70"x5" thick fell from the roof striking the employee on the middle and lower back. The rock fell between the left rib pins and the rib.
Pulled cable off the back of the ride and struck my left elbow against a car cable hanger plate and cut it open, requiring seven sutures.
Breaking a band on a bundle of roof bolts and the band flew up and cut right forearm, requiring 5 sutures.
Loading opposite operators side of roof bolter. Reached across supply trailer for a box of glue, as he turned toward the bolter his left forearm contacted a band on a bundle of pins causing a laceration that required three sutures.
While attempting to re-hang a pinner cable, the roof bolter moved up in the next place over, not knowing the cable reel was locked. Cable came down, knocking employee to the ground. Incident only, finished shift. Wasn't reported to Safety Dept until 05/15/2012 Hernia surgery on 6/19/2012 making this a lost time accident.
Employee was working on the 2nd Southwest tailpiece when the belt framing slipped and struck left leg. The resulting laceration required 7 sutures on the inside of the cut and 11 on the outside for a total of 18 sutures.
Employee was on the engine compartment of #8 Getmam Tractor removing the cover bolts, Employee slipped off the tractor and landed on his left knee. Worked until knee surgery on 09/14/2012
An unplanned roof fall occurred, discovered at 10:20 a.m in the 2nd Southwest mains at the #79 crosscut, between the #9 and #10 Entries.
Bolter was hung up in mud in #4 entry, scoop was pulling bolter out and employee had his hand on top of his canopy. His hand was mashed between canopy and the top.
Opening 6'x6' door in stopping stepped backward and left knee popped. Worked full duty until knee surgery on 6/14/2012 making this a lost time accident.
Employee was hanging cables when he turned and caught his right ear on a piece of hog panel hanging from the roof. Laceration required 9 sutures.
Roof Fall was found at 8:00 am in the #11 seam at 2nd Main North xcut 190 #6 entry. The fall is 55'x 18'x 3-8' high. The fall has been cribbed off and did not affect an escape way.

Employee was sitting on the ground in an intersection working on the miner cats when a scoop came through a curtain out-by, struck their ride that was out-by the miner, pushing it into the injured man pinning him between the ride and the miner.
I was hanging miner cable in #4 entry on #2 unit. I lifted the cable up and felt a sharp stabbing pain in my stomach by my belly button. I didn't think anything about it at first, but after I got home I couldn't move without severe pain in my stomach.

Getting out of the operators compartment of #2 diesel scoop lost his footing and fell. While trying to catch himself he hit his arm on the deck of the scoop. Fracturing both bones in his left forearm.
Roof fall was found in the #9 seam in the 6th South Panel, #6 entry crosscut 80-81. The fall measures 80' long by 18' wide by 6' high. Primary support in that area is 5' stick pins. The fall is not in an escapway and ventilation is not affected. The fall will be dangered out, with no plans of cleaning up.
Walking on belt line said too much weight on right leg, slipped and fell on back. Had pain in back and right leg was trying to step up on walk board which was wet and muddy.

The employee picked up a belt roller and when he turned to install it he fell to the ground and was having pain in his lower back and left hip. The employee was not taken off of work by a doctor until 10/10/2011.
While operating a battery scoop, there were some chains in the operators compartment and one of the chain ends fell through a hole in the operators compartment and was run over by the wheel and as the chain jerked the hook end struck the employee on the index finger of the left hand. He received 6 sutures.
While installing roof bolts the employee had just drilled the first part of a two part hole and as he lowered his boom and reached for the second drill steel, the first drill steel came out of the hole unexpectedly and struck him on the top of the left thumb fracturing it.
Employee was installing roof bolts and as he was kneeling on some rock his knee slipped and as he reached to catch himself with his left hand he hit the boom lift lever and as the boom lifted, his right hand was on the boom and his index finger was mashed between a bolt on the boom and the boom itself. He received 6 sutures.

The primary Roof Support was 6' Tension re-bar bolts, and 9' supplemental support in the intersections. The roof fall occurred 9,951' from the #1 working section. The fall was timbered out with cribs built in intersections connected to area.
Employee was walking up the beltline when right knee gave out and he felt a sharp pain in right side of knee. Worked regular duty until he had knee surgery on 1-18-12, becoming a lost-time accident.

While unloading some steel pipes, one of the pipes shifted unexpectedly and pinched the employee's left wrist between two of the pipes. The employee received a hairline fracture.

While prying on a cat pad, the pry bar slipped and the employee hit the back of his hand on a jagged piece of metal causing a laceration. The employee received 1 suture to the middle finger on his left hand.
A roof fall occurred in the #11 seam in the 2 Main North at crosscut #115 in the intersection of #6 entry. The fall measured approximately 25'x18'x6'. The area was supported with 4' roof bolts.
While operating a roof bolting machine the drill steel became stopped up and when the employee dropped the boom to check the problem he positioned his hand on top of the boom and the drill steel fell out of the hole and struck him on the finger. The employee received sutures.
While getting ready to drill a hole with a roof bolting machine the employee positioned the steel against the roof and accidentally moved the lever in the wrong position jamming the steel into the mine roof and the steel popped back abruptly hitting the employee on the index finger on his left hand causing the finger to be dislocated.
