Found an unintentional roof fall in the #4 Entry at Break 75 on Slowpoke Mains

Employee was walking and twisted his right knee and experienced some pain and swelling after getting down on knees to hang a cable. Started missing work on 10/12/2015.

Employee was loading roof bolter with supplies and a particle of dirt got into his eye. Antibiotic drops were prescribed for his eye.

Employee was taking belt structure apart and caught his right index finger between the belt rail and a belt stand and cut his finger. Sutures were required. He returned back to work the next shift.

Employee was installing a roof bolt and pushed the bolt forward and felt a "pop" in his left shoulder. He started missing work due to the pain getting worse over time. He started missing work on 8/19/2015.

The employee was struck by a falling rock brow and suffered a broken leg. Surgery was required to repair the leg. He started missing work on 8/10/2015

Employee was installing a bearing onto the #7 tailpiece roller and turned the bearing and caught his right middle and ring finger between the bearing and tailpiece. He cut his fingers and damaged a tendon. Sutures and repairing of the tendon were required.

Found an unintentional roof fall next to the #2 seal at the #6 set of seals adjacent to Old #1 Switch.

Employee got his foot stuck in the mud and twisted his right knee. He started missing work 6/10/2015.

Employee was riding on a rubber tired mantrip buggy and his foot caught the corner of a coal rib and it fractured his leg just above the ankle.

Employee dropped a scoop battery lid on his left hand and cut his left 3rd digit finger. Sutures were required.

Employee was climbing up onto the CAT Forklift and his right knee popped. He continued to work but his knee was not improving. He started missing work on 12/1/2014.

Employee stated he was helping make a belt setup and his back started hurting after he left and was on his way home. He did not report the injury to the company until 11/20/2014.

Found an unintentional Roof Fall in the #2 Entry at Break #84 of the Slow Poke Mains.

Employee was letting a LoLo dolly down with a lifting jack and the jack bar sprung back up and struck his chin. 3 sutures were required.

Employee was changing a battery on rail personnel carrier and the battery popped and splashed battery acid into his eyes. Returned to work his next shift then went to the Dr. and started missing work 9/15/14.

Employee was hanging a curtain and turned around and hit his back on a nail sticking down from a roof bolt plate and cut his back. 3 Sutures required.

Found an unintentional fall approximately 20 breaks inby #1 Switch on Doty Mains.

Employee was standing beside the flat bed truck when another employee was attempting to unload a ram car boom jack. The jack spun around on truck and fell off truck and one end of the jack struck the employees left foot. Fractured foot.

Employee was observing the continuous miner and a piece of draw rock fell and struck the back of his leg and ankle. Fractured ankle.

Employee was getting a stuck rock out of a belt-drive chute, as he had a hold of the rock, it dropped down and caught his left 4th and 5th digit fingers between the rock and the belt scraper bracket and cut both fingers. Sutures were required.

Found an unintentional roof fall at Break #61, #6 Heading, parallel with the #6 Beltline. The fall did not affect ventilation or affect the escapeways. The other entries through the area were common to this fall area and were travelable.

Employee was picking up a main pump for a roof bolter and the motor shaft cut his left hand between the thumb and index finger. 5 sutures were required.

A portion of a coal rib was being removed with a jack hammer for the purpose of correcting an issued close clearance citation. The injured employee was removing the loose coal away from the rib when a rock brow fell onto him.

Employee was using a steel rail to push the low structure over and the rail flipped up and struck his jaw and head. He has been diagnosed with a fractured jaw and will require surgery. Started missing work on 2-13-14.

Employee was cutting and squaring up the end of a piece of belt and his knife slipped and cut him on the right leg...4 sutures required

Employee was operating a rail motor and traveling on the track and hit a curve too fast and jarred his back. Employee returned back to work the next day but his back was not getting better and he started missing work 8/26/2013.

Employee was getting a rock off of a curtain and hurt his right arm. Tendon torn and surgery was required. Employee had surgery and started missing work on 7/12/2013.

Employee was using a screwdriver to help peel the jacket off of a cable and slipped and struck the palm of his left hand.

Employee strained back while pulling and lifting on a ATRS extension. It was just learned that employee did return to work on his next shift and worked 5 days restricted duty before being released to full duty.

Employee was cutting a piece of belt with a utility knife and the knife slipped and cut his left hand.

EE was cutting a boot off of a roof bolter cable and cut his left thumb.

Employee was replacing a belt roller and strained his back.

EE fell forward in the deck of a rail motor when it derailed and wrecked. EE was diagnosed with two fractured ribs.

Hit his ear against a piece of rock. 2 sutures required.

Employee was moved bolter forward and had his left hand on the canopy when a small rock fell and hit his left thumb. Diagnosed with a stress fracture-Employee is working his regular job.

Employee was trimming a piece of belt on a tailpiece guard and cut his right hand. Sutures required - no lost time.

Employee was sitting down in chair attending to the L-3 communication system and CO monitor system. He was getting up and fell forward out of the chair. He has been diagnosed with a fracture to his shoulder. He started missing work 2/1/12.

He was pulling a roller out from the belt and caught his right pinky finger between the roller and a belt stand. Fractured finger.

Employee was picking up a hydraulic jack and felt a "pop" in his right shoulder. Injury is going to require surgery started missing work 1/24/2012

He was operating a supply motor on the track and it derailed and caused him to hit the mine roof with his face and shoulder. Diagnosed with cuts and contusions. He is working at a restricted duty status.

He was helping lay track and caught his right index finger and middle finger between the bottom of a track tie and the bottom and cut the tips off each finger.

He was walking in low top and struck his back on a nail hanging in a roof plate and cut his back. Sutures were required.

Employee was pulling a piece of track steel from a supply car and slipped and fell and the track steel hit him on the left ankle - fractured ankle. Started missing work 7-6-2011.

EE started to push drill steel up to drill hole and a piece of rock cut his left hand.

EE was using a hacksaw to cut down loose roof bolt and cut his right wrist.

A contract employee caught his hand between the roof bolter and the coal rib and cut his hand. Sutures were required.

Using a hand held grinder and grinder wheel broke and cut his finger. Sutures were required.

Employee was plastering a brattice and a block fell and struck him on the hand. He is working at a "restricted duty" status.

EE was riding in a supply motor and hit his head on a overcast. Sutures were required.

Walking across section and hit back on a nail hanging down from a roof plate. Sutures were required.

Putting a supply car on the track and the car hit him on the left knee and it come onto the track. Started missing work 3/15/11.

Trimming splice and knife came into contact with left hand. Sutures required. No lost time.

He was standing beside the Continuous miner when the cable and cathead was pulled off and the cathead struck him on his right middle finger. 5 sutures were required.

Beating a race into wheel and pin slipped and cut finger. 2 sutures required.

Got out of mantrip car, stepped up and twisted knee. Started missing work 11-16-2010.

EE was cutting belt, knife slipped and he cut his left hand. 4 sutures required.

EE caught his left ankle between the rib and a rail motor and fractured ankle. Started missing work 10/2/10.

EE stood up and something in his foot popped. Diagnosed with a simple hairline fracture.

EE was tramming scoop and hit his hand on a rock and fractured his left index finger.

EE was using a lifting jack and it fell over and hit his right middle finger. Fractured finger.

EE stood up in rail motor and accidentally hit the tram switch which caused him to fall down against the top of the equipment. Started missing work 9-9-2010.

EE stepped off rail motor and twisted his right knee. Started missing work 8-13-2010.

EE was setting timbers and cut his right thumb on a sharp rock.

He was taking a rub rail off of the miner and caught his right middle finger between the rub rail and lolo structure. Finger was amputated to the 1st joint.

Letting bolter cable down and cable hook caught his right hand between the thumb and index finger cutting his hand. Sutures were required - no lost time.

Found an unintentional roof fall on the #9 belt. Fall was 40' x 20' x 5'H.

He dropped a lifting jack on his right thumb. Thumb was fractured. Co just found out 6/4/2010 by medical records.

EE was standing beside the mine track at the portal and the rockduster wrecked off the track and caught his left leg. Fractured leg. Started missing 5-5-2010 and RTW 8-23-10 (75 days lost). EE started missing again due to complications on 11-1-2010.

Lifting jack kicked out and put pressure on arm and jerked jack bar out of hand - hurt shoulder. Started missing work 11-29-2010.

EE was driving a buggy in the return and ran over a rock and caused the steering wheel to hit his arm.

Scoop was tramming through a break and pushed a piece of 4" waterline into employee's ankle. Fracture ankle. Started missing work 1/22/2010.

Shale roof broke away from sandstone top.

He was loading belt structure and had a pain in his groin area. Diagnosed with a hernia. Had surgery on 1/15/2010.

Pulling a metal band and lot right arm-* sutures required. no lost time or restricted duty.

Rockfall in neutral entry

EE cut his hand on the banding around a bundle of roof bolts. Sutures required.

EE was throwing block off a supply car, he slipped and fell on his right shoulder. EE started missing work on 08/27/2009.

A chain broke and a piece hit him on the lip. Sutures were required.

Putting up belt curtains and a rib rolled off and struck his right foot. Started missing 5-4-2009.

Lifting on belt structure and felt something pull in his lower back. Started missing work 4-22-09.

Trying to get belt roller unstuck.

Using a hand hydraulic drill and the wire that holds drill steel on caught his shirt and cut his left forearm. Sutures required.

Employee was knelt down (drinking water) and a piece of rock (about 4 lbs.), fell from the top and landed on his left hand, injuring ring and middle fingers.

Employee was struck by a rock that caused a fracture to his left leg and left index finger. Started missing work 2/3/09.

EE was using a jack to put supply car on the truck, he slipped and fell forward and hit his finger on the rail. Sutures were needed to close the wound.

He was cutting a piece of belt, knife slipped and cut his right leg. 4 sutures required.

Employee was loading belt structure, twisted and felt sharp pain in lower back.

EE was walking up beside the continuous miner as they were setting bits and got a piece of metal in his eye. Started missing work 7/1/08

Employee strained his back while picking up a roll of belt. He started missing work 5/12/08.

A piece of drawrock fell from between the bolts and rib and struck EE on lower back. Started missing work on 4/24/08.

Cutting jacket on a cable with a utility knife and cut left thumb. Sutures required.

Putting a car on the truck and the jack bar hit him on the forehead. Sutures were required just above eyebrow.

Dropped top roller on finger and cut finger-sutures required.

Using a jack to raise up belt drive, bar on jack broke and cut his leg. Sutures required - no lost time.

Employee was shoveling belt line and twisted right knee.

EE was splicing car cable and cut left leg with knife. 3 sutures required. No lost time.

Employee was walking to shuttle car, stepped on rock and sprained his right ankle.

EE was going under a bridge conveyor and caught his leg and hip. Dislocated femur. Started missing work 10/18/07.

EE was cutting a cable and knife slipped and cut his leg. Ten sutures were required. No lost time.

Employee was walking to mantrip, stepped in hole and twisted knee.

Bolting top, a loose piece of rock fell and hit his left arm.

Employee was unloading a box of glue and twisted his lower back area.

A piece of rock brow fell after employee let bolter ATRS down and struck employee. He has a fractured pelvis and back. Started missing work 9-5-07.

Had an unintentional roof fall in the #5 entry of Gateway mains between the # 54 and #55 breaks. Fall was 6' high x 20' wide x 50' long.

Employee was hanging bolter cable, reeling up cable. The cable struck him in the mouth breaking a tooth and busting lip.

Employee was shoveling belt, he over extended and twisted his lower back.

EE was looping up belt and emergency phone line caught on scoop and broke and slapped him in the face and eye. He was diagnosed with a detached retina on 7-6-07. Started missing work on 7-9-07

Employee was shoveling belt line, lifting shovel above head and strained lower back area.

Employee was building seals and got mortar inside boot.

EE was scooping in old works and scoop bucket caught a piece of 4" plastic waterline, it sprung back and hit ee in face. Diagnosed with facial fractures. Started missing work 3/12/07.

Belt was being installed. A loop of belt caught the shuttle car cable that was hung up and it hit and knocked employee down. He has been diagnosed with a fractured tailbone. Started missing work 2/22/2007.

Scoop was pulling belt around the rib and the belt caught a shuttle car cable and pulled it down. It struck employee and knocked him down. He has a bruise on his back and soreness to his back. He started missing work 2/26/07.

Tramming bolter & got foot caught between rib & bolter.

Ee was pulling on conveyor belt and felt a pain in his groin area. Diagnosed with a hernia. Started missing work 3/16/07

Lifting belt roller and had sharp pain in lower back.

While getting out of scoop, utility knife on hip flew open and cut 1 each index finger.

Working on shuttle car, lid fell and struck his finger. Sutures required.

Employee put a bolt in, brow broke next to the bolt and a rock fell, striking right foot.

Tool box lid fell down and struck EE on his elbow and hand. Started missing work 10/3/06.

Employee was re-hanging a fire deluge and cut his finger on spray nozzle. Required sutures.

Slope car was idled due to pump motor going down.

Employee twisted his knee getting out of a rail motor. He is working restricted duty.

Employee stepped down off of a lolo structure & sprained his ankle.

EE was hitting a metal dolly wheel with a hammer and got a piece of metal in finger. Required surgery & 1 suture. He is working on restricted duty.

Opening a track switch and hurt back. He is currently working but is on restricted duty.

A rock hit his left hand and bruised his hand.

Employee was helping raise belt, slipped and fell, injuring left hand on rail.

EE was loading a waterline onto the scoop and it hit him in the nose and face.

EE was pulling a rock off of the continuous miner and cut his right 5th finger. Required 7 sutures.

A piece of rock was dislodged by bolter barrel and hit EE on upper lip.

EE cut left ring finger on a belt roller while loading into scoop bucket. Required 6 stitches.

EMPLOYEE WAS OPERATING BOLT MACHINE WHEN ROCK FELL, HITTING HIM IN THE LOWER BACK.

KNEE PAD STRAP BROKE OFF, PAD CAME OFF & EMPLOYEE HIT HIS KNEE ON A ROCK ON THE BOTTOM OF MINE.

HANGING ROPE ON DOLLY, GOT FINGER CAUGHT IN ROPE, CAUSING ROPE TO PINCH & CUT HIS FINGER.

A piece of draw rock fell between bolts and rib and struck him, pushing him down.

EE WAS WALKING AND STEPPED INTO LOOSE HANGING ROOF BOLT & CUT LEFT SIDE OF HEAD ABOVE THE EAR. 7 SUTURES REQUIRED.

EMPLOYEE WAS LIFTING A CHAIN & HIS BACK STARTED HURTING. STARTED MISSING WORK ON 3/10/06.

Employee was bolting top and a piece of rock fell and he sustained a broken hip. Started missing work 2-3-06.

Handling belt structure and mashed left middle finger between a bottom roller and belt stand. Sutures required.

Employee was plastering brattice and plaster got inside a boot irritating skin on lower legs.

AN EMPLOYEE OF KINGS TRANSPORTATION WAS DRIVING ON THE ROAD FROM THE MINE & HE PULLED OVER. HE WAS APPARENTLY HAVING A HEART ATTACK. CPR WAS ADMINISTERED FROM COMPANY EE'S. HE WAS TRANSPORTED TO A HOSPITAL, HAD SURGERY & DIED THE NEXT DAY.

Employee was picking up garbage, bent over and a rock fell between bolts, striking him in the lower back.

MINE EXAMINER FOUND A 40' LONG X 25' WIDE X 5' HIGH UNINTENTIONAL ROOF IN THE #2 ENTRY 20 BREAKS OUTBY THE 001 MMU.

EE was splicing phone line and cut top of left hand. Required 7 stitches.

Has pain and numbness in hand while pulling cables, etc. He has been diagnosed with carpal tunnel syndrome. Had surgery on 12/30/05. Started missing work 1/2/06.

WALKING BELTLINE AND EMPLOYEE STEPPED IN A HOLE, SPRAINING HIS ANKLE.

Hauling waterline on scoop and it caught top, hitting him in mouth.

Run scoop over gob and hit top, strained muscles in neck.

PIECE OF ROCK FELL AND CAUGHT LEFT ARM

Tripped over concrete block, straining knee while loading glue onto bolt machine.

EE bumped his left elbow against the rig. He later had some swelling and pain. He did not report injury until 8-29-05. Started missing 8/23/05. RTW 9/6/05.

MOVING BELT STRUCTURE FOR NEXT BELT SETUP. EE WAS CARRYING 48" ROLLER AND TURNED TO GO TO POWER CENTER AND TWISTED HIS BACK.

Employee cut his left thumb while using a utility knife. 2 sutures required.

EMPLOYEE WAS BUILDING A BRATTICE AND A CONCRETE BLOCK FELL AND STRUCK HIS FINGER, REQUIRED SUTURES

EMPLOYEE WAS SHOVELING BELT AND STRAINED BACK. STARTED MISSING WORK 7/24/2005.

EMPLOYEE DROPPED BELT RAIL ON LEFT LITTLE FINGER AND SUFFERED A FRACTURE AND LACERATION

EMPLOYEE SLIPPED AND FELL INTO A RUNNING BRIDGE CONVEYOR AND SUFFERED A FRACTURED RIB AND BRUISED ABDOMEN. STARTED MISSING WORK 6/20/05

EMPLOYEE RECEIVED 8 SUTURES AFTER STRIKING HIS ARM AGAINST THE ROOF BOLTER CANOPY.

Employee was shoveling belt line and said his arm was sore.

EMPLOYEE WAS USING AN ACETYLANE TORCH TO CUT A WATER SPRAY BLOCK OFF OF THE CONTINUOUS MINER. EE GOT A PIECE OF METAL SLAG IN HIS RT. EYE & WENT TO THE DR. TO GET IT OUT.

Oxygen tank fell over while employee was untying tank from top and it struck ee's right ring finger. 3 sutures required.

EMPLOYEE WAS PUTTING TOGETHER LOLO, SLIPPED AND FELL DOWN CAUSING A CONTUSION TO THE RIGHT LEG.

EE STARTED MISSING WORK ON 03/07/2005 DUE TO PAIN AND SWELLING FROM BEING ON KNEES. EE HAS SOME FLUID DRAWN OFF LEFT KNEE

EE GOT BETWEEN SCOOP AND THE COAL RIB AND SCOOP PUSHED AGAINST EE AND CAUGHT EE AGAINST THE RIB. EE STARTED MISSING WORK 03/02/05.

EE WAS GETTING ROCK OFF BELT LINE AND ROCK DROPPED DOWN AND CAUGHT EE'S RIGHT INDEX FINGER. THIS PARTIALLY TORE THE END OF EE'S FINGER OFF. EE RECEIVED SUTURES. NO LOST TIME.

FORWARD & REVERSE RELAY SWITCH MALFUNCTIONED ON SLOPE HOIST CAR.

Slope hoist was inoperative for approx 1 1/2 hours due to a faulty relay switch.

Employee fell down on hand and fractured wrist.

EE KNEELED DOWN AND SOMETHING IN RIGHT KNEE POPPED. EE IS OFF WORK UNTIL AN MRI CAN BE DONE AND EVALUATED. EE STARTED MISSING WORK 10/21/04.

EE WAS OPERATING A ROOF BOLTER AND A PIECE OF DRAW ROCK FELL AND STRUCK EE ON THE BACK OF HIS HEAD AND NECK. EE HAS ONLY A CONTUSION. STARTED MISSING WORK 10/21/04.

EE CAUGHT GLOVE BETWEEN DRILL STEEL AND ROOF BOLTER CHUCK AND TWISTED HIS LEFT WRIST. AFTER A BONE SCAN WAS DONE A SLIGHT FRACTURE SHOWED UP IN HIS ARM. EE STARTED MISSING WORK ON 10/18/04.

Employee had an allergic reaction to a "block bond" brattice sealant and started missing work on 9-29-2004 due to swelling & lesions on hand and arm.

Employee started having numbness and tingling in both wrists after shoveling belts. Diagnosed with carpal tunnel syndrome. Had surgery and started missing work 12/20/04.

EE SLIPPED FORWARD WHILE HOLDING A RAIL MOTOR COUPLING AND STRAINED HIS LOWER BACK. EE STARTED MISSING WORK 9/8/04.

Employee was scooping up old belt drive hole and was struck on the nose by a dislodged piece of water line.

Employee was putting a bearing on a shaft and got a small piece of metal in eye. Required surgery to remove particle.

Employee cut his left hand on a piece of drill steel that stuck in the top while bolting.

EMPLOYEE RECEIVED AN ELECTRIC SHOCK TO HIS BODY AS HE WAS USING A "THUMPER" TO DIAGNOSE A GROUNDED CABLE. HE WAS DIAGNOSED WITH ONLY A CONTUSION TO HIS ARMS AND WAS TREATED AND RELEASED FROM THE HOSPITAL. NO LOST TIME.

SLOPE CAR WAS INOPERATIVE DUE TO A FAULTY FAST/OVERSPEED SWITCH. SWITCH WAS REPLACED AND SLOPE CAR WAS PUT BACK INTO OPERTION AT 7:15 AM.

EMPLOYEE HIT LOW BROW WHILE RIDING MANTRIP BUGGY AND HAD SOME SORE RIBS. DIAGNOSED WITH ONLY A BRUISE. STARTED MISSING WORK ON 07/20/04 & RTW ON 07/26/04.

SLOPE CAR WAS SHUT DOWN AS A PLANNED PREVENTIVE MAINTENANCE ON THE SLOPE ROPE. ROPE WAS TURNED AROUND AND RESTOCKED TO HOIST & SLOPE CAR. SLOPE/HOIST WAS DOWN FOR APPROX. 6 HOURS.

EMPLOYEE HIT ELBOW AGAINST BIT TRAY ON ROOF BOLTER. HE HAS BEEN TAKEN OFF WORK FOR TERAPY FOR THE TENDON IN HIS RIGHT ELBOW. EMPLOYEE STARTED MISSING WORK 8/10/04.

CONTINUOUS MINER WAS STUCK IN PILLAR LINE & THEN SOME ROCK FELL ON MINER. MINER RETRIEVED ON 6/29/04 AT APPROX. 12:00PM.

Hurt back while pulling old ball gears away from drive.

Employee bumped his elbow on a belt tailpiece causing some pain and swelling. Had surgery on 7/2/04. Started missing work on 7-2-04.

AN UNINTENTIONAL ROOF FALL OCCURRED ON THE 009 MMU AT BRK #29 IN THE BELT ENTRY. ROOF FALL WAS APPROX. 50' WIDE X 25' LONG X 6' HIGH. LAMINATED SANDSTONE/SLATE.

EE COMPLAINED OF PAIN AND A HERNIA THE DAY AFTER WORKING ON 5/28/04. EE HAS A RUPTURED HERNIA IN THE GROIN AREA AND SURGERY IS SCHEDULED FOR 6-10-2004. EE STARTED MISSING WORK 6-1-2004.

EE WAS GETTING A ROCK OUTO F A BRIDGE CONVEYOR AND ANOTHER ROCK SLIPPED DOWN & CAUGHT EE'S LEFT LITTLE FINGER & CUT END OF FINGER & GLOVE.

SLOPE HOIST WAS INOPERATIVE DUE TO A "SHORT" IN A CONTROL WIRE AND A TIMING RELAY WAS REPLACED.

2' TO 3' OF ROCK FELL ON THE HEAD OF CONTINUOUS MINER CAUSING IT TO BE STUCK FOR APPROX. 4 & 1/2 HOURS IN THE PILLAR LINE OF THE 009 MMU.

SLOPE CAR WAS INOPERATIVE DUE TO BATTERIES & DEFECTIVE WIRING

A SMALL PIECE OF DRAWROCK STRUCK EE ON LEFT HAND AND FRACTURED LEFT INDEX FINGER.
