The employee smashed the tip of right small finger between the outside of the boom and the inside of the valve assemble located on the operators compartment while installing chain hangers.

The employee was standing beside of EE's tractor (7024) when the 7041 tractor that was parked across the road from EE rolled into EE's right leg causing a laceration to EE's right leg that required sutures.

There was an unplanned roof fall at #9 1st East Main between 10+40 and 11+50 in the #4 entry.

Employee was loosening a J-bolt on a bottom belt hanger bracket with right hand and left hand contacting the top chair for leverage. While loosening the bolt left small finger accidentally contacted the top roller causing a friction burn from contacting the roller. They received three stitches to the finger.

Employee had finished bolting the #6 entry on the #4 unit and was standing beside the roof bolter between the dust box and the boom when a hose fitting broke causing the employee to get oil in their eyes. Employee was treated for a scratch to their eye.

While testing the drum brake on the Hanson hoist it was discovered to allow the drum to move while under a load from the electric motor. The drum was found to have a residue that was removed and the system then passed the performed test.

Employee was drilling the roof when a piece of slate broke loose and came down the drill steel revealing a small nodule that also came loose from the roof and contacted the top of the employees left boot. No fractures found and the employee was treated for a bruise to EE's left foot.

Employee was installing a steel cable to help pull the belt, when gloved right hand was cut by a burr on the cable causing a laceration that required sutures.

There was an unplanned roof fall in the #9 North Main, between 28+65 and 29+55 in the #2 entry.

Employee was attempting to remove a steering jack pin with a sledge hammer. During the process right index finger was caught between the handle of the hammer and the frame of the scoop causing a laceration. They continued to perform their normal duties until 8/13/25 when they were treated for a fracture and given 5 sutures for the laceration.

Employee was using a bolter wrench to tap the dust out of EE's drill steel. After striking the drill steel the employee struck self with the wrench in the mouth. It was determined on 8/11/25 that EE had a chipped tooth that would need repaired.

Employee had stopped to move a bundle of pin plates out of the travel road that had fell from a pallet of supplies. While picking up the plate with EE's gloved right hand, EE received a small scratch to EE's ring finger that later became infected. EE is being treated for the infection and was taken off work. First missed day was 8/1/25.

Employee was in the process of breaking slate that had fell on EE's drill steel when a small piece contacted EE's left knee causing EE to twist EE's left knee. An MRI revealed a tear to the left knee.

Employee was greasing the feeder when EE slid down a pile of material beside of the feeder causing EE to strain EE's back. EE continues to perform EE's normal duties but started physical therapy on 8/18/25.

Employee was pulling slack to hang a cable in front of the #1 unit sub when they strained their back. Employee continues to do their normal duties but started physical therapy on 8/13/25.

There was an unplanned roof fall in the #9 1st East Main, crosscut 16, between #3 and #4 entry.

Employee was installing wire mesh to the mine roof during the bolting process when a piece of slate fell between the mesh contacting the employees gloved hand that was resting on the tray resulting in a laceration that required stitches.

While adjusting the width of the forks on the #1 unit mini trac the employees finger was caught between the mast and the back of the fork removing the skin from the tip of right index finger.

Employee was attempting to unstop a water pump discharge 4" line. After leveling the line the employee was stepping away from the line when the coupling broke apart causing the water and other material from inside the discharge line to contact the right side of EE's face, neck, and back causing superficial cuts to the skin.

A roof fall was discovered on the 9-54 road way between xc 61 and 62 #3 entry

There was an unplanned roof fall in the #4 entry between crosscut 6 1/2 and 7 1/2 in the #9 1st South Corridor.

Employee was in the process of removing a broken conveyor shaft from the miner when their right gloved hand and right arm came in contact with the hot oil from the conveyor motor causing burns to both their hand and arm.

Employee was operating a roof bolter on #3 unit, while on EE's knees with EE's lower leg behind EE a piece of slate struck EE's left foot twisting it causing a sprain/strain. The employee finished EE's shift but was taken off work for EE's sprain the next day.

Employee was removing paint on the new 11-54 header so welding could be performed when a piece of material made it over EE's safety glasses and in EE's right eye. The material was removed from EE's eye and EE is off while the eye is healing.

The 630 counterweight steel rope was discovered to have a small area of distortion near the end closest to the counterweight. The hoist was removed from service while repairs were made.

Employee was reclaiming belt framing on the old 4C belt when the lo-trac EE was operating came in contact with a piece of hanging framing that shifted contacting the employees left arm causing a laceration that required sutures.

The employee twisted while lifting a block to be cut for the construction of a brattice while on knees, when employee felt pain in lower back.

Employee was walking to bolter and slipped on a wet rock that caused employee's knee to buckle under employee. Employee was treated for a strained knee. The employee became incarcerated on 2/13/25 for an undetermined amount of time.

Employee tripped and fell on a pallet with EE's right knee resulting in soreness and some bruising. Employee continues to work but started physical therapy on 2/20/2025.

Employee was walking and slipped on loose material that was on the mine floor and while attempting to catch self, employee over-extended right shoulder. Employee is being treated for a strained shoulder.

Employee was trimming belt with a knife when the knife slipped contacting the employees right forearm resulting in 5 sutures.

Employee was riding on the 5114 mantrip going to the working section when the mantrip contacted a steel square set leg, causing the mantrip to come to an abrupt stop and the employee to hit the interior on the ride with right shoulder causing a suspected sprain/strain.

Employee was walking through a mine curtain when their hard hat contacted a piece of slate that fell striking their left arm causing a laceration. The laceration required sutures.

Employee was switching the direction EE was facing in EE's shuttle car when EE felt a pain in EE's lower back. Employee continues to work but was scheduled physical therapy that started on 2/17/25.

Employee was in the process of moving the roof bolter when cable slack was picked up by the bolter operator engaging the reel, causing the cable to contact the injured employee in the mouth, resulting in sutures to employee's bottom lip.

Employee was operating a roof bolter when a nodule (2"x 8"x 4") that was unknown to the operator to be resting on the canopy slid off and struck the employee in the right hand resulting in a laceration that required sutures.

Employee was working beside of a belt header that was being installed when EE stepped back tripping over an unknown object causing EE's back to contact a timber resulting in a fractured rib.

There was an unplanned roof fall in Panel District 6, 4th South between 15+40 and 16+10 on the 3C roadway.

Employee was loading supplies on the roof bolter by hand when employee's right index finger was caught between the bundle of plates and the roof bolter. This resulted in a laceration that required 4 stitches in the tip of employee's right index finger.

Power was lost at the mine causing the Hanson hoist to stop on the way down with three employees inside. When power was restored on the hoist the employees were hoisted to the surface.

Employee was loading a toolbox on their ride when they twisted to place the toolbox causing a sharp pain in their lower back. Employee continued to work until 9/17/24 when they had surgery to repair their lower back.

Employee was positioned at the rear of the roof bolter, when a piece of slate fell from between the rows of installed bolts landing on the bolting machine and striking the employee in the lower back. There was no visible injuries, but the employee is complaining of back pain and is waiting on an MRI to be performed.

Employee was installing EE's middle bolt in the #4 right entry when a piece of slate fell from beside EE's ATRS and middle pin striking EE's canopy, falling to the mine floor, then contacting the employees left leg resulting in a fractured lower left leg.

Employee was attempting to place a cable bolt in the bolter chuck when the bent bolt spun/shifted causing employee to sprain left wrist.

Employee was cutting down damaged wire mesh on 2/54 belt when dust/rust fell off the wire and went in behind the employee's safety glasses resulting in a scratch to the left eye.

Employee was drilling in the #4 right entry when a piece of slate broke loose in front/ beside of the ATRS causing a piece to land beside of employee and fell over contacting employee's right leg resulting in a strained right knee.

Employee was walking beside of a metal skid that supplies are stored on when EE caught left arm on the corner of the skid resulting in a laceration that required stitches.

Employee was greasing a shuttle car when a piece of slate 4"x 8"x 1/4"-paper thin struck EE in the left arm resulting in a laceration that required stitches.

Power was lost at the mine causing the Hanson hoist to stop 1/3 of the way down with an employee inside. When power was restored the employee was hoisted to the surface.

Employee was in the process of cleaning the dust box on the roof bolter when a piece of coal rib popped off behind the employee striking the right calf, resulting in a fractured fibula.

As an employee was securing a chain and chain hanger with a rib pin, a pump cable that wasn't visible was pinched and resulted in a non-life threatening injury to the employee's left arm.

The employee was installing a screw head in a roof jack pipe. While sliding the head in the pipe, EE's right index finger was caught between the screw head and pipe resulting in a laceration that required stitches.

The employee was installing a transmission on a diesel ride. While aligning the transmission it shifted forward catching employee's right little finger between the transmission and the torque convertor resulting in a laceration to the tendon that required surgical repair.

Employee was in the process of installing a roof bolt, while lining the bolt into the drill chuck they inadvertently spun the chuck causing their gloved hand to be pulled resulting in a fracture to their left ring finger.

The employee was crossing a shuttle car cable near the anchor. As they stepped on the cable to cross, the car picked up cable slack from two entries away causing the cable to contact the bottom of the employee's boot resulting in a fracture to their left foot.

There was an unplanned roof fall in XC3 between 3D road and 3D belt entry in panel district 6. Both the XC and adjacent entries had standing support previously installed. Neither the roadway nor the belt entry was affected by the fall.

A record setting barometric pressure drop on 1/12/24 resulted in methane levels rising above 2 percent in a return air course. In an abundance of caution the mine was evacuated, and power was de-energized in the affected area while continuing to monitor the methane concentration.

Employee was lifting a shuttle car anchor to hang on the #6 unit when they felt a pain in their groin. They continued to work at full duty until 10/21/25 when it was determined that surgical repair would be needed for a diagnosed tear.

Barometric pressure drop on 1/9/24 resulted in methane levels rising above 2 percent in a return air course. As barometric pressure began to rise the methane levels reduced below 2 percent in the return air course. In an abundance of caution the mine was evacuated, and power was knocked while continuing to monitor.

Employee was struck by a piece of slate that resulted in a cut to EE's left ear requiring stitches.

The employee was using EE's ride and a strap to pull the needed miner slack on #7 road, EE placed the strap on the end of EE's boot that was in the operators compartment. Before the cable could be pulled the strap got under EE's rear tire causing the strap to be pulled towards the rear wheel causing EE's foot to be pulled out of the ride resulting in a sprained foot/ankle.

The employee was attempting to straighten the roof bolt in the #9 entry on the #5 unit when the bolt spun causing EE to strain left shoulder. EE is being treated for a strained left shoulder.

The employee was letting EE's cable down from the mine roof when EE felt a pain in EE's left shoulder. EE is being treated for a strain in EE's left shoulder.

While the employee was trimming the roof with the miner in 8 right a piece of metal struck EE in the upper left bicep that required 2 stitches on the initial visit. The metal was surgical removed on 12/08/2023.

There was an unplanned roof fall on 8-54 East crosscut 15(15+90) in the #4 entry.

Employee was unloading 12' cable bolts from bolting machine when EE felt pain in the middle of their back. EE was given restrictions that wouldn't allow EE to perform the job.

Employee was lifting a parts trailer tongue on the back of a diesel ride when the tongue slipped off the rear of the ride landing on the top of EE's right foot causing a fracture.

The Employee was traveling on 4-54 road at crosscut 5 when a roof bolt flipped up from the tire striking Employee in the forehead causing a laceration that required 5 stitches.

Employee was trying to get the belt back in the airlock skirts when EE's left index finger was smashed between the belt and the roller resulting in a fracture and a laceration that required stitches.

While advancing the bolter to the next row of bolts to be installed on #3 unit the employees left foot was ran over by the rear tire of the bolter resulting in a bruised foot. The employee wasn't able to return on 10/17/23 because of the bruising.

Employee was using a come-a-long to pull a gear from the miner on the Nebo yard. When the gear released from the miner the employees right arm stuck the concrete wall behind EE causing a laceration that required 5 stitches.

Employee tripped and fell striking head on the rear of the unit lo-trac resulting in 5 stitches to the forehead.

The employee was turning into crosscut 16 on the 10/48 road when a pin buried under some loose material came up and struck EE in the forehead resulting in a laceration that required 6 stitches.

The employee had lowered the bolter cable from the tie wire that had it held to the mine roof, when EE stepped forward the piece of wire EE had just released the cable from went between EE's glasses and hard hat causing a corneal abrasion to EE's right eye.

There was an unplanned roof fall on 3/54 road #9 1st South corridor between 2+50 and 5+00.

The employee was trying to remove a bent cable bolt from work area when they lost balance causing them to land on their right hand resulting in their right ring finger being jammed. The employee suffered a fracture in their right ring finger.

Employee was helping hang a miner cable and felt pain in EE's lower stomach. The employee was diagnosed with an umbilical hernia.

The employee was attempting to lift a High Voltage plug from the storage container when the employee felt a pain in lower back.

The employee was tying wire mesh to the rib when EE's hand slipped off of the wire causing EE to contact EE's right eye with EE's gloved thumb causing a corneal abrasion.

The employee had unhooked a hauler from a water tank, while walking back to the operators deck EE noticed EE's lunchbox wasn't in a good place so EE stepped next to the hauler tire with EE's left foot to reach for EE's lunchbox when the hauler moved causing the tire to contact EE's left foot resulting in a fractured big toe.

Employee was trying to free a drill steel from EE's bolter chuck when EE came in contact with the broken drill steel resulting in a cut to EE's right forearm that required stitches.

Employee was walking beside the #6 unit power center when EE tripped on a shuttle car cable landing on EE's right shoulder. Employee has been taken off from work until an MRI can determine the extent of EE's injury.

Employee was lifting a concrete block onto a pallet when EE twisted resulting in a strained back.

The employee was cutting a piece of mining belt with a knife, when the knife slipped off the belt causing the knife to puncture EE's left leg. The wound required 3 stitches.

A worn coupler was discovered on the Hanson hoist. This required the hoist to be down, and the coupler to be replaced.

Employee was walking towards the mantrip when EE tripped and fell on the mine floor, while catching self EE received a cut to EE's left thumb that required 5 stitches.

While tightening a ratchet strap the employee felt a pain in their right bicep. A torn bicep was confirmed and the employee was taken off from work.

The employee was swinging the boom of the roof bolter in when a piece of coal rib broke loose striking the employee in the upper back. The employee received a few scratches to their back and some soreness in their right rib area.

An employee was striking a fitting with a 2 lb sledge hammer when EE lost grip of the hammer causing the hammer to travel 6'-8' striking the injured employee in the right side of EE's face. The employee suffered a broken zygomatic arch on the right side of EE's face.

Employee was stepping onto a catwalk and lost EE's balance causing EE to fall on EE's right wrist. Employee's first missed day was 4/19/23 when EE was taken off from work.

Employee was riding behind the driver on a four man diesel when the drivers hat caught a piece of wire hanging from the mine roof that flipped back striking the employee in the upper lip resulting in a cut that required 5 stitches.

A roof fall occurred in the #11 third main east at crosscut 56 in entry #11. The fall is approximately 30' in one direction, and 60' in the other direction, and 8' thick.

While moving Belt/power employee lifted a miner cable in a higher than normal area, straining EE's left lower back. Employees first missed day was 3/10/23.

A historic drop in barometric pressure resulted in methane levels rising above 2 percent in a return air course. As barometric pressure began to rise methane levels dropped below 1 percent. In an abundance of caution the mine was evacuated, and power was knocked while continuing to monitor.

There was an unplanned roof fall on 5/54 roadway between the road and the belt entry at crosscut 24.

Employee was lifting a belt wiper blade and felt a sharp pain in the left side groin. First missed day was 3/1/23.

Employee was working on a piece of equipment when the hinged cover fell over pushing the injured employee into the metal bar the cover normally rest on resulting in a laceration that required fourteen stitches.

Employee had a loose cable hanger hook, to hook in EE's nose while examining the roadway resulting in 2 stitches.

There was an unplanned roof fall on the #9 1st West Corridor entry #6 (Roadway) between 41+25 and 42+05.

Employee was backing out of a crosscut when a wooden runner flipped up striking the employee's left hand smashing the middle and ring finger resulting in a minor fracture in the tip and stitches in the ring finger and stitches in middle finger.

Employee was walking through a curtain and tripped causing self to fall, resulting in a laceration on EE's fifth digit. The laceration required 5 stitches.

A roof fall occurred on the 4-54 road at crosscut 56 in the 8th South Panel #5 Entry. The fall is approximately 60' to 70' long, 20' wide and 7' to 8' high.

A ATRS pad had flipped over. While flipping the pad back over, employee's thumb was caught between the pad and T-bar resulting in a fractured thumb.

Employee was pulling roof jacks of the top of a refuge chamber. EE felt a pain in the abdomen resulting in a hernia.

Employee was putting boards under the feeder. There was a miscommunication between the feeder operator and the employee. The feeder was lowered instead of raised causing the employee's middle left finger to be caught between the boards. The employee's middle left finger was mashed between the boards.

Employee was moving a miner cable and felt a pop in shoulder. Employee starting missing work on 2/2/2023.

At approximately 8:30 a.m. there was an ignition of coal dust at the head of the left miner. There were no injuries or scorching as a result of the ignition.

Employee was cutting belt to make a splice. The belt knife EE was using cut through the belt and then cut EE's right thigh. The cut required 6 stitches.

A roof fall occurred in the 2nd East Parallel, #6 entry, spad 56+99. The fall is approximately 19 feet by 19 feet by 12 feet high.

Employee was examining the 2nd 54 belt line. They moved their upper body to avoid a water line that was hung from the roof when their back began to hurt. Employee continued to work until exiting the mine and going to the ER for medical care.

Employee was cutting open a box and knife and it slipped and cut EE's left index finger at base. The lacerations where required 5 stitches.

Employee was pinning in #7 entry while installing EE's first pin with wire mesh and a rock fell hitting EE on EE's right leg and body.

Employee was cutting a wear plate off of a continuous miner. As EE was cutting, the torch hit some dirt causing some slag to go into EE's eye.

Employee started to rotate pinner steel to the mine roof and a piece of rock fell striking employee above the left eye causing a laceration that required stitches.

Employee was busting a rock with a sledge hammer off the feeder, the rock busted in half and half of the rock smashed employee's right index between the rock and the hammer handle. The right index finger was fractured.

Employee was filling up lube ride with oil at the Hanson oil stations, while walking around the lube ride employee stepped on coal that had fallen off the coal rib, tried to catching self thus twisting left knee.

Employee was pulling a pin out of a tailor tongue, the pin had a bend in it, employee started pulling hard on it and the pin came out striking employee in the mouth. Employee's mouth was cut and receiving stitches.

Employee hit head on car canopy after employee ran over a rock in the travel way, employee had a small cut that required two staples.

Employee dropped a bundle of cable bolt plates on their hand, fracturing left middle finger.

Employee was tramming the roof bolter, when a piece of the mine floor came out from under the bolter tire and rolled the employee's right ankle. Employee has a sprained ankle.

The Wolf Hollow Man Hoist was down due to a damaged control wire.

Employee had swung the boom out to install a rib pin when a rock fell out of the mine roof and struck them in the back head. They received a cut to their head and the laceration required six staples.

Employee was backing the hauler out of a cross cut and had right hand resting on the side of the hauler. When employee steered the hauler, employee's little finger was caught between a guard and the steer stop that was mounted near the center section of the hauler. Employee finger had laceration that required stitches.

Employee had just finished installing last roof bolt, when a rock fell out of the mine roof. It fell on top of the canopy sliding down hitting employee in the center of back. Thus knocking employee down and sticking employee's forehead on pinner tray. This cut employee's forehead that required stitches.

Was lapping belt with channel locks and felt pain in upper right arm. Employee is waiting on an MRI to see what's wrong with the arm.

Employee was pre nailing boards on side of bolter. When EE swung the hammer, EE missed the nail and hit hand between the index finger & thumb. This cut EE's left hand and the laceration required switches.

Employee was helping change a planetary on a miner. Later on in the shift employee felt some pain in abdomen. Employee started missing work on 7/26/22 due a hernia repair.

Getting out of a ride when employee felt knee pop. Employee started missing work on 3-1-23 because of having surgery to repair the left knee.

Employee was loading supplies on the bolter and hit EE's elbow on the tray. EE was throwing a pin and hit EE's elbow on the head of a roof bolt of a roof bolt. EE's elbow began to swell on Monday. EE worked Tuesday while experiencing pain. EE has a small puncture wound on EE's left elbow.

Employee put EE's pinner steel in EE's tray after putting a pin up. When the pinner steel got caught by the boom on the way down and it hit employee in the mouth and chipped EE's teeth.

Employee was moving a toolbox on the golf cart and felt pain in upper arm near EE's right shoulder.

Employee was shoveling coal back on the belt. The belt shut off for about 4 minutes EE was shoveling the belt when it started back & the shovel spun around and hit EE in the month & chin with the handle this cut EE's chin & it required stitches.

Employee was tearing out belt framing to pull a tail piece in line for a piggy back header. A belt chain slipped causing belt framing to fall and pin employee's right wrist between the belt framing and the boom of the header. Employee's right wrist was dislocated.

Employee was resetting a jack by the tail piece and a rock fell out & hit EE on the right index finger. The finger was cut and required 8 stitches.

Employee was putting a timber under the cat of a miner that was hung up, employee was holding pressure on the timber and using cat to pull timber under the cat when the timber popped up smashing left thumb. Employee's left thumb got 8 stitches.

Employee was pinning in a header hole when a rock came out of the rib and struck employee on the back pushing employee into the canopy, cutting forehead.

Employee was pinning a turnout. It was a little dirty so they threw the pin out with the board and plate away from them. When they pulled the pin back, they felt a sharp pain in shoulder. Employee is being treated for a strained shoulder.

EE had repaired a leak in the suction hose on a co-worker's boom. EE got on the bolter so EE could listen for leaks around the cyclone, troubleshooting. EE told co-worker to drill a test hole so EE could see if there were any leaks. Co-worker swung the boom in pinning EE's foot between the L Arm and the bolter frame. Co-worker was on knees and could not see where EE was.

Employee picked up tool box and felt pain run down both legs.

Employee was pinning 9L and had just finished putting up cable bolts and was walking to the back of the bolter when EE slipped on slack and got left foot under tire of the bolter as it was moving back.

Employee was grinding on a shuttle car, the grinder kicked back, causing a cut on EE's left thumb. Employee started missing 4/29/22 do it an infection.

A suspected ignition on the #3 unit occurred on 04/11/2022. After an investigation of federal inspectors, state inspectors and company officials there was no evidence to support an ignition had occurred.

Employee was installing bottom roller behind take up, they hung one side and went to hang other side when pushing up to hang they heard & felt pop in right shoulder. Employee started missing work on 7/5/2022.

The Hanson Man Hoist was down due to a damaged strain the hoist rope.

Employee was spotting a pin in the mine roof when a rock fell out between the ATRS and the pin EE was installing. The rock hit EE on the lower left leg.

Employee was installing EE's inside pin & glue to the mine roof. EE's right knee twisted on a pile of loose material.

Employee was driving a scoop on the 3rd North supply road when a rock fell out of the mine roof striking EE on the head & right shoulder.

There was an unplanned roof fall in the 2nd East Mains in the #9 Entry at spade 60+97.
