An unintentional roof fall occurred in the #4 entry at the #67 crosscut on the #3 belt. The fall measured approximately 20' wide x 6' high x 40' long. Area was supported by 4' fully grouted bolts. No injuries, no equipment caught & ventilation was not interrupted.

Employee was in a kneeling position organizing roof bolting supplies on the Fletcher roof bolter when a rock fell from between bolts striking him on the ankle of his right foot. Rock measured 36" long x 26" wide x 2" thick.

No injuries, no equipment caught & ventilation was not interrupted. Fall will not be cleaned up. Roof fall was found during weekly examination.

Employee was loading supplies onto motor, operator told him he was moving. Injured employee did not realize how close he was to motor. The motor caught the tip of his left boot and rolled it over against the ground, causing an injury.

EE COMPLETED WORK SHIFT. WENT TO DR ON 1-10-03. X-RAYS OF HIS ANKLE REVEALED FRACTURE TO HIS ANKLE (LEFT).
