A non-injury roof fall occurred in the #4 right crosscut and extened to #6 entry of the last open crosscut on 001 mmu for approximately 100 feet long and 5 feet high. Survey station 5128 located in #2 entry of the last open crosscut.

Found Roof Fall on weekly bleeder examination that was blocking the bleeder. One break inby spad number 5053, Fall was 175 feet long, 20 feet wide, 9 feet high.

Belts had stop running started to investigate why belts were off and found Rock Fall on #1 belt 75 feet underground from surface. Fall was 22 feet long 18 feet wide and 15 feet hight.

Placing beams against mine roof with jacks, when one of the jacks kicked out, throwing the beam sideways and striking EE. He had to have 6 stitches for a laceration above his left eye.

He turned to tram the roof bolter forward, twisted back. He felt something pop in the middle of his back. Also had pain in neck area.

Employee was traveling from #3 belt line to #4 head drive on a Johnson Industries 2 man carrier, when a piece of rock fell from in-between the roof bolts hitting on the Johnson carrier. The employee had his right hand on the steering wheel and the rock struck his right hand and left leg. The rock measured 48" long 24" wide and 5" thick.

Employee was operating the left side of a dual boom bolter when he got his left hand entangled with the drill steel and a wrench. This resulted in an amputation of his left hand and arm near his elbow. He was treated on site and then taken by ambulance to a local hospital. He had surgery and his arm was reattached. He was released from the hospital on 4-4-08.

Employee was operating the continuous miner, when a piece of draw rock measuring 2 feet by 4 feet by 2 inches fell, strking him on his back and hips. Employee has no broken bones but is loosing time.

A potential inundation of water occurred as the miner cut into a bleeder area of the mine to establish additional bleeder entries. Water came on to the section and after a while created a possible inundation.

Non EE received injuries that proved to be fatal after he wrecked an ATV on a section of road that is not a part of the mine. ICG Knott County does not believe submission of this report is required by the Mine Act or regulations and is submitting it under protest.

On May 5, 2007 at 10:45 am a fire occurred involving the mine office, employee change room and a supply trailer. All buildings were located on the surface at the mine site. All three buildings were destroyed and there were no injuries.

Employee was operating one side of a dual boom bolter when a piece of draw slate measuring 2 inches by 2 feet by 3 feet fell striking him on the left arm and back. He continued to work until 08/21/06 at which time he was taken off work due to the ongoing pain in his back.
